One-year glargine treatment can improve the course of lung disease in children and adolescents with cystic fibrosis and early glucose derangements

Diabetes increases morbidity and mortality in cystic fibrosis (CF) patients, but several studies indicate that also prediabetic status may have a potential impact on both nutrition and lung function. To evaluate the effect of glargine on the clinical course in CF patients with early glucose derangements. CF population was screened for glucose tolerance. CF patients with age > 10 yr were screened with fasting hyperglycemia (FH). CF patients with age > 10 yr without FH and those with age < 10 yr with occasional FH were evaluated for glucose abnormalities on the basis of oral glucose tolerance test and/or continuous glucose monitoring system. All CF patients with glucose derangements were enrolled in an open clinical trial with glargine. Body mass index (BMI) z-score, forced expiratory volume in the first second (FEV(1)), number of acute pulmonary exacerbations and hemoglobin A1c, were as outcome measures at baseline and after 1 yr of treatment. After 12 months of therapy, BMI z-score improved only in patients with baseline BMI z-score less than -1 (p = 0.017). An 8.8% increase in FEV(1) (p = 0.01) and 42% decrease in the number of pulmonary exacerbations (p = 0.003) were found in the whole group compared with previous 12 months of therapy. Glargine could represent an innovative strategy to prevent lung disease progression in CF patients with early glucose derangements. Larger controlled trials are needed to better clarify the effects of insulin on clinical status in CF patients with early glucose derangements.

Raw bioelectrical impedance analysis variables (phase angle and impedance ratio) are significant predictors of hand grip strength in adolescents and young adults

Screening for hypertension in young people with obesity: Feasibility in the real life

Summary: The study aimed to assess the feasibility of two-repeated measurements of blood pressure in outpatient youths with overweight/obesity and to identify the features of those who missed the follow-up and the predictive role of clinical and/or anamnestic features on confirmed hypertension. The study found a high dropout rate in the obese population and highlighted the potential of family history of hypertension in identifying children at higher risk of confirmed hypertension.

Multidisciplinary Treatment for Childhood Obesity: A Two-Year Experience in the Province of Naples, Italy

Summary: This study assessed the adherence and outcomes of an outpatient service for childhood obesity treatment in Naples, Italy. Perceived Difficulties in Physical Tasks and Physical Fitness in Treatment- and Non-Treatment-Seeking Youths with Obesity

Summary: Youths with obesity who seek treatment tend to experience more physical and psychosocial distress, leading to limited physical activity and decreased quality of life. This study found that treatment-seeking youths with obesity had lower physical fitness and higher perceived difficulties in physical tasks compared to non-treatment-seeking youths. These findings highlight the importance of addressing perceived difficulties in physical tasks and providing personalized counseling to youth with obesity.

Summary: This study aimed to assess the prevalence of hypertension and its association with glomerular function and left ventricular geometry in overweight/obese youths based on the European Society of Cardiology consensus document (ESC-CD) or the Hypertension Canada guidelines (HTN-CGs). The ESC-CD identified a higher number of hypertensive youths before the appearance of glomerular impairment, offering advantages in terms of primary cardiovascular prevention.

Which Is the Most Appropriate Cut-Off of HbA1c for Prediabetes Screening in Caucasian Youths with Overweight or Obesity?

Summary: This cross-sectional study aimed to determine the best cut-off of HbA1c for detecting impaired fasting glucose, impaired glucose tolerance, beta-cell impairment, and cardiometabolic risk in overweight or obese Caucasian youths. The study found that an HbA1c cut-off of 5.5% was the most effective for detecting impaired fasting glucose and impaired glucose tolerance. In overweight or obese youths, an HbA1c level of >= 5.5% may be a more appropriate screening marker for glucose dysmetabolism.

Is Metabolic Syndrome Useful for Identifying Youths with Obesity at Risk for NAFLD?

Summary: The definition of metabolic syndrome (MetS) in childhood is controversial. In this study, a modified version of the International Diabetes Federation (IDF) definition was used to analyze the prevalence of MetS in overweight/obese youths and its association with non-alcoholic fatty liver disease (NAFLD). The results showed that whichever definition was used, one third of overweight/obese youths had MetS, and neither definition was superior in identifying those at risk for NAFLD.

A height-weight formula to measure body fat in childhood obesity

Summary: The estimated fat mass and percentage of body fat calculated by a height-weight equation showed reasonable agreement with the results from a BIA equation. Both fat mass and percentage of body fat were significantly associated with BMI Z-score, with a stronger correlation observed when using the height-weight equation. This formula can be used as a surrogate for estimating body fat when instrumental tools are not available.
