Summary: This study aimed to develop a three-month mortality predictive model for patients with advanced heart failure and found that, in addition to age, gender, potassium levels, low body mass index, and low glomerular filtration rate, dependence for activities of daily living is also an important factor in predicting three-month mortality.

Summary: In the COAPT trial, patients treated with TMVr had less severe residual MR at 30 days compared to those treated with GDMT. Patients with lighter residual MR at 30 days had lower rates of death or heart failure hospitalizations between 30 days and 2 years, and experienced better quality of life.

Summary: Information about health-related quality of life (HRQoL) in heart failure (HF) in older adults is limited. This study described the HRQoL of a cohort of older patients with recent HF hospitalization who received multidisciplinary intervention by a geriatrician and a cardiologist compared to a control group who received intervention by a cardiologist alone. The results showed that almost half of the patients had impaired HRQoL, and those with worse HRQoL had higher mortality and hospitalization rates.

Summary: This study aimed to investigate the predictive roles of pre-operative left ventricular size and ejection fraction in ejection fraction improvement and outcome following revascularization in patients with coronary artery disease and left ventricular dysfunction. The results showed that pre-operative left ventricular end-systolic diameter and ejection fraction were predictive of ejection fraction improvement after revascularization, and smaller left ventricular end-systolic diameter and lower ejection fraction were associated with better outcomes.

Summary: This study quantitatively characterized the systemic impairment reflected by conventional biomarkers among patients hospitalized for heart failure, and found that the abnormal values of these biomarkers had a substantial cumulative adverse influence on clinical outcomes and quality of life.

Summary: Recent trials have shown that sodium-glucose cotransporter inhibitors and other new molecules have significant efficacy in treating heart failure, improving patient prognosis. There is also new evidence for therapeutic approaches for HF patients with mid-range or preserved left ventricular ejection fraction.

Summary: There are significant differences in HRQL among patients with HF in different regions, with lower HRQL being associated with adverse outcomes. Lower HRQL is a strong and independent predictor of all-cause death and HF hospitalization across all geographic regions worldwide.

Summary: Professional medical interpreters focus intensively on accuracy in end-of-life discussions, despite facing emotional and professional challenges. External factors such as rapid discussion tempo and perceived non-empathetic behaviors of providers may unintentionally or intentionally impact the interpretation accuracy.

Summary: This study evaluated the understanding of disease burden and prognosis in children with advanced heart disease by parents and physicians. It found that parents were generally more optimistic than physicians, expecting fewer limitations in physical activity, learning/behavior, and life expectancy for their children. Physicians tended to underestimate symptom burden reported by parents. Interventions involving patient-reported outcomes, parent education, and physician communication tools may be beneficial in aligning perspectives and improving care for these children.

Summary: This study aimed to explore the prognostic value of abnormal electrocardiogram (ECG) at baseline in patients with heart failure with preserved ejection fraction (HFpEF). The results showed that abnormal ECG was significantly associated with higher risks of cardiovascular events and HF hospitalization in HFpEF patients. Specific abnormalities like bundle branch block and atrial fibrillation were also found to be predictive of adverse outcomes.

Summary: This study examined the sex differences in Chinese patients with heart failure. The findings showed that women had worse functional capacity, quality of life, and prognosis compared to men. Therefore, attention should be paid to sex differences in the management of heart failure.

Summary: Prognostication is considered as an essential component in medical care, but it becomes increasingly challenging as research advances bring new treatments for complex conditions like cancer and dementia. This article, written by palliative care clinicians with expertise in geriatrics, pulmonology, and oncology, aims to provide a helpful framework for prognostication in these illnesses.

Summary: Referral to palliative or supportive care services is challenging for patients with pulmonary arterial hypertension (PAH). Experts reached a consensus that patients should be referred to these services when disease symptoms become unmanageable or for end-of-life care. Pain management techniques, end-of-life care, and psychosocial recommendations were identified as important services. Discussing palliative or supportive care with patients, focusing on their overall health and treatment adherence, as well as prioritizing psychosocial aspects of care, was found to be important.
