Hepatitis B virus (HBV) infection has shown an intermediate or high endemicity level in low-income countries over the last five decades. In recent years, however, the incidence of acute hepatitis B and the prevalence of hepatitis B surface antigen chronic carriers have decreased in several countries because of the HBV universal vaccination programs started in the nineties. Some countries, however, are still unable to implement these programs, particularly in their hyperendemic rural areas. The diffusion of HBV infection is still wide in several low-income countries where the prevention, management and treatment of HBV infection are a heavy burden for the governments and healthcare authorities. Of note, the information on the HBV epidemiology is scanty in numerous eastern European and Latin-American countries. The studies on molecular epidemiology performed in some countries provide an important contribution for a more comprehensive knowledge of HBV epidemiology, and phylogenetic studies provide information on the impact of recent and older migratory flows.

Robotic transabdominal retromuscular rectus diastasis (r-TARRD) repair: a new approach

D. Cuccurullo, L. Guerriero, G. Mazzoni, C. Sagnelli, E. Tartaglia

Summary: This study presents an innovative robotic approach for the treatment of rectus diastasis with concurrent primary or incisional ventral hernias. The results demonstrate that the operative time and hospital stay were relatively long, while complications and recurrence rate were low.

SARS-CoV-2 infection: a hurricane that does not ignore chronic hepatitis

Caterina Sagnelli, Margherita Macera, Clarissa Camaioni, Annabella Salvati, Nicola Coppola, Evangelista Sagnelli

Summary: The COVID-19 pandemic has had a detrimental impact on screening and treatment for chronic hepatitis patients, as well as liver transplantation. It has also caused delays or suspensions in HBV infection screening and treatment, as well as universal hepatitis B vaccination programs. The WHO's goal of eliminating HCV infection as a global public-health issue by 2030 has become unattainable due to the pandemic.

Physical activity in elderly kidney transplant patients with multiple renal arteries

Armando Calogero, Caterina Sagnelli, Gaia Peluso, Antonello Sica, Maria Candida, Silvia Campanile, Gianluca Minieri, Paola Incollingo, Massimiliano Creta, Luigi Pelosio, Vincenzo Tammaro, Alessandro Scotti, Akbar Jamshidi, Marcello Caggiano, Evangelista Sagnelli, Concetta A. Dodaro, Nicola Carlomagno, Michele Santangelo

Summary: The study found that kidneys with vascular abnormalities can be successfully transplanted, with similar rates of surgical complications and 5-year survival rates in both the SRA and MRA groups. Individualized physical activity programs also yielded comparable excellent results in improving physical capacities, arterial pressure, lipid metabolism, insulin sensitivity, quality of life, and physical and mental status for both groups.

Mastroianni, Massimo Andreoni, Laura Monno, Davide Guarneri, Nicola Coppola, Andrea Crisanti, Massimo Galli, Gianguglielmo Zehender

Summary: This study characterized new SARS-CoV-2 genomes sampled in Italy and investigated the origin and evolutionary dynamics of the virus in Italy and Europe. The findings suggest that China and Italy played significant roles in the widespread transmission of the virus in Europe during the early phase of the pandemic.

Summary: Patients with oligometastatic urothelial carcinoma represent a challenging subset to manage. Metastasis-directed radiation therapy has the potential to facilitate local disease control and overall survival, but definitive recommendations cannot be provided due to the lack of high-quality trials.

Prevention of HBV Reactivation in Hemato-Oncologic Setting during COVID-19

Caterina Sagnelli, Antonello Sica, Massimiliano Creta, Alessandra Borsetti, Massimo Ciccozzi, Evangelista Sagnelli

Summary: Onco-hematologic patients are highly susceptible to SARS-CoV-2 infection and may experience HBV reactivation during the COVID-19 pandemic. Vaccination and adherence to social distancing measures are crucial for these patients, and healthcare facilities should be adequately equipped and staffed.

Outcomes of Thoracoscopic Lobectomy after Recent COVID-19 Infection

Beatrice Leonardi, Caterina Sagnelli, Giovanni Natale, Francesco Leone, Antonio Noro, Giorgia Opromolla, Damiano Capaccio, Francesco Ferrigno, Giovanni Vicidomini, Gaetana Messina, Rosa Maria Di Crescenzo, Antonello Sica, Alfonso Fiorelli

Summary: This study evaluated the impact of recent COVID-19 infection on the outcomes of patients undergoing thoracoscopic lobectomy for lung cancer. The results showed that COVID-19 infection did not increase postoperative complications or 90-day mortality. However, patients with recent COVID-19 infection had a higher incidence of pleural adhesions. Therefore, the treatment of lung cancer patients with recent COVID-19 infection should not be delayed and should not differ from that of the general population.

Multidisciplinary Approach to the Diagnosis and Therapy of Mycosis Fungoides

Paola Vitiello, Caterina Sagnelli, Andrea Ronchi, Renato Franco, Stefano Caccavale, Maria Mottola, Francesco Pastore, Giuseppe Argenziano, Massimiliano Creta, Armando Calogero, Alfonso Fiorelli, Beniamino Casale, Antonello Sica

Summary: Mycosis fungoides is a common primary cutaneous T-cell lymphoma characterized by skin-homing CD4+ T cells. It has an indolent course and low-grade malignancy. Diagnosis is difficult due to the absence of specific features and lesional polymorphism. Treatment requires staging and a multidisciplinary team approach is needed for advanced stage diseases.

Alveolar-Arterial Gradient Is an Early Marker to Predict Severe Pneumonia in COVID-19 Patients

Giuseppe Pipitone, Marta Camici, Guido Granata, Adriana Sanfilippo, Francesco Di Lorenzo, Calogero Buscemi, Antonio Ficalora, Daria Spicola, Claudia Imburgia, Ilenia Alongi, Francesco Onorato, Caterina Sagnelli, Chiara Iaria

Summary: The aim of this study is to evaluate the alveolar-arterial oxygen gradient (D(A-a)O-2) as a marker for predicting severe pneumonia in COVID-19 patients, in comparison to the PaO2/FiO(2). The results show that D(A-a)O-2 is more appropriate than PaO2/FiO(2) to identify COVID-19 patients at risk of developing severe pneumonia early.

Summary: Epithelioid hemangioma is a rare reactive vasoproliferative disease characterized by painless vascular nodules in the dermal and subcutaneous tissues of the head and neck. Diagnosis can be challenging, as the main symptom is progressively tender swelling next to a vessel course. There have been few reported cases of epithelioid hemangioma localized to the nose. This article presents a case of a 47-year-old woman with a nose lesion, discussing its diagnosis and treatment.
