Growth and body composition in very young SGA children

Infants with a very low birth weight are at risk of a reduced number of nephrons predisposing to kidney disorder, hypertension, and metabolic syndrome. Approximately 3% of infants are born small for gestational age (SGA), defined as birth weight and/or length at least 2 SD below the mean for gestational age (GA), independently of whether these children are born prematurely or at term. About 10% of these children do not show postnatal catch-up growth and remain of short stature during childhood. Most of these infants are not growth hormone (GH)-deficient, but may have GH resistance. Although GH-resistant, the majority of patients benefit from GH therapy, normalize height during childhood, maintain a normal growth velocity during puberty, and attain a normal adult height. To date, GH has been shown to be safe and no significant adverse effects have been demonstrated. Children with congenital chronic kidney disease (CKD) are born with subnormal birth weight and length and about 25% are born SGA. Shortness and need for GH treatment is highly correlated with weight at birth and gestational age. Primary renal disorders modify the response to GH treatment. Analysis of whether SGA is an additional risk factor for CKD regarding the development of hypertension, metabolic syndrome and cardiovascular complications is required.

Summary: This study aimed to investigate the long-lasting effects of maternal malnutrition during gestation/lactation on inflammation, lipid metabolism, and endocannabinoid signaling in the adult offspring hypothalamus, as well as the role of hypothalamic astrocytes in these changes. The results showed that maternal malnutrition caused sex-specific alterations in hypothalamic endocannabinoid signaling and lipid metabolism. Additionally, hypothalamic astrocytes were found to play a possible role in this process.

Summary: The central actions of leptin and insulin on hepatic lipid metabolism may be opposing, with the GH axis playing an important role in these effects. Leptin treatment led to an increase in hepatic lipolysis and a decrease in serum NEFA and triglyceride levels, while insulin treatment normalized these levels through activation of the GH axis.

Precocious sexual maturation: Unravelling the mechanisms of pubertal onset through clinical observations

Summary: Puberty is a crucial biological process involving sexual and somatic maturation, regulated by genetic, endogenous, and environmental factors. Recent research has revealed the essential roles of kisspeptins and neurokinin B in modulating the signaling pathways of pubertal maturation, as well as the effects of melanocortins and metabolic sensors. Precocious puberty, primarily affecting females, can be caused by alterations in the central nervous system and genetic mutations, such as those in the MKRN3 gene. Understanding the genotype-phenotype relationship in patients with precocious puberty is important for further research.

The Congenital and Acquired Mechanisms Implicated in the Etiology of Central Precocious Puberty

Summary: The etiology of central precocious puberty (CPP) is diverse, including congenital and acquired causes, with associations to structural and functional brain alterations. All causes of CPP lead to premature secretion of hypothalamic GnRH, resulting in premature reactivation of the hypothalamic-pituitary-gonadal axis. Excitatory factors and inhibitory factors play important roles in the mechanisms of CPP, which are delicately balanced. Hypothalamic hamartoma is the most well-known congenital cause of CPP, and genetic and epigenetic factors have been found to be influential in the etiology of CPP.

Summary: Excess body weight is often associated with low-grade inflammation, and evidence suggests a relationship between obesity and various diseases, including cancer and diabetes. Leptin, a hormone mainly produced by adipose tissue, plays a role in energy metabolism, neuroprotection, and regulation of peripheral tissues. Resistance to leptin can be related to neurological disorders and compromise its actions. This review discusses recent advances in understanding the molecular mechanisms of leptin and its potential implications for disease treatment.

Molecular basis of normal and pathological puberty: from basic mechanisms to clinical implications

Summary: Puberty, a major maturational event, is determined by genetic factors and influenced by endogenous and environmental cues. Research progress has been made in understanding the neuroendocrine networks governing puberty, but the precise timing and mechanisms of puberty and its alterations are still being deciphered. This review provides a comprehensive overview of the physiological basis of puberty, including the roles of kisspeptins and other central transmitters, the underlying molecular and endocrine mechanisms, and the pathways involved in pubertal modulation. Molecular features of precocious and delayed puberty are also summarized. The review highlights the need for further research in this active field of endocrinology.

Endocrine Sequelae in 157 Pediatric Survivors of Hematopoietic Stem Cell Transplantation (HSCT)

Summary: More than half of children surviving HSCT will develop endocrinopathies, with obesity being the third most frequent endocrine disruption, mainly due to steroids. Lymphoblastic leukemia is the condition with the highest rate of endocrine abnormalities. Female sex, older age at HSCT, pubertal stage, allogenic transplant, radiotherapy, alkylating drugs, and graft-vs-host disease pose risk factors for endocrine disturbances.

Summary: Leptin can regulate lipid metabolism and reduce adipose tissue stores by inhibiting food intake and altering adipocyte sensitivity to insulin. This study investigated the effects of chronic central leptin administration on lipid metabolism and inflammatory- and insulin-signaling pathways in epididymal adipose tissue. Changes were observed in the expression of key enzymes involved in lipid metabolism, as well as insulin-signaling targets and inflammatory markers.
