Summary: Pain is a common clinical symptom caused by nerve injury, tissue damage, or diseases. It affects daily physical function and quality of life, resulting in physiological and psychological torture. The underlying molecular mechanisms and signaling pathways of pain are not fully understood, making pain management challenging. Autophagy, an intracellular process, plays a vital role in maintaining neural function and has been linked to neuropathic pain. Traditional Chinese medicine monomers have also been found to involve autophagy in pain relief, suggesting autophagy as a potential target for pain management.

Summary: Skin biopsies from patients with postherpetic neuralgia (PHN) showed no correlation between nerve fiber density and baseline pain, and concurrent loss of nerve fibers on the affected side was common. The role of Nav1.7 in PHN pain is still unclear.

Summary: The analgesic efficacy and safety of EMA401 in patients with postherpetic neuralgia and painful diabetic neuropathy were assessed in two phase 2b studies, showing consistent clinical improvement in pain intensity reduction. However, premature termination of the studies prevented definitive conclusions from being drawn.

Summary: The introduction of a reimbursement application system significantly reduced the dispensing rate of Versatis(R), mainly through reducing expenditure under the GMS scheme. It is recommended to restrict the prescription of Versatis(R) to patients with PHN to ensure evidence-based use of the medication.

Summary: This study investigates the use of capsaicin 8% patch (C8P) in the treatment of neuropathic pain (NP) in patients with spinal cord injury (SCI). The results show that C8P can effectively reduce pain and improve mobility in SCI patients, but does not improve quality of life (QOL).

Summary: Lumbar sympathetic block is effective in treating lower-limb PHN, as demonstrated by two cases with relief of symptoms, suggesting its potential role in the treatment of this neuropathic pain syndrome. Additional reports and controlled trials are necessary for further understanding its effectiveness.

Summary: A study found that there is dysbiosis in the gut microbiota of postherpetic neuralgia (PHN) patients. The microbiome differences can be used to distinguish PHN patients from healthy individuals with high sensitivity and specificity, and altered gut microbiota are related to clinical manifestations. This provides potential novel directions for the prevention and treatment of PHN.

Summary: This study demonstrates that a new sustained-release pregabalin formulation is noninferior to immediate-release pregabalin in reducing peripheral neuropathic pain in Korean patients with diabetic peripheral neuropathy or postherpetic neuralgia. The new formulation is well-tolerated after 12 weeks of treatment.

Summary: This study compared the whole brain networks of herpes zoster (HZ) patients and post-herpetic neuralgia (PHN) patients. It found that the functional connectivity changes between the ventral frontal cortex and occipital gyrus may be a robust neuroimaging marker for the transition from HZ to PHN. This research provides knowledge on the effectiveness of whole-brain network analysis in distinguishing HZ and PHN patients and predicting pain intensity.

Summary: The study aimed to quantitatively analyze the research outputs of studies on post-herpetic neuralgia (PHN) published over the past 30 years using bibliometric analysis. The results showed an increasing trend in the number of PHN-related publications and the academic achievements of Korean pain physicians in this field, but there is a relatively small proportion of studies on interventional treatments. Therefore, Korean pain physicians need to strengthen academic research on interventional treatment to expand their role in this field and improve the outcomes of PHN patients.
