Summary: This study investigated the characteristics of lower respiratory tract in Alpha-1-antitrypsin deficient (AATD) individuals prior to the onset of clinically significant chronic obstructive pulmonary disease (COPD). The results showed that mild inflammation was present in the lower respiratory tract and airways of AATD individuals despite having normal lung function. Additionally, a declining trend in lung function was correlated with the pro-inflammatory phenotype of their lower respiratory tract. Thus, early anti-inflammatory therapies may be a potential strategy to prevent progression of lung disease in AATD individuals.

Summary: A retrospective evaluation of 162 pediatric patients' abdominal MRI examinations revealed a positive correlation between pancreatic thickness and age, height, weight, and BMI, with no significant difference based on gender. The defined normal pancreatic thickness values are mostly consistent with previously published US and CT based values.

Summary: This study examined the relationship between spirometry parameters and BMI among healthy Sri Lankan school children aged 5-7 years. The results showed that lower lung function parameters (FVC and FEV1) were associated with thinness among children without apparent lung disease. The study also found that family income had the greatest effect on lung functions, with low-income families having smaller FVC and FEV1. This suggests that appropriate nutritional intervention may play a role in improving respiratory health in children.

Summary: This study aimed to establish normal strain values and Z scores for two commonly used strain analysis platforms (Philips QLAB 10.8 and TomTec AutoSTRAIN) and compare them with a previous version (QLAB 10.5) using a large dataset of healthy children. The study found that strain values vary with age and there are differences among the analyzed platforms. These findings are important for the standardization of pediatric echocardiography.

Summary: This narrative review discusses available data on lung function in children with primary ciliary dyskinesia (PCD), focusing on risk factors for lung function impairment. Recent studies have shown that the majority of PCD children have normal lung function, although some literature describes a pulmonary impairment. In addition to spirometry, the Lung Clearance Index has been used to detect peripheral airway disease, and it may have a role in early assessment of mild lung disease. Further studies are needed to analyze lung function prospectively from childhood into adulthood and evaluate the impact of PCD clinical phenotype, ultrastructural ciliary defect, or genetic background on lung function trajectories.

Summary: Longitudinal studies have shown that altered airway function shortly after birth is a risk factor for wheezing illnesses and asthma later in life. Factors such as extreme prematurity and cigarette smoke can continue to affect airway function and increase the risks of wheeze and asthma. Other factors, including immune system responses, respiratory viruses, airway microbiome, genetics, and epigenetics, can indirectly influence early airway function and asthma risk. Few interventions have been proven to alter the risk of developing asthma, but vitamin C supplementation during pregnancy may help mitigate the effects of in utero smoke exposure on lung function. The size of the airways and the factors influencing it play a crucial role in determining the lifelong risk of asthma. There is a need for progress in asthma prevention through well-designed interventions in longitudinal birth cohorts with early airway function assessments.
