Combination of cyst fluid CEA and CA 125 is an accurate diagnostic tool for differentiating mucinous cystic neoplasms from intraductal papillary mucinous neoplasms

Background/objectives: Despite advances in imaging techniques, diagnosis and management of pancreatic cystic lesions still remains challenging. The objective of this study was to determine the utility of cyst fluid analysis (CEA, CA 19-9, CA 125, amylase, and cytology) in categorizing pancreatic cystic lesions, and in differentiating malignant from benign cystic lesions. Methods: A retrospective analysis of 68 patients with histologically and clinically confirmed cystic lesions was performed. Cyst fluid was obtained by surgical resection (n = 45) or endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) (n = 23). Cyst fluid tumor markers and amylase were measured and compared between the cyst types. Results: Receiver operating characteristic (ROC) curve analysis of the tumor markers demonstrated that cyst fluid CEA provided the greatest area under ROC curve (AUC) (0.884) for differentiating mucinous versus non-mucinous cystic lesions. When a CEA cutoff value was set at 67.3 ng/ml, the sensitivity, specificity and accuracy for diagnosing mucinous cysts were 89.2%, 77.8%, and 84.4%, respectively. The combination of cyst fluid CEA content >67.3 ng/ml and cyst fluid CA 125 content >10.0 U/ml segregated 77.8% (14/18) of mucinous cystic neoplasms (MCNs) from other cyst subtypes. On the other hand, no fluid marker was useful for differentiating malignant versus benign cystic lesions. Although cytology (accuracy 83.3%) more accurately diagnosed malignant cysts than CEA (accuracy 65.6%), it lacked sensitivity (35.3%). Conclusions: Our results demonstrate that cyst fluid CEA can be a helpful marker in differentiating mucinous from non-mucinous, but not malignant from benign cystic lesions. Sporadic and Familial Adenomatous Polyposis-associated Foveolar-type Adenoma of the Stomach

Summary: Gastric foveolar-type adenoma (FA) is a rare benign neoplasm that can either occur sporadically or in patients with familial adenomatous polyposis (FAP). This study aimed to investigate the molecular features of FA and the relationship between sporadic and syndromic lesions. The results showed that sporadic FA includes at least two morphologically and genetically distinct subtypes: flat and raspberry-like FA. Additionally, it was found that flat FA represents a sporadic counterpart of FAP-associated FA.

Long-Term Prognosis and Prognostic Indicators of Stage IA Lung Adenocarcinoma

Summary: This study found that the prognosis and survival of lung cancer patients after 5 years of surgery were associated with age rather than stage classification. It suggests that the long-term follow-up strategy does not need to be modified based on stage classification, and screening for secondary primary lung cancer should be considered.

Feasibility and effectiveness of segmentectomy versus wedge resection for clinical stage I non-small-cell lung cancer

Summary: In this study, we found that segmentectomy is a feasible procedure with comparable safety outcomes and better surgical margins and cancer control than wedge resection, particularly for clinical stage IA2 NSCLC. Segmentectomy significantly improved the recurrence-free survival compared with wedge resection, especially for patients with clinical stage IA. Subgroup analysis also demonstrated that segmentectomy had better recurrence-free survival in clinical stage IA2 and pure-solid tumors compared to wedge resection.

Summary: The diagnostic ability of performing endoscopic ultrasound-guided tissue acquisition (EUS-TA) using a 19G needle has been retrospectively reviewed. The 19G TopGain needle showed favorable sensitivity, specificity, and accuracy for confirmed diagnosis. However, the use of 19G fine-needle biopsy (FNB) needles may increase adverse events. EUS-TA with a 19G FNB needle is mainly indicated in lesions where comprehensive genomic profiling may be necessary or the diagnosis could not be determined via EUS-TA using the 22G needle.

Summary: This study evaluated the clinical efficacy of using a novel self-expandable metallic stent (SEMS) for endoscopic ultrasound-guided intervention without fistula dilation (EUS-IV WoD). The results showed a technical success rate of 100% and a clinical success rate of 72.8% for EUS-IV WoD. Early adverse events, including mild abdominal pain and moderate bleeding, were observed in 27.3% of the patients. The novel stent may be useful for EUS-IV WoD, especially in EUS-biliary drainage.

Impact of age for overall survival in head and neck sarcoma

Summary: The present study aimed to investigate the relationship between age at surgery and survival outcomes in patients with sarcoma in the head and neck. A total of 26 patients who underwent surgery for head and neck sarcoma from 2003 to 2017 were included. Patients who did not receive chemotherapy were significantly older at the time of surgery. Receiver operating curve analysis determined 55 years as the cutoff age for predicting death. Older age was associated with shorter overall, disease-specific, local recurrence-free, and disease-free survival rates. Multivariate analysis using Cox's proportional hazards model adjusted for chemotherapy (absence/presence) showed that age (>= 55 years/<55 years) was correlated with shorter overall survival. In conclusion, older age predicts worse overall survival in patients with head and neck sarcoma.

Summary: This study identified 12 novel susceptibility variants for lung adenocarcinoma and a total of 28 loci associated with the disease in East Asian populations. Additionally, new candidate genes were discovered, contributing to the understanding of the disease etiology. These findings have important implications for the research and translational applications of lung adenocarcinoma in individuals from East Asian populations.

Summary: This study aimed to clarify the clinicopathological and molecular features of HER2-amplified and HER2-low colorectal cancers (CRCs). The results showed that HER2-amplified and HER2-low CRCs have different clinicopathological and molecular features, and HER2 expression can be assessed through biopsy. Therefore, this study is of great significance for the identification of CRC patients eligible for systemic HER2-targeted therapy.

Clinical and pathological features of second primary neoplasms arising in head and neck reconstructive skin flaps

Summary: The incidence of second primary neoplasms arising in the skin reconstructive flap (SNAF) is increasing. The prognosis, optimal treatment, and clinicopathological-genetic features of SNAFs are difficult to determine. This study retrospectively reviewed SNAFs over a 20-year period and found that they are slow-growing tumors with good prognoses, characterized by altered p53 and TP53 rates and high p16 positivity.

Summary: This study found that preoperative pancreatic juice cytology (PJC) has low accuracy in differentiating malignant and non-malignant lesions in intraductal papillary mucinous neoplasm (IPMN) of the pancreas and is not recommended as a routine examination. Although the sensitivity is low, the positive predictive value is high, suggesting that PJC should only be performed when the patient is uncertain about surgery.
