Clinical Assessment of Stereoscopic Optic Disc Photographs for Glaucoma: The European Optic Disc Assessment Trial

Purpose: To determine the diagnostic accuracy of judging optic disc photographs for glaucoma by ophthalmologists. Design: Evaluation of diagnostic test and technology. Participants: A total of 243 of 875 invited ophthalmologists in 11 European countries. Methods: We determined how well each participant classified 40 healthy eyes and 48 glaucomatous eyes with varying severity of the disease on stereoscopic slides. Duplicate slides were provided for determining intraobserver agreement. All eyes were also imaged with the GDx with variable corneal compensation (GDx-VCC) (Carl Zeiss Meditec AG, Jena, Germany) and the Heidelberg Retina Tomograph (HRT) I (Heidelberg Engineering GmbH, Heidelberg, Germany). Diagnostic accuracies of clinicians were compared with those of the best machine classifiers. Main Outcome Measures: Accuracy of classification, expressed as sensitivity, specificity, and overall accuracy. Intraobserver agreement (kappa). Results: The overall diagnostic accuracy of ophthalmologists was 80.5% (standard deviation [SD], 6.8; range, 61.4%-94.3%). The machine classifiers outperformed most observers in diagnostic accuracy; the GDx-VCC nerve fiber indicator and the HRT's best classifier correctly classified 93.2% and 89.8% of eyes, respectively. The intraobserver agreement (kappa) varied between -0.13 and 1.0 and was on average good (0.7). Conclusions: In general, ophthalmologists classify optic disc photographs moderately well for detecting glaucoma. There is, however, large variability in diagnostic accuracy among and agreement within clinicians. Common imaging devices outperform most clinicians in classifying optic discs. Summary: After 6 years of treatment, selective laser trabeculoplasty (SLT) has been shown to be a more effective and cost-effective primary treatment for open-angle glaucoma (OAG) and ocular hypertension (OHT) compared to intraocular pressure (IOP)-lowering eye drops. The SLT group had better glaucoma symptom scores and 69.8% of eyes in this group achieved or maintained the target IOP without the need for medical or surgical treatment.

Ageing changes in retinal outer nuclear layer thickness and cone photoreceptor density using adaptive optics-free imaging

Summary: This study investigated the age-related changes of outer nuclear layer (ONL) thickness and cone density in healthy participants using a modified narrow scan-angle Heidelberg Retina Angiograph (HRA2). The results showed that both cone density and ONL thickness decreased with age. However, there was no strong correlation between cone density and ONL thickness, suggesting that factors other than cone density contribute to ONL thickness measurements.

Nd:YAG laser capsulotomy rates in the Netherlands: practice variation and association with physician practice styles

Summary: The purpose of this study was to examine the associations between alcohol consumption and glaucoma and related traits, assess whether genetic predisposition to glaucoma modified these associations, and perform MR experiments to investigate causal effects. The findings showed consistent and adverse associations between alcohol intake and glaucoma and related traits, even at levels below the current guidelines in the UK and the US.

Summary: This study investigated self-reported dual sensory impairment (DSI) in European population and found higher prevalence of DSI in Eastern and southern countries. Social isolation and poor self-rated health status were associated with DSI, while higher income was associated with lower risk. Human Development Index explained almost 38% of the variance of age-adjusted prevalence of DSI.

Summary: This study analyzed retinal vascular parameters in type 1 diabetes patients without diabetic retinopathy using OCT-A and found lower vascular density in the SCP. Additionally, there was a correlation between central vascular density in the DCP and total daily insulin intake, and the use of the FSL device was associated with higher vascular densities in both the SCP and DCP.

A Case of Bilateral Choroidal Effusion after XEN Gel Stent Implantation

Summary: This study describes a rare case of bilateral choroidal effusion following a XEN45 implantation. The patient experienced choroidal detachment and hypotony in both eyes, which were successfully treated. The case highlights the importance of careful postoperative follow-up and timely intervention in XEN45 implantation cases, as well as the potential risk factor of choroidal effusion in the fellow eye.

The Role of Mitophagy in Glaucomatous Neurodegeneration

Summary: This review aims to provide a better understanding of the emerging role of mitophagy in glaucomatous neurodegeneration, which is the primary cause of irreversible blindness worldwide. Increasing evidence from genetic and other experimental studies suggests that mitophagy-related genes are implicated in the pathogenesis of glaucoma in various populations. The association between polymorphisms in these genes and increased risk of glaucoma is presented.
