Summary: Pure red cell aplasia (PRCA) following allogeneic haematopoietic stem cell transplantation (aHSCT) with major ABO incompatibility has a cumulative incidence of 10.5%. Pre-transplant high isohaemagglutinins titre is associated with an increased risk of PRCA, but PRCA does not affect overall survival. Specific treatments like rituximab or erythropoietin did not show significant difference in resolving PRCA compared to untreated subjects.

Summary: Anti-EPO antibody-mediated pure red cell aplasia (PRCA) is a rare disease primarily seen in patients with chronic kidney disease receiving recombinant human erythropoietin (rHuEPO) treatment. Treatment options include discontinuation of rHuEPO, immunosuppressive agents, intravenous immunoglobulin, plasmapheresis, or kidney transplantation. We reported a successful case of a kidney transplant recipient with anti-EPO antibody-mediated PRCA, who achieved remission after consecutive plasmapheresis sessions and adjustment of immunosuppressive regimen.
