Summary: This study compared the health outcomes associated with the two most commonly used beta-blockers, carvedilol and bisoprolol, in patients undergoing hemodialysis, finding that bisoprolol initiation was associated with a lower 2-year risk of death and major adverse cardiovascular events, mainly due to lower risks of heart failure and ischemic stroke.

Summary: This case series study found that approximately half of the patients who underwent outpatient hemodialysis after receiving LVAD either recovered kidney function or underwent combined heart and kidney transplantation. Additionally, the study suggests that outpatient hemodialysis centers, in partnership with LVAD treatment teams, can successfully provide hemodialysis to patients on LVAD support.

Summary: Chronic kidney disease affects over 800 million people globally and can lead to end-stage renal disease necessitating maintenance dialysis. Dialysis patients have an increased risk for serious respiratory infections, including COVID-19. Studies have shown that mRNA COVID-19 vaccines elicit immune responses in dialysis patients, although the response is often attenuated and wanes quickly. Furthermore, dialysis patients have a significantly higher risk of COVID-19 hospitalization and mortality compared to the general population. This article reviews the immunogenicity and real-world outcomes of COVID-19 mRNA vaccination in dialysis patients, focusing on the extended primary series and additional doses.

Summary: This study analyzed the coexistence of chronic liver disease (CLD) in dialysis patients and found a high prevalence of cirrhosis and non-alcoholic fatty liver disease (NAFLD). Hepatitis B and C were identified as risk factors for cirrhosis. However, further research is needed to understand the impact of NAFLD and NASH on cirrhosis.

Summary: Through analysis of survey data from patients receiving dialysis and their caregivers, it was found that the treatment burden of dialysis, lifestyle restrictions, uncertainty about the future, developing self-reliance, and adapting to lifestyle changes all contribute to mental health issues in this population. Increased attention to monitoring and managing mental health is needed for patients and their caregivers undergoing dialysis.

Summary: Fatigue is a common symptom in patients undergoing hemodialysis that affects their quality of life. There is limited understanding of the associated risk factors and pathophysiology. Several hypotheses have been proposed to explain the pathophysiology, but there is currently no convincing data to support them. Fatigue is associated with various clinical factors, including cardiovascular and hemodynamic effects of the dialysis procedure, laboratory abnormalities, depression, and physical inactivity. Further robust studies are needed to establish the pathophysiology and management of this symptom.

Summary: Parathyroidectomy is associated with improvement in nutritional status in maintenance hemodialysis patients, especially for those with severe hyperparathyroidism. This finding is crucial as protein-energy wasting is common in kidney failure patients and can have negative impacts on survival and outcomes.

Summary: The prevalence of sarcopenia in Australian maintenance dialysis patients was found to be 18%, with low serum albumin and phosphate levels identified as significant risk factors. Increasing age and female sex were also potential risk factors. Low lean muscle mass, reduced hand grip strength, and prolonged TUG were common within the cohort, with hand grip test showing good correlation with lean muscle evaluation and TUG.

Summary: This study evaluated the incidence of adverse outcomes among patients with nonvalvular atrial fibrillation receiving oral anticoagulants based on the CHA(2)DS(2)-VASc risk score, thromboembolic event history, and age group. The results provide insights into the importance of specific risk score range, thromboembolic event history, and age group strata in clinical decision-making, research, and policy.

Summary: Depression and anxiety are common among patients receiving maintenance hemodialysis (MHD) and have an impact on their quality of life. Higher Charlson Comorbidity Index, lower educational status, and usage of multiple oral medications may be associated with depression, while longer dialysis duration and multiple oral medications may be associated with anxiety in MHD patients.

Summary: This study examined the impact of PD catheter insertion technique, timing of gastrostomy placement, and prophylactic antibiotics on peritonitis risk. The results showed that PD catheter insertion technique had no significant impact on peritonitis risk, while timing of gastrostomy placement may have some impact. There was no significant association between prophylactic antibiotics and peritonitis risk.

Summary: Patients receiving maintenance dialysis show variable cellular and humoral immune responses after SARS-CoV-2 vaccination. Anti-spike IgG antibody levels tend to decline rapidly after full immunization, and only half of the patients develop T cell immune response. Higher antibody levels may predict better cellular immunity. SARS-CoV-2-naive patients may benefit from a third vaccine dose.

Summary: Ischaemic stroke is more frequent than haemorrhagic stroke among patients with chronic kidney disease, but the relative frequency of haemorrhagic stroke seems to increase as kidney function declines. Presence of CKD is associated with worse clinical outcomes in patients with acute ischaemic stroke treated with thrombolysis compared with patients with preserved kidney function.

Summary: This study found an association between vitamin K status and cognitive performance in individuals with chronic kidney disease. Participants with lower levels of plasma (dp)ucMGP, indicating higher vitamin K status, had better cognitive performance. Plasma phylloquinone levels were not significantly associated with cognitive performance. These findings suggest that vitamin K may play an important role in cognitive function in individuals with chronic kidney disease.

Summary: This study compared the incidence of adverse cardiovascular events in older adults with primary hyperparathyroidism treated with parathyroidectomy versus nonoperative management. The results showed that parathyroidectomy was associated with a lower incidence of cardiovascular events compared to nonoperative management, which is relevant for patients with a long life expectancy.

Summary: This study investigated racial disparities in prognostic discussions, beliefs about future health, and completion of advance care planning (ACP)-related documents among Black and White patients receiving maintenance dialysis. The results showed that there were differences in health expectations and completion of ACP-related documents between Black and White patients. Therefore, systemic efforts are needed to investigate differences in health beliefs and address racial disparities in the completion of ACP-related documents.

Summary: The study demonstrates that intensive treatment for hypertension can slow the progression of cerebral white matter hyperintensities and is associated with changes in functional brain connectivity. Specifically, the auditory-salience-language network showed a significant decline in connectivity score in the standard treatment group compared to the intensive treatment group. Additionally, changes in the mean connectivity score of the default mode network correlated significantly with changes in white matter hyperintensities.
