Infection is a common phenomenon following stroke, and adversely affects outcome. Previous studies suggest that interleukin-1 receptor antagonist (IL-1ra) and single nucleotide polymorphisms (SNPs) in the IL1RN gene might influence the risk of post-stroke infection and outcome. In this study, we addressed the effects of the rs4251961 SNP in IL1RN on infection risk and outcome. Subjects with acute ischemic stroke were enrolled within 72 h of symptom onset and followed up to 1 year. Plasma IL-1ra was measured at multiple time points and outcome assessed at 1, 3, 6, and 12 months. Active surveillance for infection occurred while subjects were hospitalized. Subjects were genotyped for the IL1RN rs4251961 polymorphism. In the population of 113 subjects for this study, those with the minor C allele of rs4251961 polymorphism in IL1RN were more likely to be Caucasian, hypertensive, and to be afflicted with coronary heart disease. Higher plasma IL-1ra was associated with an increased risk of infection (other than pneumonia), and the minor C allele of rs4251961 was independently associated with a decreased risk of infection (other than pneumonia). Initial plasma IL-1ra was not predictive of long-term outcome, but patients with the minor C allele of rs4251961 were more likely to experience good (modified Rankin Score < 2) long-term outcome. These data indicate that IL-1ra and IL1RN may influence the risk of infection after stroke, but this influence seems limited to infections other than pneumonia. Further studies are needed to better understand the complexities of immune regulation on infection and outcome after stroke.

Kappa Free Light Chains in the Context of Blood Contamination, and Other IgA- and IgM-Related Cerebrospinal Fluid Disease Pattern

Summary: In this study, researchers investigated the synthesis of intrathecal FLC-k in cerebrospinal fluid samples from 296 patients. They found that FLC-k synthesis can occur independently of the antibody class produced, and can help distinguish between an inflammatory process and blood contamination. This suggests that additional determination of FLC-k in quotient diagrams may aid in clinical diagnostics.

Summary: The research revealed that Prom1-expressing myeloid progenitor cells can contribute to the generation of new microglia both in vitro and in vivo, specifically in the uninjured brain. However, in response to ischemia/reperfusion, little proliferation was observed from these Prom1-expressing progenitors, with the majority of new microglia derived from Prom1-negative cells.

Microglial depletion abolishes ischemic preconditioning in white matter

Margaret A. Hamner, Ashley McDonough, Davin C. Gong, Levi J. Todd, German Rojas, Sibylle Hodecker, Christopher B. Ransom, Thomas A. Reh, Bruce R. Ransom, Jonathan R. Weinstein

Summary: Ischemic preconditioning (IPC) is a phenomenon where exposure to non-injurious ischemia enhances tolerance to subsequent ischemic challenges. In a study testing the hypothesis that microglia are necessary for IPC in white matter (WM), researchers found that depletion of microglia eliminated IPC-mediated protection, highlighting the critical role of preconditioned microglia in WM IPC.

Summary: This cohort study aimed to investigate whether further analysis of intrathecal inflammation can be omitted if the free light chain kappa (FLC kappa) quotient is within the reference range in the corresponding quotient diagram. The results showed that using FLC kappa-IF as a first line analysis can significantly reduce the workload of detecting immune responses in cerebrospinal fluid with high negative predictive value.

Summary: Using simulated odorant-binding proteins (OBPs) and machine learning methods, a sensor array was designed to detect a characteristic VOC fingerprint associated with COVID-19. The sensor array directly transduces vapor composition into multi-channel electrical signals, allowing for rapid and non-invasive monitoring, disease diagnostics, and environmental exposure assessment. The use of chimeric VOC- and solid-binding peptides and carbon nanotube transistors demonstrated selective peptide binding and VOC sensing.

Summary: This study compared two social cognition tests in multiple sclerosis patients and found that emotion recognition had better accuracy compared to theory of mind. Cognitive factors were associated with both social cognition tasks, with emotion recognition being more influenced. However, no relationship was found between social cognition and quality of life.
