Mechanical allodynia (other pain) is a painful sensation caused by innocuous stimuli like light touch. Unlike inflammatory hyperalgesia that has a protective role, allodynia has no obvious biological utility. Allodynia is associated with nerve damage in conditions such as diabetes, and is likely to become an increasing clinical problem. Unfortunately, the mechanistic basis of this enhanced sensitivity is incompletely understood. In this review, we describe evidence for the involvement of candidate mechanosensitive channels such as Piezo2 and their role in allodynia, as well as the peripheral and central nervous system mechanisms that have also been implicated in this form of pain. Specific treatments that block allodynia could be very useful if the cell and molecular basis of the condition could be determined. There are many potential mechanisms underlying this condition ranging from alterations in mechanotransduction and sensory neuron excitability to the actions of inflammatory mediators and wiring changes in the CNS. As with other pain conditions, it is likely that the range of redundant mechanisms that cause allodynia will make therapeutic intervention problematic.

Summary: Patients with osteoarthritis and a likely neuropathic pain component may represent a specific phenotype where joint damage is not the main cause of pain and disability. Those with a higher painDETECT score had less radiographic joint damage but impaired physical function compared to those with a lower painDETECT score. Severe pain was also found in joints other than the index knee, indicating the need for targeted treatment approaches.

Human IAPP is a contributor to painful diabetic peripheral neuropathy

Summary: This study found that human islet amyloid polypeptide (hIAPP) may be involved in peripheral neuropathy associated with type 2 diabetes mellitus (T2DM). In vitro experiments and mouse models showed that hIAPP aggregates can reduce neurite outgrowth and increase reactive oxygen species in sensory neurons, leading to neuropathy. The presence of more hIAPP aggregates in the skin of T2DM patients further supports the role of hIAPP in T2DM neuropathy.

NLRP3 inflammasome activation in sensory neurons promotes chronic inflammatory and osteoarthritis pain

Summary: Pain is a debilitating symptom in rheumatic diseases, and can persist even after total knee replacement or in the absence of inflammation. Mitochondrial dysfunction and NLRP3 inflammasome activation have been found to promote chronic pain. This study investigated the role of mitochondrial dysfunction and NLRP3 inflammasome activation in the transition from acute to persistent inflammation-induced nociplastic pain and in persistent monoiodoacetate-induced osteoarthritis pain. The results suggest that targeting the NLRP3 inflammasome pathway may be a promising approach to treat chronic pain.
