Alkylresorcinol, a biomarker for whole grain intake, and its association with osteoarthritis: the MOST study

J. -P. Zertuche, G. Rabasa, A. H. Lichtenstein, N. R. Matthan, M. Nevitt, J. Torner, C. E. Lewis, Z. Dai, D. Misra, D. Felson

Summary: Higher fiber intake is associated with a lower risk of osteoarthritis (OA). The study found a possible association between levels of alkylresorcinol (AR), a marker of whole grain intake, and OA. High AR levels may have a protective effect against OA.

The Relationship of Pain Reduction With Prevention of Knee Replacement Under Dynamic Intervention Strategies

S. Reza Jafarzadeh, Tuhina Neogi, Daniel K. White, David T. Felson

Summary: Appropriate pain interventions can significantly reduce the need for knee replacement surgery in patients with osteoarthritis, with larger reductions in risk achieved when pain is reduced to a greater extent and interventions are introduced at lower pain levels.

Meaning of patient global assessment when joint counts are low in rheumatoid arthritis

David Felson, Vivi Feathers, Chinmayi Naik, Daniel H. Solomon, Michael E. Weinblatt, Nancy Shadick

Summary: In rheumatoid arthritis patients with low 28-joint tender and swollen joint counts but who perceive their disease as active, there may be pain in uncounted joints that could respond to rheumatoid arthritis treatment.

Summary: This study aimed to externally validate a revised Boolean remission criteria using a higher patient global assessment (PtGA) threshold and validate the provisionally endorsed index-based criteria. The results showed that using the higher PtGA threshold increased the proportion of patients classified as achieving remission under the "Boolean2.0" criteria, improved agreement with index-based criteria, and did not compromise predictive value for radiographic or functional outcomes. The study findings were endorsed by ACR and EULAR.

Relation of Temporal Asymmetry During Walking to Two-Year Knee Pain Outcomes in Those With Mild-to-Moderate Unilateral Knee Pain: An Exploratory Analysis From the Multicenter Osteoarthritis Study

Patrick Corrigan, David T. Felson, Cara L. Lewis, Tuhina Neogi, Michael P. LaValley, K. Doug Gross, Michael C. Nevitt, Cora E. Lewis, James C. Torner, Joshua J. Stefanik

Summary: The study aimed to investigate the relationship between unilateral knee pain severity and temporal asymmetry during walking, as well as the associations of temporal asymmetry with changes in ipsilateral and contralateral knee pain over 2 years in individuals with mild-to-moderate unilateral knee pain. The study included participants with unilateral knee pain from the Multicenter Osteoarthritis Study, and gait and pain assessments were conducted at baseline and 2 years. The results showed that unilateral knee pain severity was not associated with temporal asymmetry during walking. However, certain measures of temporal asymmetry during self-selected and fast walking were related to longitudinal knee pain outcomes.

Summary: The revised Boolean remission criteria, using a higher patient global assessment threshold, classify more patients as achieving remission and increase the agreement with index-based remission criteria without jeopardizing predictive value for radiographic or functional outcomes.

A clinician's perspective on the role of imaging in knee osteoarthritis in clinical practice

David Felson, Tuhina Neogi

Summary: Knee osteoarthritis is a common and disabling disease. Diagnostic imaging is usually not necessary for diagnosis of chronic knee pain in individuals aged 45 and over, as treatment does not vary based on imaging results. However, imaging may be helpful for atypical presentations or for locating the joint during corticosteroid injections. Guidelines currently discourage imaging in the diagnosis or management of most persons with OA, but this may change for individuals with correctible lesions or specific subtypes of the disease.

Increased risk of osteoarthritis in patients with atopic disease

Matthew C. Baker, Khushboo Sheth, Rong Lu, Di Lu, Ericka P. von Kaeppler, Archana Bhat, David T. Felson, William H. Robinson

Summary: This retrospective cohort study found that patients with atopic disease have a higher incidence of osteoarthritis (OA) compared to non-exposed patients. The study showed that patients with asthma or atopic dermatitis had a higher risk of developing OA, particularly those with both conditions. These findings suggest a potential link between allergic pathways and the prevention or treatment of OA.

Gait, physical activity and tibiofemoral cartilage damage: a longitudinal machine learning analysis in the Multicenter Osteoarthritis Study

Kerry E. Costello, David T. Felson, S. Reza Jafarzadeh, Ali Guermazi, Frank W. Roemer, Neil A. Segal, Cora E. Lewis, Michael C. Nevitt, Cara L. Lewis, Vijaya B. Kolachalama, Deepak Kumar

Summary: This study developed a machine learning model that incorporates gait and physical activity to predict medial tibiofemoral cartilage worsening over 2 years in individuals without advanced knee osteoarthritis. The model showed good performance in predicting cartilage worsening and identified influential predictors in the outcome.

Hip Abductor Weakness and Its Association With New or Worsened Knee Pain: Data From the Multicenter Osteoarthritis Study

Cara L. Lewis, Neil A. Segal, Gabriela V. Rabasa, Michael P. LaValley, Glenn N. Williams, Michael C. Nevitt, Cora E. Lewis, David T. Felson, Joshua J. Stefanik

Summary: This study aimed to evaluate the relationship between hip abductor strength and worsened or new-onset frequent knee pain. The study found that women with weaker hip abductor strength had a higher risk of worsened knee pain, but there was no association with incident frequent knee pain in men or women.

New approach to testing treatments for osteoarthritis: FastOA

David Felson, Martin K. Lotz, Yuxuan Jin, Morgan Jones, Jason S. Kim, Kurt Spindler

Summary: Animal models have shown promising treatments for post traumatic osteoarthritis, but human trials have mostly failed to find effective treatments. This viewpoint suggests that the frequent failure in drug and treatment development for osteoarthritis may be due to the advanced disease stage of the patients in trials and proposes that mirroring the animal model approach could be more successful. It suggests selecting trial participants who are likely to quickly develop post traumatic osteoarthritis as a way forward.
