In the past, prostate cancer (PC) could only be detected clinically, and delayed diagnosis of locally advanced or metastatic disease at presentation was common. Prostate-specific antigen testing and magnetic resonance imaging led to PC detection in a much earlier stage. However, controversy about the best treatment for locally advanced PC remains. Recent refinements in surgery and radiation therapy have improved outcomes, but no comparative study has yet conclusively determined superiority of one option over the other. In this review, we present the most recent evidence about the role of radical prostatectomy for locally advanced PC treatment from a surgeon's perspective. (C) 2015 Elsevier Inc.

Robotic parastomal hernia repair

Marcos Tobias-Machado, Daniel Coser Gomes, Eliney Ferreira Faria, Hamilton de Campos Zampolli

Summary: This study demonstrates the step-by-step robotic approach for parastomal hernia repair, showing favorable outcomes with shorter operative time, minimal blood loss, and no recurrence. The robotic platform proves to be a powerful ally in complex sutures and intracorporeal dissection, suggesting superiority over conventional laparoscopy.

Penile Cancer-Derived Cells Molecularly Characterized as Models to Guide Targeted Therapies

Hellen Kuasne, Luisa Matos do Canto, Mads Malik Aagaard, Juan Jose Moyano Munoz, Camille De Jamblinne, Fabio Albuquerque Marchi, Cristovam Scapulatempo-Neto, Eliney Ferreira Faria, Ademar Lopes, Sebastien Carreno, Silvia Regina Rogatto

Summary: In this study, five human primary penile cancer-derived cell cultures were established, including two epithelial and three cancer-associated fibroblast (CAF) cells. Epithelial penile cancer-derived cells showed good response to cisplatin and CAF signature markers were identified in CAF cells, indicating their suitability for penile cancer microenvironment studies.

Role of salvage lymph node dissection in patients previously treated for prostate cancer: systematic review

Joao Paulo Pretti Fantin, Maria Claudia Bicudo Furst, Marcos Tobias-Machado, Roberto Lodeiro Muller, Roberto Dias Machado, Alexandre Cesar Santos, Wesley Justino Magnabosco, Cinthia Alcantara-Quispe, Eliney Ferreira Faria

Summary: Salvage lymphadenectomy is a promising approach for lymph node metastases in prostate cancer patients, showing potential to delay systemic therapy despite not significantly increasing biochemical recurrence rates in all studies. However, long-term impact on overall survival and quality of life, as well as optimal case selection strategies, still need further investigation.

Summary: In a prostate cancer screening program in Brazil, distance from a referral center was associated with decreased compliance for biopsy among men living farther away but not among those living within 150 km. Distance did not affect the risk or grade of prostate cancer in this cohort.

What is the impact of dynamic contrast-enhancement sequence in the Vesical Imaging, Reporting and Data System (VI-RADS)? A subgroup analysis

Thaisa Gvozdenovic Medina Bricio, Gabriel Lion Gouvea, Rafael Vasconcelos Barros, Fernando Chahud, Jorge Elias, Rodolfo B. Reis, Valdair F. Muglia

Summary: This study evaluated the accuracy and reproducibility of the Vesical Imaging - Radiological and Data System (VI-RADS) score for assessing the risk of muscle layer invasion in bladder cancer. The results showed that the VI-RADS system had high accuracy in demonstrating muscle-invasive bladder cancer, regardless of the use of a dynamic-contrast enhancement (DCE) sequence. However, there was a significant difference between readers in the assessment of small lesions using the biparametric analysis.

Summary: Robotic-assisted radical prostatectomy may offer better functional outcomes than open surgery, but some surgical steps are not adequately described, potentially impacting final results. An expert panel in Brazil developed best practices for RARP through various steps, with majority of questions reaching consensus but some remaining controversial.
