Reversal agents for current and forthcoming direct oral anticoagulants

Summary: In the past 20 years, there has been a transition from vitamin K antagonists to direct oral anticoagulants (DOACs). DOACs include dabigatran, apixaban, edoxaban, and rivaroxaban. Although DOACs are associated with less serious bleeding, reversal agents may be needed in cases of serious bleeding or urgent surgery/intervention. This paper provides insight into reversing the effects of DOACs, discusses new reversal agents, and outlines a strategic framework for reversing factor XI inhibitors in clinical trials.

Assessing balance in people with bilateral vestibulopathy using the Mini-Balance Evaluation Systems Test (Mini-BESTest): feasibility and comparison with healthy control data

Summary: Bilateral vestibulopathy (BVP) causes unsteadiness in walking, especially in darkness or on uneven ground, leading to falls. The study found that the Mini-BESTest is feasible in BVP, with participants scoring significantly lower than healthy participants. The negative correlation between age and balance is more significant in BVP.

Plasma Kallikrein as a Forgotten Clotting Factor

Summary: It was previously believed that plasma kallikrein (PKa) only functioned to activate factor XII (FXII) in the coagulation cascade. However, recent studies have revealed a new pathway where PKa can directly activate factor IX (FIX). These studies identified the binding of FIX/FIXa with PK/PKa, the PKa-induced thrombin generation and clot formation independent of FXI, and the increased formation of FIXa:AT complexes due to PKa activity. The existence of both a canonical (FXIa-dependent) and non-canonical (PKa-dependent) pathway of FIX activation is suggested.

Dataset of 3D gait analysis in typically developing children walking at three different speeds on an instrumented treadmill in virtual reality

Summary: In this article, gait data of typically developing children walking at different speeds is shared. The raw and processed data, including data of each single step, as well as the demographics and physical examination results of the subjects, are presented. The gait data is also presented per age group, providing insight into the normal gait pattern of TD children of different ages. Gait analysis was performed using a virtual environment, and the data was processed using Matlab algorithms.

PAR4 Inhibition Reduces Coronary Artery Atherosclerosis and Myocardial Fibrosis in SR-B1/LDLR Double Knockout Mice

Summary: This study investigated the effects of PAR4 inhibitory pepducin (RAG8) on coronary artery disease in SR-B1/LDLR double knockout mice fed a high-fat, high-cholesterol diet. The results showed that RAG8 reduced coronary artery atherosclerosis and myocardial fibrosis, as well as platelet and leukocyte accumulation in atherosclerotic coronary arteries.

Physical Functioning in Patients with a Recent Fracture: The Can Do, Do Do Framework Applied to Explore Physical Capacity, Physical Activity and Fall Risk Factors

Summary: Physical capacity (PC) and physical activity (PA) are used to categorize physical performance in the can do, do do framework. This study examined the physical performance of patients attending the fracture liaison service (FLS). It was found that approximately 20% of the patients were unable to perform certain physical activities but still engaged in them, and they had a higher prevalence of fall and fracture risk factors compared to those who could perform the activities.

Allosteric modulation of exosite 1 attenuates polyphosphate-catalyzed activation of factor XI by thrombin

Summary: Polyphosphate (polyP) serves as a template to promote feedback activation of factor XI by thrombin. This study determines the contribution of thrombin exosites 1 and 2 to polyP-induced potentiation of FXI activation. The results show that polyP accelerates FXI activation by thrombin and exosite 2 ligands attenuate this activation.

Rivaroxaban and apixaban are less effective than enoxaparin for the prevention of catheter-induced clotting in vitro

Summary: This study compared the effects of apixaban, rivaroxaban, and enoxaparin on the prevention of catheter-induced clotting in vitro. The results showed that apixaban and rivaroxaban were significantly less potent than enoxaparin in preventing catheter-induced clotting and thrombin generation. Clinical trials are needed to compare the efficacy of low-molecular-weight heparin with that of direct oral anticoagulants for both prevention and treatment of catheter thrombosis.

Warfarin faring better: vitamin K antagonists beat rivaroxaban and apixaban in the INVICTUS and PROACT Xa trials

Summary: Although DOACs are preferred over VKAs for stroke prevention in most patients with AF, they are not recommended for those with rheumatic heart disease or mechanical heart valves. Results from the INVICTUS and PROACT Xa trials support the use of VKAs for these specific indications. This paper reviews the findings from these trials, provides insights into why VKAs were superior to DOACs, and discusses future directions for anticoagulation in these disorders.

Adhesive properties of plasma-circulating and platelet-derived microvesicles from healthy individuals

Summary: In this study, we investigated the adhesive properties of circulating plasma-derived and platelet-derived microvesicles (MVs) and their potential role in thrombus growth. The results showed that both types of MVs weakly bound fibrinogen but did not stably adhere to immobilized fibrinogen. Moreover, the presence of these MVs did not significantly affect thrombus formation. Therefore, MVs present in the plasma of healthy individuals are unlikely to directly contribute to thrombus formation under arterial flow conditions.

Exploration of rotational thromboelastometry (ROTEM) to characterize the coagulation profiles of newly diagnosed pediatric leukemia patients

Summary: This study analyzed and evaluated leukemia-associated coagulopathy in newly diagnosed pediatric leukemia patients using CCTs and ROTEM. The results showed that fibrinogen and platelets played a significant role in causing coagulopathies in this population. This suggests the utility of using CCTs and ROTEM in the pediatric population to evaluate hemostatic function and guide blood product replacement.

Thrombotic triad in microgravity

Summary: Thrombotic disease may be underdiagnosed in microgravity conditions, and the underlying factors are still poorly understood. A case of internal jugular vein thrombosis was recently diagnosed in a low-risk female astronaut, and six out of ten additional crew members showed risk factors for jugular venous flow. Observations in space and studies on Earth suggest that microgravity affects blood flow, coagulation, and vascular function.

Driver gene KRAS aggravates cancer-associated stroke outcomes

Summary: The incidence of cancer-associated stroke has increased with the prolonged survival times of cancer patients. In this study, the researchers focused on the driver gene KRAS and found that KRAS mutations exacerbated neurological deficits after stroke and promoted tumor growth. They also identified the upregulation of pro-inflammatory responses and the promotion of STAT3 phosphorylation in mice with KRAS mutations. Additionally, the researchers found that colorectal cancer patients with stroke onset within 3 months after cancer diagnosis had a worse prognosis.

Safety of fixed-dose heparin titration boluses in underweight patients

Summary: This study evaluated the safety of fixed-dose heparin titration boluses in underweight patients with VTE. The results showed that although underweight patients required more frequent holding of heparin, there was no significant increase in bleeding risk.
