Optimizing Contrast-Enhanced Magnetic Resonance Imaging Characterization of Brain Metastases: Relevance to Stereotactic Radiosurgery

Intracranial metastases are the most common form of intra-axial brain tumor. Management approaches to brain metastases include surgical resection, whole-brain radiotherapy, and stereotactic radiosurgery (SRS). The management approach that is selected is based typically on algorithms that incorporate the number, size, and location of lesions. SRS is the treatment of choice when metastases detected on imaging are few (maximum, 3-5) and/or of small size (<= 30 mm) and offers the advantages of noninvasiveness and the ability to treat inaccessible lesions compared with surgical resection. Contrast-enhanced magnetic resonance imaging (MRI) is the standard imaging technique for determining the number, size, and location of metastatic lesions. In SRS, the capability of MRI to delineate lesion borders precisely in 3 dimensions helps reduce recurrence rates and minimize radiation necrosis in surrounding tissue. Optimization of the MRI protocol, including selection of the appropriate gadolinium-based contrast agent (GBCA), is paramount for accurate lesion imaging. GBCAs differ in their safety, tolerability, and efficacy because of their diverse physicochemical properties. Gadobutrol and gadobenate dimeglumine are high-relaxivity GBCAs that demonstrate superior efficacy for imaging metastatic lesions compared with other GBCAs, whereas gadobutrol additionally provides macrocyclic stability. This article reviews recent comparative trials of GBCAs and discusses their relevance for optimizing MRI protocols in the management of brain metastases, with particular relevance to SRS.

Summary: Trigeminal neuralgia in patients with multiple sclerosis is challenging to manage, often treated with Gamma Knife radiosurgery. This study evaluated the safety, efficacy, and durability of GKRS for TN in MS patients, finding short-term pain control was effective but long-term control was limited. Repeat GKRS was found to be safe and effective in prolonging pain relief.

Summary: The occurrence of symptomatic radiation necrosis (RN) in patients with renal cell carcinoma (RCC) brain metastases treated with immune checkpoint inhibitors (ICIs) and stereotactic radiosurgery (SRS) is relatively low, with most events being grade 1 to 3 and managed by medical means. Concurrent therapy with ICI/SRS does not appear to increase the risk of RN. Efforts to improve dose conformality may be the most successful strategy for mitigating RN in single-fraction SRS.

Summary: Gamma Knife radiosurgery (GKRS) can be used to treat drug-resistant cluster headache (CH). The outcomes of studies on GKRS for CH are inconsistent, and the ideal target and treatment parameters are unclear. This systematic review evaluates the safety and efficacy of GKRS for drug-resistant CH, showing high short-term pain reduction rates but controversial long-term results.

Summary: This study aimed to investigate whether radiomics features can improve outcome prediction in patients with Primary Central Nervous System Lymphoma (PCNSL). By extracting and selecting radiomics features from normalized MR images and using machine learning models for survival prediction, better performance was achieved compared to current clinical prognostic factors. These results indicate that radiomics features can improve prognosis stratification of PCNSL patients and pave the way for further study on their potential role in treatment choice.

Summary: Stereotactic radiosurgery (SRS) is an effective treatment for pituitary adenomas, but the mechanism of delayed endocrine deficits is not well understood. This multicenter study aimed to evaluate radiation tolerance and identify predictors of delayed hypopituitarism after SRS for these tumors. The study found that tumor control was achieved in a high percentage of patients, but a significant number developed new loss of pituitary function, with certain factors such as tumor type, age, and radiation dose playing a role.
