Summary: This study investigated the feasibility and safety of a newly developed endotherapy for GERD using intraluminal magnets referred to as a magnet closure device (MCD). The results showed successful deployment of MCDs, increased LES pressures, and no clinically significant adverse events, suggesting a promising reversible technique for GERD therapy. Future studies are needed to improve durability and explore its potential as an attractive alternative to endoluminal GERD therapy.

Summary: MicroRNAs (miRNAs) are small endogenous RNA molecules that regulate mRNA targets through RNA interference. They play a crucial role in physiological and pathological processes including cell proliferation, differentiation, apoptosis, and inflammation. This review discusses the potential of miRNAs in overall esophageal disease, focusing on eosinophilic esophagitis (EoE), gastroesophageal reflux disease (GERD), and the progression from Barrett's esophagus (BE) to esophageal adenocarcinoma (EAC). The dysregulation of miRNA expression in these diseases highlights their key role in disease progression and suggests their potential as diagnostic and prognostic biomarkers. Furthermore, targeting miRNAs could be a therapeutic option for esophageal disease.

Summary: This article reports a battery-free and deformable electronic esophageal stent for wireless stimulation of the lower esophageal sphincter without the need for open surgery. The stent is capable of adapting to the dynamic environment of the esophagus and wirelessly harvesting energy through deep tissue. In vivo experiments using pig models demonstrated that continuous electrical stimulations delivered by the stent significantly increased the pressure of the lower esophageal sphincter.

Summary: The study presented a technique for circumferential esophageal hiatal rim reconstruction in 29 brachycephalic dogs with persistent regurgitation. Short-term follow-up showed a significant reduction in regurgitation frequency post surgery. It is recommended to consider this procedure for brachycephalic dogs with a history of regurgitation unresponsive to medical management.

Summary: This study evaluated the role of the transdiaphragmatic pressure gradient (TPG) and lower esophageal sphincter (LES) dysfunction in the pathogenesis of gastroesophageal reflux disease (GERD). The results showed that abdominal pressure and TPG were higher in GERD-positive patients, while LES resting pressure was not different between groups. Relative LES pressure (LES retention pressure) was able to predict the presence and severity of GERD. The study concluded that TPG plays an important role in the pathophysiology of GERD, and the relative LES pressure is a reliable indicator for GERD.

Summary: Objective: To evaluate and characterize outcomes of MSA in patients with IEM. The study found that although MSA improves patients with gastroesophageal reflux and normal motility, many patients with IEM have lower rates of objective GERD resolution, lower resolution of existing dysphagia, higher rates of new onset dysphagia and need for dilation, which should be counseled to the patients before surgery.

Summary: Reflux hypersensitivity (RH) is a phenotype of gastroesophageal reflux disease characterized by typical reflux symptoms and positive reflux-symptom association despite normal acid exposure. The Rome IV criteria and Lyon consensus provided a clearer understanding of the pathophysiology and importance of esophageal hypersensitivity. RH often overlaps with other functional gastrointestinal disorders. Despite identified pathophysiological mechanisms, there is still a need for improved treatment options. Summary: Patients who undergo vertical sleeve gastrectomy (VSG) are at risk of postoperative gastroesophageal reflux disease (GERD). Abnormal preoperative pH studies and esophageal motility disorders are associated with higher rates of postoperative reflux after VSG. A DeMeester score greater than 24.8 predicts the occurrence of postoperative reflux in patients with abnormal preoperative pH studies.

Summary: This study aimed to investigate the prevalence of GERD-related symptoms in South-East Hungarian subjects. The results showed that the prevalence of GERD-related symptoms in this region was significantly lower than that in Western countries and closer to Eastern values. The presence of mild chronic diseases, positive family history of GERD, height, BMI, coffee consumption, and smoking were identified as significant factors associated with the increased prevalence of GERD-related symptoms.
