Surgeon, staff, and patient radiation exposure in minimally invasive transforaminal lumbar interbody fusion: impact of 3D fluoroscopy-based navigation partially replacing conventional fluoroscopy: study protocol for a randomized controlled trial

Background: Some symptomatic degenerative conditions of the lumbar spine may be treated with spinal fusion if conservative treatment has failed. The minimally invasive technique of transforaminal lumbar interbody fusion (MIS TLIF) is increasingly used but has been found to generate increased radiation exposure to the patient and staff. Modern three-dimensional (3D) C-arm devices are capable of providing conventional two-dimensional fluoroscopic images (x-rays) as well as 3D image sets for intraoperative navigation. This study was designed to compare the radiation exposure between these two intraoperative imaging techniques in MIS TLIF procedures. Methods: This study is a randomized controlled trial. Forty participants scheduled to undergo monosegmental MIS TLIF will be recruited and randomly allocated to one of two groups with respect to the applied intraoperative imaging technique: conventional fluoroscopy (FLUORO group) and 3D fluoroscopy-based navigation combined with conventional fluoroscopy (NAV group). Furthermore, patients scheduled to undergo bisegmental MIS TLIF during the recruitment period for monosegmental MIS TLIF will be assessed for eligibility and will be randomly assigned separately. The primary endpoint is the radiation exposure to the surgeon and is measured by dosimeter readings. Secondary endpoints are the radiation exposure to the assistant surgeon, scrub nurse, anesthetist, patient, and C-arm as well as radiation exposure in relation to the body mass index of the patient. Discussion: Results of this randomized study will help to compare the radiation exposure to the operating staff and patient during MIS TLIF procedures using conventional fluoroscopy versus 3D fluoroscopy based navigation combined with conventional fluoroscopy. Furthermore, recommendations regarding the appropriate use of the investigated intraoperative imaging techniques will be made to improve radiation protection and to reduce radiation exposure.

Summary: Hippocampal sclerosis with gray-white matter blurring in the anterior temporal lobe is associated with prolonged T1 relaxation and axonal loss. Patients without GMB in the temporal poles showed less reduction in axonal fraction on imaging compared to normal controls. Contralateral values did not differ significantly between patients and normal controls, but reduced axonal density and diameter were confirmed in the GMB-affected temporopolar white matter compared to temporal poles without GMB.

The Impact of Implementing a Radiation-Sparing Protocol for Percutaneous Kyphoplasty-A Prospective Dosemetric Study

Summary: This prospective study evaluated a protocol for radiation-sparing kyphoplasty by assessing dosemetrically recorded radiation exposures to both patient and surgeon. The results showed that the implementation of a strict intraoperative radiation protection protocol allows for safely performed kyphoplasty with ultra-low radiation exposure for the patient and surgeon without exceeding the annual occupational dose limits.

Mastication after craniotomy: pilot assessment of postoperative oral health-related quality of life

Summary: Patients undergoing neurosurgical cranial approaches often experience postoperative pain, reduced mouth opening, and masticatory problems due to manipulation of the temporal muscle. A study using the Oral Health Impact Profile (OHIP14) questionnaire found that postoperative oral health-related quality of life scores increase significantly after 3 months, decrease after 15 months, and do not return to preoperative levels. Postoperative impairment in mouth opening and masticatory pain can cause feedback from patients and their dentists.

The challenge of measuring spinopelvic parameters: inter-rater reliability before and after minimally invasive lumbar spondylodesis

Summary: The manual measurement technique of spinal sagittal alignment on X-rays is susceptible to rater-dependent variability. This study found that different raters had different significance levels when evaluating the pre- to postoperative change of lumbar lordosis in patients receiving lumbar spondylodesis. Therefore, it is important to describe the evaluation process and validate the consistency of results through multiple raters in every publication.

A Retrospective Evaluation of the Retrosigmoidal Approach for Petroclival Meningioma Surgery and Prognostic Factors Affecting Clinical Outcome

Summary: This study evaluated the prognostic factors influencing the progression-free survival rates of patients with petroclival meningioma. The extent of surgical resection and postoperative radiotherapy were found to be significantly associated with improved PFS. In cases of extended petroclival meningioma with infiltration of the cavernous sinus, near total resection and additional radiotherapy also showed positive effects. Surgical resection remains the most important prognostic factor, but neurological morbidity should be considered.
