Albuminuria and insulin resistance in children with biopsy proven non-alcoholic fatty liver disease

Insulin resistance may favor increased urinary albumin excretion (UAE), leading progressively to chronic kidney disease (CKD). A recent study on non-alcoholic fatty liver disease (NAFLD), a condition of insulin resistance, associated this disease with the incidence of CKD in patients with type 2 diabetes. The aim of our study was to determine whether there is an association between insulin resistance and kidney function, based on estimates of UAE and creatinine clearance in children with biopsy-proven NAFLD. Kidney function was assessed in 80 patients with NAFLD and 59 individuals of normal weight matched for age and sex. Insulin resistance was measured by means of the homeostatic model assessment-insulin resistance (HOMA-IR) and limited to NAFLD patients by using the whole-body insulin sensitivity index. The HOMA-IR was found to differ significantly between the two groups (2.69 +/- 1.7 vs. 1.05 +/- 0.45; p = 0.002), while UAE (9.02 +/- 5.8 vs. 8.0 +/- 4.3 mg/24 h; p = 0.9) and creatinine clearance (78 +/- 24 vs. 80 +/- 29 mg/min; p = 0.8) did not. We found a significant but weak inverse correlation between insulin sensitivity and creatinine clearance in NAFLD patients (r (s) = -0.25;p = 0.02). No difference was observed in kidney function between NAFLD children presenting with or without metabolic syndrome, low or normal HDL-cholesterol, and different degrees of histological liver damage (grade of steatosis a parts per thousand yen2, necro-inflammation, and fibrosis). Patients with hypertension had increased levels of UAE (p = 0.04). A longer exposure to insulin resistance may be required to cause the increase in urinary albumin excretion and to enable the detection of the effect of the accelerated atherogenic process most likely occurring in children with fatty liver disease. Longitudinal studies are needed to rule out any causative relationship between insulin resistance and urinary albumin excretion.

Circulating levels of PIN1 and glucose metabolism in young people with obesity

Summary: Impaired activity of PIN1 isomerase may be involved in disturbed glucose metabolism and neurotoxicity, neurodegeneration, and cognitive decline. This study found that PIN1 levels were not significantly increased in young individuals with obesity, but showed an inverse correlation with glucose levels. Additionally, higher levels of A beta 42 and PSEN1 were found in obese teenagers, correlated with body mass index and insulin resistance.

This study found that children with obesity have a higher amount of circulating extracellular vesicles, which results in adipogenic differentiation in mesenchymal stromal cells and a reduction in differentiation markers in mature osteoblasts. Regular physical exercise can partially restore the bone phenotype by decreasing the levels of circulating extracellular vesicles in obese children.

The Role of the GH/IGF1 Axis on the Development of MAFLD in Pediatric Patients with Obesity

Summary: The anomalies of the GH/IGF1 axis are associated with a higher prevalence of MAFLD and more rapid progression of the disease. In this study, it was found that patients with inflammation and ballooning had lower levels of GH and IGF1, which were also negatively correlated with fibrosis. Multivariate regression analysis showed that both GH and IGF1 values were negatively associated with fibrosis, suggesting their important role in MAFLD.

Summary: Adrenal insufficiency is a severe endocrine disorder characterized by insufficient secretion of glucocorticoid and/or mineralocorticoid due to impaired adrenal function or inadequate adrenal stimulation. This review focuses on rare genetic causes of primary adrenal insufficiency with isolated glucocorticoid or combined glucocorticoid and mineralocorticoid deficiencies, as well as rare syndromes of isolated mineralocorticoid deficiency. The diagnosis of these rare genetic forms of pediatric adrenal insufficiency is challenging due to their clinical heterogeneity and rarity, and this review aims to provide an overview of their genetic and clinical features, as well as diagnostic and therapeutic approaches.

Dietary Fatty Acids Contribute to Maintaining the Balance between Pro-Inflammatory and Anti-Inflammatory Responses during Pregnancy

Summary: In this study, the association between dietary fatty acids and pro- and anti-inflammatory molecules in pregnant women was investigated. Several associations were found, including those between adiponectin and specific fatty acids, endotoxin and specific fatty acids, MCP-1 and a specific fatty acid, and ICAM-1 and a specific fatty acid. Additionally, cytokines such as leptin were associated with maternal body weight, smoking habits, and gestational diabetes.

High 1-h glucose in youths with obesity as marker of prediabetes and cardiovascular risk

Summary: Testing 1-hour glucose concentration during oral glucose tolerance test can identify individuals at risk of type 2 diabetes. This study aimed to determine the diagnostic cutoffs of 1-hour glucose for impaired glucose tolerance in obese youths and evaluate their association with cardiovascular disease. High glucose concentration was associated with metabolic abnormalities and increased risk of cardiovascular disease.

From Metabolic Syndrome to Type 2 Diabetes in Youth

Summary: Type 2 diabetes develops within the context of metabolic syndrome and is influenced by a continuum of risk factors, including visceral obesity, high blood pressure, abnormal blood lipids, and impaired glucose control. This condition places a significant burden on beta-cell function during puberty, leading to irreversible deterioration and the onset of diabetes. This review aims to identify age-specific risk factors for type 2 diabetes in youth, summarize screening and diagnostic criteria, and explore potential treatment options. Visceral obesity and disrupted lipid metabolism are key factors in disease development. Genetic susceptibility to impaired beta-cell function in the presence of obesity and insulin resistance explains why some obese adolescents develop diabetes at a young age while others do not. Lifestyle interventions focusing on a healthy diet and physical activity remain the primary treatment approach for youth with type 2 diabetes. However, pharmacological management using common drugs, such as dipeptidyl peptidase-4 inhibitors and sodium-dependent glucose transport inhibitors, as well as new weight-reducing medications, like glucagon-like peptide 1 receptor agonists, could revolutionize the management of this disease. These medications have the potential to not only treat diabetes but also alleviate the burden of metabolic abnormalities associated with the syndrome and lead to significant weight loss.

A simulation model estimates lifetime health and economic outcomes of screening prediabetes using the 1-h plasma glucose

Summary: This study found that using 1-hour post-load plasma glucose (1-hPG) instead of 2-hour plasma glucose (2-hPG) for screening and assessing the risk of diabetes is more cost-effective in the long term. Screening with 1-hPG can increase disease-free years, delay the onset of diabetes, reduce the incidence of complications, and increase the number of quality-adjusted life years (QALY) gained.
