New Echocardiographic Techniques in the Evaluation of Left Ventricular Function in Obesity

Objective: Obesity has reached global epidemic proportions and is associated with numerous comorbidities, including major cardiovascular (CV) diseases. Design and Methods: It has many adverse effects on hemodynamics and CV structure and function: it increases total blood volume and cardiac output, and the cardiac workload is greater. Typically, obese patients have a higher cardiac output but a lower level of total peripheral resistance at any given level of arterial pressure. Most of the increase in cardiac output in obesity is caused by stroke volume, although heart rate typically mildly increases also due to enhanced sympathetic activation. Results: Over the last few years, experimental investigations have unraveled some important pathogenetic mechanisms that may underlie a specific form of obesity cardiomyopathy. Bariatric surgery represents an effective alternative to treat obesity when nonsurgical weight loss programs (diet + behavior modifications + regular exercise) have failed. A great numbers of questions are still open in the global comprehension of the pathophysiological interactions between obesity and heart. Conclusion: Conventional two-dimensional Doppler echocardiography, integrated by relative new technological ultrasonic approaches, represents the reference technique to study and possibly clarify both the very complex hemodynamic changes induced by obesity and those relative to obesity treatment.

COVID-induced thyroid autoimmunity

Summary: The breakdown of self-tolerance to thyroid antigens is the main cause of thyroid autoimmunity. It has been proposed that infectious diseases may trigger autoimmune thyroid diseases. This review focuses on the association between SARS-CoV-2 infection and the occurrence of autoimmune thyroid diseases. Nine cases of Graves' disease and three cases of Hashimoto's thyroiditis associated with SARS-CoV-2 infection have been reported.

Summary: The European Society for Clinical Nutrition and Metabolism (ESPEN) and the European Association for the Study of Obesity (EASO) launched the Sarcopenic Obesity Global Leadership Initiative (SOGLI) to reach expert consensus on a definition and diagnostic criteria for Sarcopenic Obesity (SO). The present paper describes the proceeding of the Sarcopenic Obesity Global Leadership Initiative (SOGLI) meeting that was held on November 25th and 26th, 2022 in Rome, Italy. This consortium involved the participation of 50 researchers from different geographic regions and countries. The document outlines an agenda advocated by the SOGLI expert panel regarding the pathophysiology, screening, diagnosis, staging, and treatment of SO that needs to be prioritized for future research in the field.

Thyroid autoimmunity, thyroglobulin autoantibodies, and thyroid cancer prognosis

Summary: The relevance of thyroid autoimmunity to the prognosis of papillary thyroid carcinoma is still uncertain. This study found that thyroid autoimmunity does not affect the prognosis of papillary thyroid carcinoma, but detectable thyroglobulin antibodies (TgAbs) can prolong the time to remission. A decreasing TgAb trend seems to be an appropriate criterion for establishing remission of papillary thyroid carcinoma.

Summary: Prognosis is good for papillary thyroid carcinoma, noninvasive follicular thyroid neoplasia with papillary-like nuclear features, and follicular thyroid carcinoma, but poor for poorly differentiated thyroid carcinoma and anaplastic thyroid carcinoma. Ultrasound can be helpful in ruling out tall cell variant papillary thyroid carcinoma.

Transcriptional changes in multiple endocrine organs from lethal cases of COVID-19

Summary: Altered gene expression in endocrine organs is observed in COVID-19, with enhanced interferon response and specific changes in endocrine-specific genes. Adipose tissue shows independent enhanced interferon response regardless of virus presence. Important genes such as insulin, thyroid-stimulating hormone receptor, and leptin are affected in COVID-19.

Outcomes of the Tall-Cell Variant of Papillary Thyroid Carcinoma in Patients with Different Ages: A 17-Year Mono-Institutional Experience

Summary: This study analyzed the clinicopathological characteristics of 989 patients with tall-cell variant of papillary thyroid carcinoma (TCPTC) and found that this type of cancer is not exclusively found in older patients. The study also revealed higher rates of local recurrence in the younger and older age groups, and a higher rate of distant recurrence in older patients. The results suggest that the prognosis of TCPTC is age-dependent and further studies are needed.

Oxidative and DNA damage in obese patients undergoing bariatric surgery: A one-year follow-up study

Summary: The pathogenesis of obesity and related comorbidities is associated with oxidative stress. Excessive adipose tissue produces free radicals that cause chronic inflammation and oxidative stress, which can be improved by reducing adipose tissue. In our study, we evaluated cell membrane oxidation and DNA damage in lymphocytes of normal weight controls and obese patients before and after bariatric surgery. We found that obese patients had increased oxidative damage and DNA double strand breaks before surgery, but these markers improved after surgery.

Summary: This article describes a rare lipodystrophy syndrome called congenital generalized lipodystrophy type 4 (CGL4), which is caused by mutations in the PTRF gene. Two early childhood patients presented with features such as reduced subcutaneous fat, muscular hypertrophy, distinctive facial features, myopathy, and atlantoaxial instability. Blood tests showed elevated CK and triglyceride levels, and undetectable leptin and adiponectin concentrations.

Sensitivity Analysis of Whole-Room Indirect Calorimeters at the Steady-State Condition

Summary: The aim of this study is to investigate the sensitivity of a Whole-room indirect calorimeter (WRIC) system to determine the optimal experimental conditions for accurate metabolic measurements. The results suggest that maintaining a fractional concentration of carbon dioxide inside the calorimeter >0.2% can lead to a relative uncertainty <5% for outcome metabolic measurements, assuming an accuracy class of 1% for gas analyzer instruments.

The southern region had a higher prevalence of comorbidities such as diabetes, hypertension, chronic pulmonary and kidney diseases.
