Influence of antidepressants on glycaemic control in patients with diabetes mellitus

Purpose Anecdotal evidence suggests that antidepressants (ADS) may complicate glycaemic control. The objective of this longitudinal study was to investigate the influence of ADS on glycaemic control within diabetes patients. Methods From the pharmacy registry database PHARMO, we selected insulin users who did not use oral antidiabetics. The study population comprised: 133 patients with at least 12 months insulin use before and 6 months during an AD episode, including 16 patient., with an additional 6 months of insulin use after the AD episode; 110 patients with 24 months insulin use without an AD episode. Glycaemic control was measured as the amount of insulin used, which was calculated intra-individually in 3-month periods. We stratified for selective serotonin reuptake inhibitors (SSRIs) and tricyclic antidepressants (TCAs). Results Mean age (s.d.) of the subjects was 53.9 (19) years; 46.9% were men. Overall, the amount of insulin used did not change during or after AD use. No-AD users showed an increase of 16% in amount of insulin used over a period of 2 years (p < 0.001). SSRI users showed a decrease of 13% in amount of insulin used during the AD episode (p = 0.029), while no change was seen in TCA users. Notable was the large intra- and interindividual variation in amount of insulin used across all groups. Conclusions Overall, AD use did not influence glycaemic control in diabetes patients, The tendency for a difference between SSRIs and TCAs is suggestive for a pharmacologic effect of ADS rather than a general effect of depression on glycaemic control. Safety of off-label dose reduction of non-vitamin K antagonist oral anticoagulants in patients with atrial fibrillation

Summary: This study investigates the effects of off-label non-vitamin K oral anticoagulant (NOAC) dose reduction compared with on-label standard dosing in atrial fibrillation (AF) patients. The results show that there is no significant difference in the risk of ischemic stroke and major bleeding between patients receiving off-label reduced NOAC dose and those receiving on-label standard dose, while the risk of non-major bleeding is lower and the risk of mortality is higher in the off-label group.

Carotid Artery Stenosis and Progression of Hemispheric Brain Atrophy: The SMART-MR Study

Summary: Pharmacogenomic testing is used to prevent adverse drug reactions, particularly in statin treatment. This study aimed to investigate the usefulness of pre-emptive pharmacogenomic screening in primary care, focusing on the SLCO1B1 c.521T>C genotype as a risk factor for adverse drug reactions. However, the study did not find an association between the SLCO1B1 c.521T>C genotype and changes in statin therapy in primary care. Prospective data collection is needed to better evaluate the predictive value of the SLCO1B1 c.521T>C genotype on adverse drug reactions.

The recommendations provide practical and scalable approaches for effective screening and prevention of cardiometabolic disease following HDP in LMICs. Monitoring and evaluation of implementation of these recommendations provide opportunities for reducing the escalating burden of noncommunicable diseases in LMICs.
