It is a well-known fact that severe obesity is associated with the metabolic syndrome, type 2 diabetes, cardiovascular disease, hypertension, and other diseases. Epidemiological studies have suggested that obesity is also associated with increased risk of several cancer types. The number of people who are suffering from severe obesity is growing, and clinical data suggest that severely obese patients belong to a unique population with regards to risk, efficacy of screening, and cancer treatment. This review will point out the potential mechanism linking obesity and cancer and will discuss several challenges in various treatment modalities of cancer in obese patients.

Summary: This study found that obesity is independently associated with polyneuropathy in subjects without diabetes. However, in subjects with diabetes, although the association between obesity and polyneuropathy is not significant after adjusting for conventional risk factors, increased fibrotic gene expression in fat is associated with polyneuropathy.

Summary: This study investigated the relationship between the growth of hematopoietic clones of different sizes and age and metabolic dysregulation in obese individuals receiving usual care or bariatric surgery treatment. The results showed that the size of clones in obese individuals increased with age, but not in those who underwent bariatric surgery. Furthermore, low HDL cholesterol levels were negatively associated with clone growth.

Metabolic Profiling of Obesity With and Without the Metabolic Syndrome: A Multisample Evaluation

Summary: This study compared the plasma metabolome in obese subjects with and without metabolic syndrome (MetS) to normal-weight subjects without MetS. The results showed significant differences in metabolite profiles between obese and normal-weight individuals without MetS, with a gradual deterioration in metabolite profile from lean to obese to MetS. The pathways highlighted in this study, such as amino acid synthesis/metabolism and sphingolipid metabolism, may play a role in this metabolic deterioration, with insulin resistance as a potential mediator.

Sheehan syndrome: Cardiovascular and metabolic comorbidities

Summary: Sheehan syndrome caused by postpartum hemorrhage leads to pituitary hormone deficiency, resulting in various abnormalities including lipid and glucose abnormalities, increased body fat, insulin resistance, coagulation abnormalities, increased leptin concentration, low-grade inflammation, and endothelial dysfunction. Replacement with growth hormone in addition to standard hormone replacement improves the cardiometabolic profile of patients with Sheehan syndrome.

Summary: Researchers have developed a GLP-1/GDF15 fusion protein and tested its potential for weight loss in animals. The molecule, QL1005, showed higher potency than the GLP-1 analog semaglutide in vitro. In obese mice, QL1005 led to reductions in body weight, food intake, insulin, fasting glucose, and triglycerides. In a cynomolgus monkey model of obesity, QL1005 dose-dependently reduced body weight, food intake, insulin, and glucose with limited side effects.

Cancer patients attending treatment during COVID-19: intolerance of uncertainty and psychological distress

Summary: This study aimed to assess the experience, perceptions, and reactions of cancer patients during the COVID-19 pandemic using a mixed-methods design. The findings suggest that most cancer patients viewed the pandemic as a major threat, but their level of psychological distress was low. Health experts should pay attention to the emotional and support needs of cancer patients and identify those with high intolerance of uncertainty as a risk factor for psychological distress in times of extreme uncertainty like COVID-19.

Advance Care Planning and Attainment of Cancer Patients' End-of-Life Preferences: Relatives' Perspective

Summary: The purpose of this study is to determine whether an advance directives (AD) document or an end-of-life discussion between patient and family (DwF) improves cancer patients' likelihood of receiving preferred treatments and dying in their preferred place. The study found that conducting a DwF and having an AD document were significantly associated with treatment consistency and preferred place of death. The study highlights the importance of advance care planning in improving end-of-life care.

The Humoral Response of Patients With Cancer to Breakthrough COVID-19 Infection or the Fourth BNT162b2 Vaccine Dose

Summary: Since January 2022, high-risk populations in Israel have been advised to receive a fourth dose of the BNT162b2 vaccine for protection against SARS-CoV-2. This study monitored the vaccine-induced immunity among oncology patients undergoing anti-cancer therapy before and after the fourth dose of BNT162b2. The results showed that antibody levels were higher in patients who had breakthrough infections or received the fourth dose, compared to the control group.
