Diabetes Mellitus in Patients With Thalassemia Major

BackgroundDiabetes mellitus is a major endocrinopathy for patients with thalassemia major. Although diabetes mellitus is multifactorial, iron loading is its primary cause and its management poses a clinical challenge. Detecting the pre-diabetes stage is critical because clinical diabetes can potentially be reversed or prevented. ProcedurePatients with thalassemia major who received regular blood transfusion therapy from 1994 to 2010 were evaluated for the incidence of diabetes mellitus and glucose dysregulation. The association between patients' clinical, biochemical, and image parameters was also evaluated. ResultsThe patients with diabetes were significantly older, had higher ferritin levels, a smaller pancreas volume, and lower cardiac T2* magnetic resonance imaging (MRI) values than the patients without diabetes. The pancreas T2* MRI values were higher in the patients without diabetes, but the difference was not statistically significant. The liver iron concentration did not differ between the patients with and without diabetes. The prevalence of hepatitis C infection and hypogonadism was also higher in the patients with diabetes. In the patients without diabetes, the cardiac T2* MRI values were higher in patients with normal fasting glucose levels (P=0.03), and the homeostasis model assessment of insulin resistance level was associated with hepatitis C infection (P=0.024, r=0.32) and hypogonadism (P=0.034, r=0.301). ConclusionsFasting glucose and insulin levels were appropriate screening tools for evaluating glucose dysregulation and complemented the MRI findings. Summary: Children with primary cancer who develop a second cancer face an increased risk of unfavorable outcomes, particularly with early occurrence of secondary hematological malignancies. Age at diagnosis of the second cancer and prior radiation doses are associated with the type of second cancer, impacting treatment and survival rates for second cancers.

Summary: This study used immunohistochemistry and RNA sequencing for the molecular classification of medulloblastoma, and multiplex ligation-dependent probe amplification to determine chromosomal alterations and gene amplifications. The results showed that immunohistochemistry can provide rapid subgroup stratification of medulloblastoma, but may lead to misclassification. Therefore, validation of immunohistochemistry results with genomic data analysis by RNA sequencing or other tools is recommended. In addition, multiplex ligation-dependent probe amplification can detect important genetic alterations and help identify high-risk patients.

Acute pancreatitis in children with acute lymphoblastic leukemia correlates with L-asparaginase dose intensity

Summary: l-Asparaginase is an important therapeutic for childhood acute lymphoblastic leukemia, and the accumulated dosage of l-asparaginase is considered as a major risk factor of asparaginase-associated pancreatitis. This study found that the incidence of pancreatitis in childhood ALL patients is more strongly correlated with the dose intensity of l-asparaginase rather than with the cumulative dosage. High peak dose intensity and older age at diagnosis were identified as independent predictive factors for the development of AAP.

Summary: Identification of specific leukemia subtypes is crucial for risk-directed therapy in childhood acute lymphoblastic leukemia (ALL). Although RNA sequencing (RNA-seq) is the most effective method, its routine use is expensive in resource-limited countries. This study enrolled 295 pediatric ALL patients and used a combination of screening methods and RNA-seq to improve classification and clinical assessment of childhood ALL.

Summary: This study retrospectively analyzed the prognostic factors and treatment outcomes of primary malignant mediastinal germ cell tumors (GCTs) in Taiwanese children. The results showed that surgical treatment was associated with better outcomes, while advanced disease stage and choriocarcinoma histology subtype were associated with poorer outcomes. Additionally, there was no difference in treatment outcomes between cisplatin-based and carboplatin-based chemotherapy.

This study provides important data to inform the design of future clinical trials in Taiwan.

Evaluating brain white matter hyperintensity, IQ scores, and plasma neurofilament light chain concentration in early-treated patients with infantile-onset Pompe disease

Summary: The study aimed to investigate the CNS progression in patients with infantile-onset Pompe disease (IOPD) and its clinical impact and predictors. The findings showed that CNS involvement in IOPD expanded from white matter to gray matter, and the MRI scores were positively correlated with age and negatively correlated with IQ scores. Neuroaxonal injury and decreased IQ scores may be associated with the progression of CNS involvement in IOPD. Neurofilament light chain could serve as a biomarker for CNS involvement in IOPD.

Summary: This study investigated the utility of liver-to-psoas apparent diffusion coefficient ratios (LTPAR) and age-adjusted serum matrix metalloproteinase-7 (MMP-7) for diagnosing biliary atresia (BA) in cholestatic infants. The results showed that LTPAR was significantly lower in BA infants, while age-adjusted MMP7 ratio was significantly higher. Considering both serum and imaging parameters may improve the diagnostic performance of BA in cholestatic infants.
