Summary: The expression of TLR4 in myeloid-lineage cells, astrocytes, and neurons plays a role in the transition from acute to chronic pain, with systemic TLR4 antagonism showing improvement in chronic CRPS when administered early. Additionally, the contribution of TLR4 to chronic pain is time- and microglia-dependent in both sexes, with females also relying on peripheral myeloid-lineage cells for triggering chronic pain.

Summary: This study aimed to investigate the prevalence and characteristics of functional neurological disorder (FND) in patients with chronic pain. The results showed that among 190 chronic pain patients, 32 (17%) had a diagnosis of FND and an additional 11 (6%) had undiagnosed neurological symptoms. Chronic pain patients with comorbid FND were more likely to have chronic primary pain, widespread chronic primary pain, and depression. However, there were no significant differences in medication prescription or pain outcome between patients with and without FND.

Summary: The study showed that patients with FND do not exhibit a stronger placebo response compared to healthy controls, suggesting that increased suggestibility to placebo in FND patients may be a misconception. Instead, occasional dramatic placebo responses may occur due to the inherent variability of functional symptoms compared to organic diseases.

Summary: Functional neurological disorder (FND) is often overlooked and misunderstood by clinicians, despite being a common and accurately diagnosable condition based on clinical signs. People with FND face discrimination from healthcare providers, researchers, and the public, reflecting the neglect of disorders primarily affecting women. We argue that FND is a feminist issue, drawing on historical and contemporary perspectives from clinical, research, and social domains. We call for equal attention and support for FND in medical education, research, and clinical services to ensure adequate care for those affected.

Summary: This study investigates the role of pain-modulatory brainstem pathways in complex regional pain syndrome (CRPS) and reveals altered habituation of the nociceptive blink reflex (nBR) as a potential diagnostic biomarker. Patients with CRPS showed a significant decrease in habituation on both the stimulated and non-stimulated side. This is the first study to demonstrate the pathophysiological mechanism and diagnostic potential of altered nBR habituation in CRPS.

Summary: Through cluster analysis of 61 chronic CRPS and 31 CLP patients, three clinical phenotypes were identified: 'CRPS', 'CLP', and 'Mixed'. These groups differed in pain sensitivity and psychological state, particularly in evoked pain. This study provides important evidence for the diagnosis of CRPS and its subtypes, with TNF-alpha identified as a potential biomarker.

Summary: We present the case of a 17-year-old woman who developed complex regional pain syndrome (CRPS) after receiving the second dose of the COVID-19 vaccine. She was referred to our department for surgical treatment of a movement disorder seven months after the vaccination. Intrathecal baclofen and zolpidem were administered to improve her symptoms, but only zolpidem had a significant effect on reducing involuntary movements in her right hand. It is important for healthcare providers to be aware of the potential risk of CRPS following COVID-19 vaccination.

Summary: Functional neurological disorder (FND) is a prevalent, disabling, and costly condition at the neurology-psychiatry intersection. Recent advances in diagnosis, mechanisms, aetiologies, treatments, and stigma have been made in the past decade, but challenges still remain. Despite this, a patient-centered integrated clinical neuroscience approach is poised to continue the momentum of the past decade into the future.

Summary: Prompt and appropriate treatment of chronic low back pain is crucial for preventing disability and reducing healthcare costs. The concept of functionality in chronic pain management is still lacking a shared definition, leading to different opinions among specialists and patients. Although there are instruments available for assessing functionality, there is no consistency in their use.

Summary: This study aimed to explore the effects of age, side, and other functional conditions on pressure pain threshold (PPT) and sensory abnormalities in elderly stroke survivors. The results showed that older patients had higher sensitivity in both sides and the affected side had more somatosensory abnormalities, particularly hypoalgesia. Elderly patients in good functional conditions seemed to have more equal pain sensitivity between both sides.

Summary: Cardiac rehabilitation (CR) is essential for improving cardiovascular health outcomes in cardiac patients. This study aimed to investigate the relationship between baseline health-related quality of life (HRQOL) and exercise capacity improvement during CR. The results showed that lower emotional HRQOL and higher physical HRQOL were associated with less improvement in exercise capacity.

Summary: This study investigates the role of steroid treatment in identifying cancer patients who are distressed or mentally disordered. By analyzing the medical records of 12,298 cancer patients, the study identifies four subgroups of patients using different dosages of prednisone equivalents. The study finds that patients with higher dosages of prednisone are more likely to receive psychotropic drug administration, while patients with lower dosages have a slightly increased probability of psychiatric assessment and psychotropic drug administration. The subgroup with the lowest probability of receiving steroid treatment also has the lowest likelihood of receiving psychiatric assessment and psychotropic drug administration.

Summary: International guidelines recommend screening for cancer patients’ distress, but rates are insufficient. This study developed a survey to assess systemic barriers to distress screening. Through a three-step approach, an initial survey with 53 questions was reduced to a final set of 14 validated questions using qualitative content analysis and factor analysis. Major barriers identified include timing of screening, lack of capacity, patient and staff overload, and refusal of patients to be referred to support services. The validated questions enable quick identification and improvement of screening programs.

Summary: The COVID-19 pandemic has negatively affected the mental health of ICD patients, leading to fear, anxiety, depression, and PTSD. Factors such as unemployment, younger age, low social support, prior COVID-19 infection, and inadequate knowledge about COVID-19 predicted these mental health issues. Fear of COVID-19 had an impact on anxiety, depression, and PTSD, with positive and negative affects mediating these relationships. Psychological and educational interventions are crucial for vulnerable subgroups of ICD patients.

Summary: This study aims to explore factors associated with declining psycho-oncological support in order to increase nurses' efficiency in screening patients. The developed model has a high sensitivity in detecting patients who decline psycho-oncological support, with older patients, patients with lower distress levels, fewer comorbidities, fewer physical problems, and those who do not feel sad, afraid, or worried being more likely to refuse psycho-oncological support.
