A Randomized Trial of Planned Cesarean or Vaginal Delivery for Twin Pregnancy

BackgroundTwin birth is associated with a higher risk of adverse perinatal outcomes than singleton birth. It is unclear whether planned cesarean section results in a lower risk of adverse outcomes than planned vaginal delivery in twin pregnancy. MethodsWe randomly assigned women between 32 weeks 0 days and 38 weeks 6 days of gestation with twin pregnancy and with the first twin in the cephalic presentation to planned cesarean section or planned vaginal delivery with cesarean only if indicated. Elective delivery was planned between 37 weeks 5 days and 38 weeks 6 days of gestation. The primary outcome was a composite of fetal or neonatal death or serious neonatal morbidity, with the fetus or infant as the unit of analysis for the statistical comparison. ResultsA total of 1398 women (2795 fetuses) were randomly assigned to planned cesarean delivery and 1406 women (2812 fetuses) to planned vaginal delivery. The rate of cesarean delivery was 90.7% in the planned-cesarean-delivery group and 43.8% in the planned-vaginal-delivery group. Women in the planned-cesarean-delivery group delivered earlier than did those in the planned-vaginal-delivery group (mean number of days from randomization to delivery, 12.4 vs. 13.3; P=0.04). There was no significant difference in the composite primary outcome between the planned-cesarean-delivery group and the planned-vaginal-delivery group (2.2% and 1.9%, respectively; odds ratio with planned cesarean delivery, 1.16; 95% confidence interval, 0.77 to 1.74; P=0.49). ConclusionsIn twin pregnancy between 32 weeks 0 days and 38 weeks 6 days of gestation, with the first twin in the cephalic presentation, planned cesarean delivery did not significantly decrease or increase the risk of fetal or neonatal death or serious neonatal morbidity, as compared with planned vaginal delivery. In this randomized trial comparing delivery strategies in women with twin gestation, planned cesarean section did not significantly increase or decrease the risk of fetal or neonatal death or serious neonatal morbidity, as compared with planned vaginal delivery. Because of assisted reproductive technologies, twin pregnancy occurs more frequently now than in the past, and it complicates 2 to 3% of all births.(1),(2) Twins are at higher risk for an adverse perinatal outcome than singletons.(3),(4) Planned cesarean section, as compared with planned vaginal delivery, may reduce this risk.(5) Although a small, randomized, controlled trial did not show better perinatal outcomes with planned cesarean section than with planned vaginal delivery,(6) several cohort studies have shown a reduced risk of adverse perinatal outcomes for both twins, or for the second twin, when twins at or near term were delivered

One-year follow-up of immediate postpartum contraceptive implant insertion in adolescents

Summary: This study evaluated the acceptability, adverse effects, and continuation rates of the etonogestrel (ENG) subdermal implant among adolescents compared to other contraceptive methods after childbirth. Results showed that students were more likely to accept the ENG implant and had a longer adherence time. The majority of adolescents were satisfied with the implant, but a few requested early removal due to menstrual irregularity and local discomfort.

Childhood follow-up of the GRoW randomized trial: Metformin in addition to dietary and lifestyle advice for pregnant women with overweight or obesity

Pregnancy and postpartum experiences of women under- going hemodialysis: a qualitative study

Summary: This study explores the experiences of women with CKD undergoing hemodialysis in relation to their reproductive history. The results reveal that pregnancy complications may cause or worsen CKD and being a woman on hemodialysis comes with additional burdens. Therefore, considering the specificities of CKD in women is important for diagnosis and treatment adherence, and the reproductive life history of women on hemodialysis should be taken into consideration for promoting their holistic health, including mental health aspects.

Gestational hypertension as a factor associated with chronic kidney disease: the importance of obstetric history of women undergoing hemodialysis

Summary: This study evaluated the sociodemographic, clinical, and obstetric history of women undergoing hemodialysis. The results showed that hypertension was the main cause of chronic kidney disease, and pregnant women had higher rates of adverse perinatal outcomes. Hypertension was associated with gestational hypertension and hypertension up to one year after birth. Understanding the reproductive history of pregnant women can help delay the progression of end-stage renal disease.

Antenatal cardiotocography in primary midwife-led care: Women's satisfaction

Summary: This study examines the satisfaction and experiences of pregnant women who received antenatal cardiotocography (aCTG) in midwife-led care and explores factors associated with high satisfaction. The results show that most women were highly satisfied with receiving aCTG in midwife-led care, with a mean satisfaction score of 9.2. Women between 33 and 36 weeks' gestation were more likely to be highly satisfied, while a less comfortable position during the aCTG decreased the odds of being highly satisfied.

Acceptance rate of post-placental plac ement of intrauterine devices during the SARS-CoV-2 (COVID-19) pandemic

Summary: Post-placental IUDs were found to be a viable alternative during the COVID-19 pandemic. The acceptance rate of PPIUD was 65.6%, with the main reason for refusal being the desire for another contraceptive. Younger women, those without a partner, and those who had a vaginal delivery were more likely to accept PPIUD.

Perceptions of Brazilian women at a public obstetric outpatient clinic regarding domestic violence: a qualitative study

Summary: The study aimed to explore women's perceptions of violence, its causes, manifestations, consequences, and responses to prevent and confront domestic violence against women in Brazilian society. A qualitative study with individual, semistructured interviews was conducted in an antenatal and postnatal care service in the Brazilian National Health System. The findings revealed that Brazilian women during pregnancy and the postpartum period had a multifaceted view of domestic violence, highlighting the difficulties in interrupting the cycle of violence and accessing support networks.
