End-stage renal failure and regulatory activities of CD4+CD25bright+FoxP3+ T-cells

Background. The defensive immune system in patients with end-stage renal failure is impaired at multiple levels. This state of immune incompetence is associated with continuous activation of the immune system. An additional explanation for this state of activation may be the disturbed function of CD4(+)CD25(bright+)FoxP3(+) regulatory T-cells. Methods. The phenotype and function of peripheral regulatory T-cells from patients with end-stage renal failure (N = 80) and healthy controls (N = 17) was studied by flow cytometry, RT-PCR and mixed lymphocyte reaction. Patients were on haemodialysis (N = 40), peritoneal dialysis (N = 26) or not treated with dialysis yet (N = 14). The latter group had a glomerular filtration rate of < 20 ml/min/ 1.73 m(2). Results. The basal IL-2 mRNA level was high in patient-PBMC (P = 0.0002 versus healthy controls). The absolute number of CD4(+)CD25(bright+) T-cells was low in patients (P < 0.05 versus healthy controls). Furthermore, proliferation of patient-PBMC upon allogeneic stimulation was impaired (P < 0.0001 versus healthy controls). The regulatory function of CD4(+)CD25(bright+) T-cells was determined in the setting of direct allorecognition. First, the effect of depletion of CD25(bright+) cells from patient-PBMC on proliferation was low. Second, co-culture of CD25(bright+) cells with CD25(neg/dim) cells (1:10 ratio) showed impaired regulatory function (P < 0.001 versus healthy controls), which was especially pronounced in patients on dialysis. The FOXP3 mRNA level was also low upon stimulation (P = 0.0002 versus healthy controls). Conclusions. In line with previous studies, we observed an overactivated but functionally compromised immune system in patients with end-stage renal failure. It now appears that in this setting, regulation by CD4(+)CD25(bright+)FoxP3(+) T-cells is also impaired.

Summary: This study aimed to evaluate the safety and feasibility of using MSC treatment during NMP in a porcine autotransplantation model, and found that infusing MSCs during ex vivo NMP of porcine kidneys was safe and feasible. However, no beneficial effects of ex vivo MSC therapy could be demonstrated during the short posttransplant follow-up period of 14 days.

Variations in DNA methylation and allograft rejection

Summary: DNA methylation is important for gene transcription and cellular function, with potential clinical utility for monitoring transplant rejection. However, research on DNA methylation in relation to allograft rejection is scarce, making it challenging to determine its clinical value. Further research is needed to investigate the potential of DNA methylation in early recognition and successful treatment of allograft rejection.

Circulating cell-free nucleosomes as biomarker for kidney transplant rejection: a pilot study

Summary: In this pilot study, total circulating cell-free nucleosomes (H3) concentrations are increased during acute kidney transplant rejection. The high negative predictive value suggests that when a patient experiences loss of renal transplant function and total CCFN (H3) is not increased, causes other than acute rejection should be considered. Clinical implementation of total CCFN (H3) measurement may help avoid unnecessary and potentially harmful kidney transplant biopsies.

Membrane particles from mesenchymal stromal cells reduce the expression of fibrotic markers on pulmonary cells

Summary: Mesenchymal stromal cells (MSC) show promise as a therapy for inflammatory diseases, but their large size leads to entrapment in the lungs after intravenous infusion, limiting their effectiveness. Researchers have developed nanometer-sized particles from MSC membranes (membrane particles, MP) to extend the reach of MSC immunoregulatory therapy beyond the lungs. These particles exhibit immunomodulatory properties and are taken up by macrophages and human umbilical vein endothelial cells (HUVEC) in a dose, time, and temperature-dependent manner. The uptake of MP depends on specific endocytic pathways and is influenced by cell type and inflammatory conditions, with anti-inflammatory M2 macrophages showing increased uptake compared to pro-inflammatory M1 macrophages.
