We compared the global and regional distribution of white matter (WM) and gray matter (GM) damage and T2-visible lesion between patients with benign (B) and relapsing remitting (RR) multiple sclerosis (MS). BMS and RRMS patients did not differ in terms of global volumes and diffusion tensor (DT) MRI metrics of the WM and GM. Compared to controls, BMS and RRMS patients had bilateral thalamic loss. Compared to controls, BMS and RRMS patients had lower WM fractional anisotropy (FA) in the corpus callosum (CC) and in several regions of temporal and occipital lobes. BMS also had a decreased WM FA in the parietal lobes. RRMS patients had also lower WM FA in several regions of the frontal lobes. Compared to BMS, PRMS patients had decreased WM FA in the frontal lobes, while the opposite comparison showed lower WM FA in the CC, the temporal lobes and the cuneus in BMS. Contrasted to controls, both MS groups showed several regions of increased MD in WM and GM, but no difference was found between MS sub-groups. T2-visible lesions were mainly located in the posterior regions of the brain in BMS patients, while they involved also regions in the frontal lobes, in RRMS patients. BMS and RRMS patients differ in terms of the topographical distribution of WM damage rather than in the overall extent of brain structural changes. The less prominent involvement of the frontal lobe WM and of the NAWM in general in BMS might be associated to their favorable clinical status. (C) 2007 Elsevier Inc. All rights reserved.

Summary: The study explores the associations among fatigue, dual-task performance and structural and functional abnormalities of frontal cortico-subcortical network in progressive multiple sclerosis (PMS). The results indicate that both structural and functional abnormalities contribute to fatigue and worse dual-task performance in PMS, with different patterns according to the presence of fatigue.

Unraveling the heterogeneous pathological substrates of relapse-onset multiple sclerosis: a multiparametric voxel-wise 3 T MRI study

Summary: This study used multimodal MRI to investigate the regional distribution of different pathological processes in the brain white and gray matter of relapse-onset multiple sclerosis (MS) patients. The results showed that compared to healthy controls, relapsing-remitting MS (RRMS) patients exhibited white matter, deep gray matter, and cortical atrophy, as well as significantly lower MTR and T1w/T2w ratio in periventricular and infratentorial white matter, deep gray matter, and several cortical areas. Secondary progressive MS (SPMS) patients showed more severe deep gray matter and widespread cortical atrophy, lower MTR of periventricular white matter, deep gray matter, and cerebellum, lower T1w/T2w ratio of fronto-temporal white matter regions, lower ICV_f of some fronto-tempo-occipital white matter and cortical areas, and increased QSM and T1w/T2w ratio in the pallidum.

Regional white matter and gray matter damage and cognitive performances in multiple sclerosis according to sex

Summary: In this study, the researchers investigated whether the regional distribution of white matter lesions, normal-appearing white matter microstructural abnormalities, and gray matter atrophy contribute differently to cognitive performance in MS patients based on their sex. The study found that in female MS patients, the association between white matter lesions and microstructural abnormalities with cognitive performance, especially global cognition, is stronger. In contrast, male MS patients showed worse performance on verbal memory tasks.

Summary: This study summarizes four cases of amyotrophic lateral sclerosis (ALS) patients with Tako-Tsubo syndrome (TTS) and reviews the relevant literature, revealing the association between ALS and TTS. Among these cases, one ALS patient developed TTS during hospitalization, and three additional cases with ALS-TTS were identified retrospectively. The study found that cardiovascular autonomic dysfunction in ALS is especially pronounced in cases with bulbar involvement.

Summary: This study aimed to evaluate the impact of aging on the functional connectivity of pivotal regions in the human brain connectome and to examine whether these effects influence the overall functional and structural changes of the brain. By analyzing functional connectivity and cortical thinning data, it was found that aging leads to changes in functional connectivity of key regions and affects the structural alterations of specific brain regions.

Depressive symptoms, anxiety and cognitive impairment: emerging evidence in multiple sclerosis

Monica Margoni, Paolo Preziosa, Maria A. A. Rocca, Massimo Filippi

Summary: Neuropsychiatric abnormalities in multiple sclerosis (MS) can be categorized into mood, affect, and behavior disorders and abnormalities affecting cognition. These manifestations have a significant impact on patients' quality of life and treatment adherence, especially in progressive and more severe forms of the disease. The pathogenesis of these manifestations is not fully understood yet, but brain structural and functional abnormalities observed with MRI, along with genetic and immunologic factors, play important roles. However, these symptoms are often overlooked in clinical practice, and the efficacy of available treatments is limited due to multiple underlying mechanisms and methodological limitations. A better evaluation and understanding of these manifestations may lead to the development of personalized treatment strategies.

Summary: This study investigated the impact of different cut-offs in the MSISQ-19 on the prevalence of sexual dysfunction (SD) and associated risk factors in people with MS. Depending on the chosen cut-off, 45% to 54% of individuals with MS reported SD. SD defined as an MSISQ-19 score >30 was associated with age, cognition, and anxiety, while SD defined as a score >3 on any MSISQ-19 item was associated with motor disability and cognition. These findings highlight the importance of considering the chosen cut-off when planning studies and interpreting data.

Similarities and Differences between Vestibular Migraine and Recurrent Vestibular Symptoms-Not Otherwise Specified (RVS-NOS)

Summary: Meniere's disease and vestibular migraine (VM) are two common inner ear disorders. Recurrent Vestibular Symptoms-Not Otherwise Specified (RVS-NOS) refers to patients who experience recurring episodes of vertigo but do not fulfill the criteria for either disease. This study aimed to compare RVS-NOS with VM in terms of clinical history, bedside examination, and family history. The findings showed similarities between the two disorders, including age of onset, duration of attacks, motion sickness, and family history. However, some aspects suggest that RVS-NOS may be a heterogeneous disorder.

Summary: This study proposes a multi-penalized state-space model for analyzing unobserved dynamics, using a data-driven regularization method. Novel algorithms are developed to solve the model, and a cross-validation method is introduced to evaluate regularization parameters. The effectiveness of this method is validated through simulations and real data analysis, enabling a more accurate exploration of cognitive brain functions.
