Nondermatomal somatosensory deficits in chronic pain patients: Are they really hysterical?

Patients with chronic pain disorders frequently show nondermatomal somatosensory deficits (NDSDs) that are considered to be functional. Typically, NDSDs show quadratomal or hemibody distribution ipsilateral to the areas of chronic pain. According to the Diagnostic and Statistical Manual of Mental Disorders, 4th edition and the International Classification of Diseases, 10th revision, such functional somatosensory deficits are classified in the chapter conversion disorder. Many publications also used the term hysterical sensory loss. However, doubts are increasing about this one-sided psychiatric view. We aimed to better characterize the biopsychosocial factors associated with NDSDs. Therefore, we compared 2 groups of inpatients with chronic pain disorder, of whom 90 suffered from NDSDs and 90 did not. The patients with NDSDs all showed widespread somatosensory deficits with hemibody distribution. On logistic regression analysis, history of a prior physical trauma was positively predictive for patients with NDSDs. Personality disorder and adverse childhood experiences were positively predictive for the control group with chronic pain disorders without NDSDs. The frequencies of comorbid depression and anxiety disorder did not differ statistically between groups. In conclusion, pain patients with NDSDs are, psychopathologically, by no means more noticeable personalities than patients with chronic pain disorder without NDSDs. Similar to complex regional pain syndromes, we assume a multifactorial etiology of NDSDs, including stress. Based on our observations, terms like hysteric should not be applied any longer to patients with NDSDs who suffer from chronic pain. (C) 2012 International Association for the Study of Pain. Steriod-associated psychiatric burden in cancer patients

Summary: This study investigates the role of steroid treatment in identifying cancer patients who are distressed or mentally disordered. By analyzing the medical records of 12,298 cancer patients, the study identifies four subgroups of patients using different dosages of prednisone equivalents. The study finds that patients with higher dosages of prednisone are more likely to receive psychotropic drug administration, while patients with lower dosages have a slightly increased probability of psychiatric assessment and psychotropic drug administration. The subgroup with the lowest probability of receiving steroid treatment also has the lowest likelihood of receiving psychiatric assessment and psychotropic drug administration.

Sympathetic nervous system responses to acute psychosocial stress in male physicians with clinical burnout

A short screening tool identifying systemic barriers to distress screening in cancer care

Summary: International guidelines recommend screening for cancer patients’ distress, but rates are insufficient. This study developed a survey to assess systemic barriers to distress screening. Through a three-step approach, an initial survey with 53 questions was reduced to a final set of 14 validated questions using qualitative content analysis and factor analysis. Major barriers identified include timing of screening, lack of capacity, patient and staff overload, and refusal of patients to be referred to support services. The validated questions enable quick identification and improvement of screening programs.

Summary: The COVID-19 pandemic has negatively affected the mental health of ICD patients, leading to fear, anxiety, depression, and PTSD. Factors such as unemployment, younger age, low social support, prior COVID-19 infection, and inadequate knowledge about COVID-19 predicted these mental health issues. Fear of COVID-19 had an impact on anxiety, depression, and PTSD, with positive and negative affects mediating these relationships. Psychological and educational interventions are crucial for vulnerable subgroups of ICD patients.

Early impact of the COVID19 pandemic on psycho-oncological support: a latent class analysis

Summary: This study is the first to examine the impact of the COVID-19 pandemic on the provision of psycho-oncological care, stage of cancer at first diagnosis, and duration of hospitalizations. The retrospective latent class analysis of 4639 electronic patient files revealed that the pandemic had no effect on the provision of psycho-oncological support.

The chronic stress risk phenotype mirrored in the human retina as a neurodegenerative condition

Summary: The brain is the key organ that orchestrates the stress response which translates to the retina. The retina is an extension of the brain and retinal symptoms in subjects with neurodegenerative diseases substantiated the eye as a window to the brain. The retina is used in this study to determine whether chronic stress reflects neurodegenerative signs indicative of neurodegenerative conditions.

Clinically Significant Distress and Physical Problems Detected on a Distress Thermometer are Associated With Survival Among Lung Cancer Patients

Summary: This study aimed to assess the association between psychological distress and survival time in cancer patients, and to explore the predictive value of a problem list. Results showed that patients with higher psychological distress had shorter survival time. Additionally, certain items from the problem list were negatively associated with survival time.

Summary: This study aimed to test an integrative model for explaining the severity of insomnia in family caregivers of people with dementia. The results showed that predisposing, precipitating, and perpetuating factors were all associated with caregivers' insomnia severity. The proposed integrative model may be useful in developing interventions for insomnia symptoms in family dementia caregivers.

Predictive Factors Associated with Declining Psycho-Oncological Support in Patients with Cancer

Summary: This study aims to explore factors associated with declining psycho-oncological support in order to increase nurses' efficiency in screening patients. The developed model has a high sensitivity in detecting patients who decline psycho-oncological support, with older patients, patients with lower distress levels, fewer comorbidities, fewer physical problems, and those who do not feel sad, afraid, or worried being more likely to refuse psycho-oncological support.
