Summary: This study retrospectively detected the risk factors for post-ERCP cholangitis in patients with hepatic alveolar echinococcosis, finding that hilar bile duct stenosis and endoscopic retrograde biliary drainage were risk factors for cholangitis, and stent diameter and number of stent implantation may influence the incidence rate of post-ERCP cholangitis. Sample size and clinical heterogeneity are two insurmountable difficulties, and a larger sample size is needed to verify the risk factors for screening.

Summary: This study compared the use of SBS and PSIS in the treatment of unresectable hilar malignant biliary stricture. The results showed no significant differences in clinical success rate, adverse event rate, time to recurrent biliary obstruction, or overall survival between the SBS and PSIS groups, except for the significantly longer procedure time in the PSIS group.

Summary: In this study, the functional success rate and TRBO did not show a significant difference between unilateral and bilateral biliary drainage. However, the unilateral group required more additional drainage, and the incidence of early adverse events tended to be higher in the bilateral SEMS group.

Summary: This study compared the efficacy of combined ERCP and EUS-BD with bilateral PTBD in patients with MHBO, and found that the former had significantly lower RBO rates at 3 and 6 months. Additionally, the former also had fewer reinterventions and longer time to RBO occurrence.

Summary: This study retrospectively assessed the feasibility, efficacy, and safety of EB-RFA for malignant hilar obstruction (MHO). The results showed that SEMS placement after EB-RFA in MHO was not associated with improvement in the stent patency and patient survival. Further prospective randomized studies are necessary to establish the effectiveness of EB-RFA with stents in MHO.

Summary: Plastic endobiliary stents may migrate into the bowel after endoscopic retrograde cholangiopancreatography, potentially causing bowel perforation. It is crucial for endoscopists to be vigilant about this risk and consider early intervention when necessary.

Summary: This study aimed to investigate the feasibility and safety of endoluminal RFA for occluded bilateral hilar metal stents due to tumor ingrowth in patients with malignant hilar bile duct obstruction. The results showed that RFA was successful in all patients, with a clinical success rate of 72.7%. Stent dysfunction post-RFA was managed successfully with ERCP, and the overall survival median was 289 days. Endoluminal RFA seemed to be a safe and useful rescue therapy for selected patients.

Summary: The study found that the use of new anti-reflux plastic stents (ARPS) compared to traditional plastic stents (TPS) can reduce the risk of stent-related cholangitis and prolong stent patency for patients with biliary strictures. There were no significant differences in clinical success rates and other adverse events between the two groups.

Summary: This study compared the outcomes of needle-knife precut (PS-NKP) and wire-guided cannulation (PS-WGC) after pancreatic stent insertion in biliary interventions. The results showed that the success rate of cannulation was higher and the incidence of complications was lower in the PS-NKP group compared with the PS-WGC group.

Summary: Palliative EBS should be considered carefully for elderly patients with choledocholithiasis who have poor prognosis, as it has a shorter cholangitis-free period and higher retreatment rate compared to complete stone removal. Although there is no significant difference in mortality rate and procedure-related adverse events between the two groups, cholangitis in the EBS group is mainly caused by stent migration.
