Barriers to living kidney donation identified by eligible candidates with end-stage renal disease

Background. Among eligible transplant candidates with end-stage renal disease, only a minority receive a living donor kidney transplant (LDKT), suggesting that there are barriers to receipt of this optimal therapy. Methods. A validated questionnaire was administered to adults active on the deceased donor transplant waiting list, identified from the Southern Alberta Renal Program database. The questionnaire included both quantitative and qualitative items addressing issues related to LDKT in the categories of knowledge, opportunity, fear and guilt. Results. Of the 196 subjects invited to complete the questionnaire, 145 (74%) responded. Not knowing how to ask someone for their kidney was the most frequently reported barrier, identified by 71% of respondents. Those that stated that living donation did not pose significant long-term health risks to the donor [odds ratio (OR) = 3.40, 95% CI 1.17-9.46, P = 0.01] and those who understood how and why to begin the living donation process (OR = 4.21, 95% CI 1.41-12.04, P = 0.002) were more likely to have discussed living donation with potential donors. Conclusions. Knowledge about living donation was associated with having discussed living donation with a family member or friend. Studies examining the impact of educational programmes which address these barriers to living donor kidney transplantation are required.

Summary: This study aimed to describe the patterns and patient-level factors associated with the use of sodium-glucose cotransporter-2 inhibitors (SGLT2is) among adults with diabetes in Alberta, Canada. The use of SGLT2is is increasing among patients with diabetes, but remains low even in those with cardiovascular disease (CVD). Patient characteristics associated with lower odds of filling an SGLT2i prescription included female sex, older age, and lower income.

Learning high-order interactions for polygenic risk prediction

Summary: Within the precision medicine framework, the stratification of individual genetic susceptibility based on inherited DNA variation is crucial. Traditional Polygenic Risk Scores (PRS) approaches face challenges in modeling complex high-order non-linear SNP-SNP interactions and their effect on the phenotype. In this study, we propose a novel approach called High-order Interactions-aware Polygenic Risk Score (hiPRS), which incorporates high-order interactions and provides a simple and interpretable model. Through comprehensive simulations and real data analysis, hiPRS demonstrates superior performance in scoring and interpretability compared to state of the art methods.

Therefore, they should be considered for routine risk assessment and can enhance the initiation of preventive therapies.

Summary: This study aimed to describe the evolution of frailty over time in individuals undergoing hemodialysis, and to determine the association between improvements in frailty and clinical outcomes. The results showed that frailty was associated with increased risk of mortality, hospitalizations, and long-term care placement. Moreover, patients who showed improvement in frailty had better clinical outcomes compared to those who remained frail.
