Voiding Dysfunction in Patients With Parkinson's Disease: Impact of Neurological Impairment and Clinical Parameters

Aims: We assessed the lower urinary tract symptoms (LUTS) of patients with Parkinson's disease (PD) and their association with different clinical parameters. Methods: We prospectively evaluated 110 patients (84 men), with a mean age of 61.8 +/- 9.6 years. Mean duration of the disease was 12.3 +/- 7.2 years. Neurological impairment was assessed by the Hoehn-Yahr and the Unified Parkinson Disease Rating scales. LUTS were assessed by the International Continence Society questionnaire. We evaluated the impact of age, PD duration, neurological impairment, gender, and use of anti-Parkinsonian drugs on the voiding function. Results: On multivariate analysis, voiding dysfunction increased with the neurological impairment, but not with patient's age or disease duration. Quality of life (QOL) was affected by the severity of LUTS, and the symptoms with the worst impact were frequency and nocturia. Sixty-three (57.2%) patients were symptomatic. They did not differ with the asymptomatic as to age and disease duration, but had more severe neurological impairment. No impact on LUTS was associated with the use of levodopa, anticholinergics, and dopamine receptor agonists. Men and women were similarly affected by urinary symptoms. Conclusions: The severity of the neurological disease is the only predictive factor for the occurrence of voiding dysfunction, which affects men and women alike. Neztrourol. Management of functioning pediatric adrenal tumors

Summary: This study retrospectively analyzed 75 children with hormonally active adrenal tumors who were evaluated at the endocrinology clinic and operated at the institution between 1983 and 2019. The results showed that proper diagnosis and management at the referral center were associated with a high cure rate, even in cases of malignant tumors, highlighting the importance of familial surveillance.

Summary: The pure laparoscopic HidES technique proved to be safe, feasible, and not inferior to traditional nephrectomy, with better cosmesis and higher patient satisfaction rates. The shorter operative time and lower blood loss in HidES group indicate its potential advantages over Traditional Ports Placement (TPP) nephrectomy. The study suggests that the location of scars plays a significant role in patient and parental satisfaction, supporting the benefit of HidES procedure.

Modern management of and update on prune belly syndrome

Urinary extracellular matrix proteins as predictors of the severity of ureteropelvic junction obstruction in children

Summary: This study found that urinary ECM proteins can help differentiate the severity of urinary obstruction in children with unilateral UPJO. In the obstructive HN group, the urinary concentrations of uTIMP-1/Cr and uTIMP-2/Cr decreased postoperatively, while uMMP-1/Cr, uMMP-2/Cr, and uMMP-9/Cr increased. Protein analysis in the non-obstructive HN group remained stable after six months of observation. ROC curve analysis showed promising diagnostic values for TIMP-1/Cr, TIMP-2/Cr, and MMP-2/Cr.

Summary: This study evaluated 50 patients with retained encrusted ureteral stents who underwent a single session combined endourological approach. The FECal classification was useful for surgical planning, and the stone-free rate was lower in grades III to V. Percutaneous nephrolithotomy was the most common procedure for grades IV and V. The endoscopic combined approach in the supine position was safe and feasible for removal of retained and encrusted stents in a single procedure.

Summary: Comparative study on two surgical approaches for 44 patients with symptomatic duplex kidneys showed that extended distal ureteral dissection can reduce the risk of distal ureteral stump syndrome and decrease the proportion of patients requiring further surgical treatment.

Summary: Pediatric adrenocortical tumors (PACTs) are rare malignancies, but they have a high incidence in the south/southeast regions of Brazil due to a germline pathogenic variant of tumor suppressor gene TP53. This retrospective analysis of PACT patients identified several factors, including age, hormone levels, tumor characteristics, histological features, and genetic factors, that were associated with a poor prognosis. These results can help identify patients who are likely to have an aggressive disease course and guide treatment decisions.
