Tumor budding is recognized as an adverse prognostic factor for a few carcinoma types. We evaluated the importance of this finding in 64 laryngeal carcinoma patients treated with surgery and radiotheraphy, with a median follow up of 53 (6-181) months. Budding was determined by re-evaluating hematoxylin-eosin (H&E) stained tumor sections, and classified as mild, moderate, and marked. Budding was not identified in 14 (21.88%) cases. Mild, moderate, and marked budding was observed in 21 (32.81%), 15 (23.44%), and 14 (21.88%) cases, respectively. Only when cases with marked budding were compared with the others, statistically significant results were obtained. in patients with and without marked budding, local disease-free survival, metastatic disease-free survival, and overall survival were 71% and 97% (p=0.72), 77% and 93% (p=0.038), and 77% and 75% (p=0.71), respectively. Marked budding was identified as a prognostic factor by univariate analysis for distant metastasis-free survival. Multi-variate analysis, by which well-recognized prognostic histopathologic parameters were evaluated, revealed that only the number of metastatic lymph nodes and budding were significantly associated with distant metastasis (p=0.02 and p=0.044), respectively. These results suggest that budding might be a valuable prognostic factor, particularly for distant metastasis in laryngeal carcinomas. (C) 2009 Elsevier GmbH. All rights reserved.

Summary: This study aims to investigate the diagnostic performance of MRI in predicting the pathologic stage of LARC after neoadjuvant CRT and the role of MRI in selecting patients with ypCR. The results showed moderate accuracy and sensitivity in predicting ypT and N stages, mainly due to underestimating tumor stage and overestimating nodal status. Furthermore, high specificity and NPV but low sensitivity were observed in predicting the complete response.

Summary: The aim of this study was to analyze the prognostic factors and treatment outcomes of cervical esophageal carcinoma (CEC) patients who underwent definitive chemoradiotherapy (CRT). Clinical data of 175 biopsy-confirmed CEC patients treated with definitive CRT were analyzed retrospectively. The study found that treatment response and patient performance status were significant prognostic factors for overall survival, progression-free survival, and local recurrence-free survival. In addition, higher radiation doses did not affect treatment outcomes. The study concluded that definitive CRT is a safe and effective treatment for CEC patients.

Summary: This study investigated the prevalence of sicca symptoms and secondary Sjogren's syndrome (SjS) in patients with systemic sclerosis (SSc), as well as evaluated the expression of alpha-smooth muscle actin (alpha-SMA) in minor salivary gland (MSG) specimens. Results showed that sicca symptoms were common among SSc patients, with nearly one-third fulfilling the criteria for SjS, particularly in the limited subtype. Older age and positive anti-SS-A antibodies were identified as predictors for SjS, and glandular fibrosis was significant but alpha-SMA staining was rare in patients with SSc.

PPY-cell hyperplasia accompanying NENs: Immunohistochemical and nuclear medicine analysis

P. Suhaj, D. Do, T. Olejar, R. Pichova, O. Lang, R. Matej

Summary: Pancreatic polypeptide cell hyperplasia (PPY-H) is a multiplication of neuroendocrine cells producing pancreatic polypeptide (PPY). The development and role of PPY-H still need to be further elucidated. This study analyzed 12 cases of PPY-H accompanying pancreatic neuroendocrine neoplasias (NEN) and found that gastrointestinal symptoms may be more related to PPY-H rather than NEN hormonal production. Strong SSTR2 expression in PPY-H suggests the potential utility of radiotracers in clinical diagnostics, but further studies are needed.

TESC acts as a prognostic factor and promotes epithelial-mesenchymal transition progression in esophageal squamous carcinoma

Summary: Tescalcin (TESC) is upregulated in esophageal squamous carcinoma (ESCC) and is associated with poor prognosis, tumor infiltration, staging, and lymph node metastasis. TESC promotes proliferation, invasion, migration, and epithelial-mesenchymal transition (EMT) progression in ESCC, as well as activates the AKT pathway. TESC may serve as a prognostic factor and provide a new therapeutic strategy for ESCC.
