The use of extracellular microRNAs (miRNAs) as circulating biomarkers is currently leading to relevant advances in the diagnosis and assessment of prognosis of several diseases. Specific miRNAs have also been shown to play a role in the pathophysiology of many neoplastic and non-neoplastic diseases. A number of studies have demonstrated that miRNAs show differential expression in various tumors, such as in the prostate, ovary, lung, breast, brain, and pituitary. Recent findings have built connections between miRNAs that are deregulated within the tumor and their presence in peripheral blood. MiRNAs have been shown to be stable in the blood where they are present in either free and/or uncomplexed form, as well as packed in microvesicles, exosomes, and apoptotic bodies, or bound to different proteins. Because the pituitary is a highly vascularized organ that releases hormones into the circulation, miRNAs would be useful biomarkers for the diagnosis of pituitary tumors, as well as for predicting or detecting recurrence after surgery. Here we review the biological significance of miRNAs in pituitary tumors and the potential value of circulating miRNAs as biomarkers.

Whorling Sclerosing Ependymoma of the Cervical Spinal Cord Presenting Tanycytic Histopathologic Features: A Rare Case Report

Cumhur Kaan Yaltirik, Emin Oguzcan Yamaner, Oznur Suakar, Sezin Gurkan, Aydin Sav, Ugur Ture

Summary: Tanycytic ependymoma is a rare neuroectodermal tumor that originates from ependymoglial cells or tanycytes. This article reports a case of a 34-year-old male diagnosed with intradural-intramedullary tanycytic ependymoma at the C4-5 level, presenting with a 9-month history of neck pain, left arm pain, and finger numbness. The lesion was completely removed by surgery and histopathological examination revealed fascicles forming nebula-like whorling structures.

Artificial Intelligence-Assisted Classification of Gliomas Using Whole Slide Images

Laya Jose, Sidong Liu, Carlo Russo, Cong Cong, Yang Song, Michael Rodriguez, Antonio Di Ieva

Summary: A deep learning-based approach for automated classification of glioma histopathology images was proposed in this study. Two different deep neural network architectures were tested, and the multiclass method outperformed the ensemble method. The proposed method can assist in the diagnosis and classification of glioma histopathology images.

Chung, Albert Lee

Summary: This study reveals the role of cyclin F in regulating substrate solubility and its contribution to the pathogenesis of ALS and FTD. The ALS and FTD-associated protein p62 is identified as a substrate of cyclin F, and its aggregation is promoted by cyclin F expression. Mutations in cyclin F result in dysregulated p62 solubility and formation of p62 foci, which are associated with ALS and FTD pathogenesis.

Determining Contemporary Barriers to Effective Multidisciplinary Team Meetings in Neurological Surgery: A Review of the Literature

Miracle C. Anokwute, Veronica Preda, Antonio Di Ieva

Summary: This study reviewed the literature on multidisciplinary team meetings (MDTMs) in neurosurgical care and identified limiting factors that may introduce bias to patient care. Productive MDTMs were found to involve quaternary/tertiary referral centers, a nonhierarchical environment, regularly scheduled meetings, inclusion of nonmedical factors, and use of clinical guidelines and institutional protocols. However, the use of artificial intelligence to reduce bias was not described in the literature.

Cellular and Molecular Mechanisms Underlying Tricuspid Valve Development and Disease

Nadia Salerno, Giuseppe Panuccio, Jolanda Sabatino, Isabella Leo, Michele Torella, Sabato Sorrentino, Salvatore De Rosa, Daniele Torella

Summary: Tricuspid valve (TV) disease is highly prevalent and has received significant attention in recent years. Comprehensive knowledge of molecular and cellular mechanisms underlying TV development and disease is necessary for better understanding and treatment of tricuspid regurgitation patients. Scientific efforts are still needed to fully understand the etiopathogenesis of TV and TV-associated cardiomyopathy, and future advances may come from combining diagnostic imaging with molecular and cellular studies. Basic science research could provide the basis for innovative valve repair and regeneration using tissue-engineered heart valves.

Clinical Application of Circular RNAs as Biomarkers in Acute Ischemic Stroke

Chiara Siracusa, Niccolo Vono, Maria Benedetta Morano, Jolanda Sabatino, Isabella Leo, Ceren Eyileten, Eleonora Cianflone, Marek Postula, Daniele Torella, Salvatore De Rosa

Summary: Despite improvements in diagnosis and treatment, ischemic stroke remains a challenge with significant morbidity and mortality. The identification of high-risk individuals, timely diagnosis, recognition of different clinical forms, treatment response assessment, and prognostic evaluation are unmet clinical needs. This article provides an overview of circular RNAs as potential biomarkers in stroke, aiming to gather comprehensive information on this promising class of molecules.

Clinical and Procedural Outcomes of IVUS-Guided vs. Angiography-Guided CTO-PCI: A Systematic Review and Meta-Analysis

Giuseppe Panuccio, Youssef S. Abdelwahed, Nicole Carabetta, Nadia Salerno, David Manuel Leistner, Ulf Landmesser, Salvatore De Rosa, Daniele Torella, Gerald S. Werner

Summary: Chronic total occlusions (CTO) in coronary angiographies present a significant challenge. This meta-analysis aimed to compare IVUS-guided and angiography-guided approaches in CTO-PCI. The study included five studies and 2320 patients with stable CAD and CTO. The results showed that routine use of IVUS did not significantly improve clinical outcomes in CTO-PCI, except for reducing stent thrombosis. Decisions in CTO-PCI should be individualized based on patient characteristics and supported by a multi-parametric approach.
