Recognition and management of stroke in young adults and adolescents

Approximately 15% of all ischemic strokes (IS) occur in young adults and adolescents. To date, only limited prior public health and research efforts have specifically addressed stroke in the young. Early diagnosis remains challenging because of the lack of awareness and the relative infrequency of stroke compared with stroke mimics. Moreover, the causes of IS in the young are heterogeneous and can be relatively uncommon, resulting in uncertainties about diagnostic evaluation and cause-specific management. Emerging data have raised public health concerns about the increasing prevalence of traditional vascular risk factors in young individuals, and their potential role in increasing the risk of IS, stroke recurrence, and poststroke mortality. These issues make it important to formulate and enact strategies to increase both awareness and access to resources for young stroke patients, their caregivers and families, and health care professionals. The American Academy of Neurology recently convened an expert panel to develop a consensus document concerning the recognition, evaluation, and management of IS in young adults and adolescents. The report of the consensus panel is presented herein.

Summary: The study analyzed the rates and risk factors for AVM recurrence in a cohort of children with ruptured AVMs. The findings suggest that infratentorial location and younger age at presentation may be associated with earlier and more frequent recurrences.

Summary: Pediatric brain arteriovenous malformations (AVMs) are a leading cause of spontaneous intracranial hemorrhage (SICH) in children. This study validates the recently created Ruptured Arteriovenous Malformation Grading Scale (RAGS) in a cohort of pediatric patients with AVMs, and demonstrates that the RAGS score outperforms other clinical grading scales in predicting clinical outcomes.

Summary: This Primer provides an overview of the mechanisms, epidemiology, diagnosis and treatment of stroke in children. Childhood stroke is a significant cause of neurological damage and survivors often carry lifelong neurological deficits. While progress has been made in pediatric stroke research, the evidence for interventions, such as rapid reperfusion therapies, remains limited and delays in diagnosis and treatment continue to pose challenges.

Summary: Incidence of arterial ischemic stroke (AIS) has decreased overall in recent decades, but has increased in young adults. This study found that the risk factors associated with stroke in older adults are present in childhood and increase with age, highlighting the importance of early prevention and control of these risk factors.

Summary: This study investigated predictors of neurocognition in pediatric cancer survivors and found factors associated with neurocognitive deficits in survivors of central nervous system tumors exposed to radiation. The presence of cerebral microbleeds (CMB), APOE epsilon 4 carrier status, hydrocephalus, and seizures were correlated with worse neurocognitive outcomes.

Summary: This study describes the largest series to date comparing the presentation, angioarchitecture, and management of incidental versus symptomatic unruptured brain arteriovenous malformations (AVMs) in children. The results showed that incidental AVMs are more common in pediatric patients and are characterized by smaller size, less deep venous drainage, earlier age of onset, and longer duration to treatment compared to symptomatic AVMs.
