microRNA expression profile in stage III colorectal cancer: Circulating miR-18a and miR-29a as promising biomarkers

Biomarkers that can facilitate disease detection, staging and prediction of outcome are highly desirable to improve survival and to help determine optimized treatment for colorectal cancer patients. microRNAs (miRNAs) are small non-coding RNAs that play a crucial role in gene regulatory networks. The deregulation of miRNA expression has been found in several types of cancer and may represent a novel class of cancer biomarkers. Our aim was to determine the miRNA signature of stage III colorectal cancer (CRC) tumors and to identify potential circulating miRNAs that may represent non-invasive biomarkers in CRC patients. Genome-wide microarray analysis of miRNA expression was performed on 12 paired tumor and non-tumor formalin-fixed paraffin-embedded tissues from stage III CRC patients. A selection of differentially overexpressed miRNAs was validated by quantitative real-time polymerase chain reaction (qRT-PCR) and determined in the serum of a set of 56 individuals (30 stage III CRC patients and 26 healthy individuals). Using 1.5-fold expression difference as a cut-off level, 43 miRNAs were identified as differentially expressed in tumor versus normal tissue. Using reverse transcription and qRT-PCR, 11 miRNAs (miR-135b, miR-141, miR-18a, miR-20a, miR-21, miR-224, miR-29a, miR-31, miR-34a, miR-92a and miR-96) were confirmed as significantly overexpressed in tumor samples when compared with normal samples. We were able to detect 9 of these 11 miRNAs in serum samples from CRC patients and healthy individuals. Serum levels of miR-18a and miR-29a were significantly higher in CRC patients when compared to levels in the controls (p<0.05). In conclusion, this study identified a substantial number of miRNAs which were differentially expressed in stage III colorectal tumors. Moreover, the findings provide relevant information concerning overexpressed tumoral miRNAs as potential circulating biomarkers and highlight serum miR-18a and miR-29a as promising biomarkers for the screening and monitoring of CRC patients.

Accreditation of specialized surgical units in general and digestive surgery: A step forward by the AEC for quality improvement and subspecialized Fellowship training

However, there has been no model for accreditation or recognition of these units or fellowship programs. The AEC has developed a regulation for the accreditation of specialized surgical units in GDS, which will serve as a model for defining subspecialty training in these areas. The accreditation process involves various quality indicators, including unit structure, process quality, and outcome indicators.

Summary: Transanal endoscopic microsurgery (TEM) allows the local excision of rectal tumors and achieves lower morbidity and mortality rates than total mesorectal excision. TEM can treat lesions up to 18 to 20 cm from the anal verge, obtaining good oncological results in T1 stage cancers and preserving sphincter function. However, TEM is technically demanding, especially for large lesions, those with high risk of perforation, those in the upper rectum or rectosigmoid junction, and those in the anal canal.

Summary: This study aims to determine the optimal time and distance for evaluating the fluorescence saturation of indocyanine green (ICG). The results showed that the fluorescence intensity was highest and relatively stable between 1.5 and 3.5 minutes after ICG administration, making it suitable for assessing vascularization. Additionally, the results obtained were most accurate when the distance between the camera and the tissue was 5 cm.

Summary: The purpose of this study was to analyze the safety and feasibility of laparoscopic and robotic surgery in upper rectum, sigmoid, and left colon resection and anastomosis, and to compare the short-term differences between these two approaches. The results showed that both laparoscopic and robotic intracorporeal resection and anastomosis were feasible and safe, and achieved similar surgical, postoperative, and pathological outcomes as described in the literature. However, there seemed to be more complications in the laparoscopic group.

Parastomal hernia prevention with permanent mesh in end colostomy: failure with late follow-up of cohorts in three randomized trials

Summary: This study aimed to evaluate the long-term occurrence of parastomal hernia (PH) after the use of a prophylactic synthetic non-absorbable mesh in elective end colostomy. Long-term data from three randomized controlled trials were collected and analyzed. The results showed that the presence of a prophylactic mesh did not significantly reduce the development of PH in the overall study population, but the open retromuscular technique had a higher reduction in PH occurrence.

Transanal versus laparoscopic total mesorectal excision for mid and low rectal cancer (Ta-LaTME study): multicentre, randomized, open-label trial

Summary: This study suggests that laparoscopic TaTME can avoid conversion to open surgery and has a significantly lower conversion rate compared to laparoscopic total mesorectal excision (LaTME), especially when performed by experienced surgeons. There were no significant differences in postoperative recovery and complications between the two groups, but laparoscopic TaTME had a lower incidence of anastomotic leakage.
