The age- and shorter telomere-dependent TERT promoter mutation in follicular thyroid cell-derived carcinomas

Telomerase activation through induction of its catalytic component telomerase reverse transcriptase (TERT) expression is essential for malignant transformation. TERT promoter mutations namely C228T and C250T that stimulate TERT transcription and telomerase activation have recently been identified in many human malignancies. We thus determined these mutations and their biological and clinical implications in thyroid carcinomas in the present study. The TERT promoter was sequenced in 10 thyroid cancer cell lines and 144 tumors from 20 patients with anaplastic thyroid carcinoma (ATC), 51 with papillary thyroid carcinoma (PTC), 36 with follicular thyroid carcinoma (FTC), and 37 with medullary thyroid carcinoma (MTC). We identified C228T or C250T mutation in 6/8 of ATC cell lines, as well as in tumor tissue from 10/20, 13/51, 8/36 and 0/37 patients with ATC, PTC, FTC and MTC, respectively. In PTC patients, these mutations were exclusively present in the group with age 445 years (P<0.0001), and highly correlated shorter telomeres (P<0.0001) and distant metastasis (P = 0.028). The previous radioactivity exposure did not induce the mutation. The presence of C228T or C250T was an independent predictor associated with shorter disease-related survival (DRS) in the entire cohort (P<0.0001), as well as among patients 445 years (P = 0.021). ATC patients carrying the mutation survived shorter than those without mutations, although not statistically significant (P = 0.129). The TERT promoter mutation was associated with overall survival (P = 0.038) and DRS (P = 0.058) of FTC patients. Taken together, age-and shorter telomere-dependent TERT promoter mutations occur frequently in follicular cell-derived thyroid carcinoma (ATC, PTC and FTC) but not in parafollicular cell-originated MTC, and may serve as a marker for aggressive disease and poor outcome.

This article presents a case of successful biliary decompression and closure of an external biliary fistula in a 23-year-old male with a thoracoabdominal penetrating gunshot wound.

The association of telomere maintenance and TERT expression with susceptibility to human papillomavirus infection in cervical epithelium

Widespread genomic/molecular alterations of DNA helicases and their clinical/therapeutic implications across human cancer

Summary: DNA helicases play a crucial role in maintaining genomic stability, and mutations in these genes can lead to genomic instability and cancer susceptibility. Interestingly, overexpression of DNA helicases is observed in certain cancers. In this study, genomic and molecular alterations in 12 important DNA helicases were analyzed across various types of cancers. The study revealed significant expression heterogeneity among these helicases, and tumors were classified into subtypes based on their DNA helicase score. The high DNA helicase score subtypes were associated with stemness, proliferation, oncogenic signaling, longer telomeres, more mutations, copy number alterations, and shorter survival. Furthermore, high DNA helicase scores were linked to sensitivity to chemo- and radio-therapies, as well as defects in DNA repair pathways like homologous recombination deficiency. Importantly, a potential DNA helicase inhibitor, Danusertib, was identified in this study. These findings highlight the dysregulation of DNA helicases in cancer and their potential as therapeutic targets.

Gunshot injury to the colon by expanding bullets in combat patients wounded in hybrid period of the Russian-Ukrainian war during 2014-2020

Summary: This study evaluated the clinical features of colon injuries in combat patients in East Ukraine and compared the differences between injuries caused by shape-stable and hollow-point bullets. The results showed that injuries caused by hollow-point bullets were more severe, with higher rates of hemorrhagic shock, left colon involvement, and larger areas of colon defects. The mortality rate was also higher in patients with hollow-point bullet injuries.

Temporary arterial shunts in combat patient with vascular injuries to extremities wounded in Russian-Ukrainian war: A case report

DKC1 serves as an independent female-specific predictor for survival and Sunitinib efficacy.

Refixation of the Excised Acetabular Labrum can Affect the Structural Changes of Cartilage Surface in the Hip Joint

Summary: Refixation of the damaged acetabular labrum is a surgical treatment that can promote joint function repair and prevent premature osteoarthritis. This study examined the condition of the hip joint in rabbits 4 months after labrum excision and after labral refixation. Various methods, including histological examination and measurement of cartilage thickness, were used to assess the condition of the articular cartilage. The results showed significant differences between the condition of the cartilage after excision and after refixation, suggesting that this surgical technique can delay or prevent progressive changes in the damaged hip joint cartilage.

Penetrating gunshot wounds to the penis: a case report of combat patient injured in the war in Ukraine

Summary: This report presents a case of successful penile salvage in a combat patient with gunshot genitourinary trauma, despite the interrupted supply of medical equipment. The patient underwent primary surgical debridement and suturing of multiple injuries, leading to functional recovery. This case sheds light on the management of penile gunshot injuries in warfare with limited medical resources.

Challenges of Differential Diagnosis Between Primary Hyperparathyroidism and Bone Metastases of Breast Cancer
