Purpose: To describe the disease characteristics and visual outcome of pediatric uveitis. Design: Retrospective, longitudinal observation. Participants. Five hundred twenty-seven pediatric uveitis patients from the National Eye Institute, University of Illinois, Chicago, and Oregon Health Sciences University. Methods: Retrospective chart review. Main Outcome Measures: Demographics, uveitis disease characteristics, complications, treatments, and visual outcomes were determined at baseline and at 1-, 3-, 5-, and 10-year time points. Results: The patient population was 54% female; 62.4% white, 12.5% black, 2.7% Asian, 2.1% multiracial, and 14.61% Hispanic. Median age at diagnosis was 9.4 years. The leading diagnoses were idiopathic uveitis (28.8%), juvenile idiopathic arthritis-associated uveitis (20.9%), and pars planitis (17.1%). Insidious onset (58%) and persistent duration (75.3%) were most common. Anterior uveitis was predominant (44.6%). Complications were frequent, and cystoid macular edema (odds ratio [OR] 2.94; P = 0.006) and hypotony (OR, 4.54; P = 0.026) had the most significant visual impact. Ocular surgery was performed in 18.9% of patients. The prevalence of legal blindness was 9.23% at baseline, 6.52% at 1 year, 3.17% at 3 years, 15.15% at 5 years, and 7.69% at 10 years. Posterior uveitis and panuveitis had more severe vision loss. Hispanic ethnicity was associated with a higher prevalence of infectious uveitis and vision loss at baseline. Conclusions: The rate and spectrum of vision threatening complications of pediatric uveitis are significant. Prospective studies using standard outcome measures and including diverse populations are needed to identify children most at risk. Summary: The Collaborative Ocular Tuberculosis Study (COTS), supported by international organizations, established expert-led consensus project to develop evidence- and experience-based guidelines for the management of tubercular uveitis (TBU). The study results indicate that experts unanimously agree to initiate anti-tuberculosis therapy in patients with TBU under specific circumstances.

Changing Incidence and Survival of Primary Central Nervous System Lymphoma in Australia: A 33-Year National Population-Based Study

Alexandra L. Farrall, Justine R. Smith

Summary: This study analyzed the incidence and survival outcomes of primary central nervous system lymphoma (PCNSL) in Australia using data from the Australian Cancer Database. The incidence of PCNSL has been increasing at an average annual rate of 6.8% over the past four decades, with an overall low survival rate of only 33% at 5 years after diagnosis. Additionally, the study found that the increase in PCNSL incidence was driven by the diffuse large B-cell lymphoma (DLBCL) type, with survival outcomes improving but still lower than for non-CNS lymphoma.

Infection of Human Retinal Pigment Epithelial Cells with Dengue Virus Strains Isolated during Outbreaks in Singapore

Summary: The prevalence of dengue retinopathy varies across epidemics, and is linked to the circulation of dengue virus serotype 1 (DENV-1). This study investigated the infectivity, molecular response, and barrier function of epithelial cells infected with DENV strains from different outbreaks in Singapore. The results showed that DENV-1 strains from the 2005 epidemic, when retinopathy was prevalent, had higher viral RNA loads and up-regulated interferon transcripts compared to DENV-1 strains from the 2007 epidemic when retinopathy was not observed. The strain of DENV may play a role in the development of retinal pathology.

Dengue Virus Infection of Human Retinal Muller Glial Cells

Genevieve F. Oliver, Liam M. Ashander, Abby C. Dawson, Yuefang Ma, Jillian M. Carr, Keryn A. Williams, Justine R. Smith

Summary: Retinopathy is a recognized complication of dengue, affecting hospitalized patients up to 10%. This study aimed to investigate the susceptibility of Muller cells to dengue virus (DENV) infection and their antiviral, inflammatory, and immunomodulatory responses in vitro. It was found that Muller cells can be infected by DENV and mount an immune response, but the response varies across different cell isolates and DENV strains. This research provides insights into the role of retinal Muller glial cells in dengue retinopathy.

Summary: Intercellular adhesion molecule 1 (ICAM-1) is a transmembrane protein that plays important roles in the immune system and is involved in various diseases. It mediates cellular adhesive interactions and is upregulated in inflammation. This review provides a comprehensive summary of the structure and regulation of ICAM1 gene and the ICAM-1 protein, discusses its functions in the immune system and diseases, and explores current therapeutics and future advancements.

Oxidative Stress and Its Regulation in Diabetic Retinopathy

Cameron D. Haydinger, Genevieve F. Oliver, Liam M. Ashander, Justine R. Smith

Summary: Diabetic retinopathy is a retinal disease associated with hyperglycemia in diabetes patients, affecting both the central and peripheral retina. Its major mechanism involves oxidative stress caused by elevated production of reactive oxygen species, which is particularly harmful to the retina due to its high metabolic activity. This review provides an overview of the role of oxidative stress in diabetic retinopathy, including the formation and effects of advanced glycation end-products, metabolic memory, and non-coding RNA, as well as potential therapeutic strategies.
