Aim: Nephropathy associated with aristolochic acid (AA) has been documented by human and animal studies. Ancient Chinese herbology claimed to reduce toxicity in their mixtures. It was the objective of this study to determine the risk of chronic kidney disease (CKD) associated with AA-related Chinese herbal products (CHP) or mixtures of herbs in a national cohort. Methods: A retrospective follow-up study was conducted, using a systematic random sample (200 000 people) in the National Health Insurance reimbursement database during 1997-2002. The incidence rates of CKD and end-stage renal disease (ESRD) were calculated for the whole sample and those that had used CHP suspected to contain AA. Cox regression models were constructed to control potential confounders, including age, sex, hypertension, diabetes mellitus, and use of non-steroidal anti-inflammatory drugs and acetaminophen. Results: A total of 199 843 persons were included in the final analysis, 102 464 (51.3%) men and 97 379 (48.7%) women, with an average incidence rate of 1964/10(6) person-years for CKD and 279/10(6) person-years for ESRD. After controlling other risk factors, the hazard ratios for development of CKD seemed to increase for patients that had consumed more than 30 g Mu-Tong, and more than 60 g Fangchi. Conclusion: Prescription of more than 30 g Mu-Tong or more than 60 g Fangchi CHP was associated with an increased risk of developing CKD. In addition to prohibiting the use of Guan-Mu-Tong and Guang-Fangchi, patients who have used these CHP should continue to be followed up.

Forest Bathing Is Better than Walking in Urban Park: Comparison of Cardiac and Vascular Function between Urban and Forest Parks

Tsung-Ming Tsao, Jing-Shiang Hwang, Sung-Tsun Lin, Charlene Wu, Ming-Jer Tsai, Ta-Chen Su

Summary: This study investigates the effects of walking in forests and urban parks on cardiovascular functions (CVFs). The results show that walking in a forest park significantly lowers blood pressure, heart rate, cardiac output, and improves vascular compliance compared to walking in an urban park. This study provides evidence of the potential beneficial effects of walking exercise in forest parks on CVFs.

Living proximity to petrochemical industries and the risk of attention-deficit/hyperactivity disorder in children

Summary: This study aimed to evaluate the association between exposure to petrochemical facilities and emissions and the risk of ADHD development in children. The results indicated that residents of townships near petrochemical plants, highly affected by petrochemical-containing prevailing winds, and with high benzene concentrations had an increased risk of ADHD development. This study provides clear evidence that living near petrochemical plants increases the risk of ADHD in children.

Investigating the invisible threat: An exploration of air exchange rates and ultrafine particle dynamics in hospital operating rooms

Po -Kai Chang, Hsiang-Hao Chuang, Ta-Chih Hsiao, Hsiao-Chi Chuang, Pau -Chung Chen

Summary: This study investigates the characteristics and transmission of pollutants in hospital operation rooms (ORs) through on-site measurements and numerical simulations. The results show that gaseous pollutants have lower air exchange rates (AERs) compared to particulate pollutants, and the AER of total volatile organic compounds (TVOC) is higher than that of carbon dioxide (CO2). The emission factors (EF) of surgical smoke particles vary depending on the surgical procedure. Coarse particles are primarily influenced by drag forces, resulting in lower AER values. Additionally, smaller particles exhibit lower AER values, potentially due to stronger Brownian motion. The combination of measurement and simulation analysis allows for the evaluation of lung deposited surface area (LDSA) concentrations, providing accurate exposure concentrations for health risk assessments.

Urban climate and cardiovascular health: Focused on seasonal variation of urban temperature, relative humidity, and PM2.5 air pollution

Tsung-Ming Tsao, Jing-Shiang Hwang, Chung-Yen Chen, Sung-Tsun Lin, Ming-Jer Tsai, Ta-Chen Su

Summary: This study aims to investigate the effects of seasonal variations on cardiovascular functions. The results show that in winter, there is an increase in blood pressure and vascular resistance, as well as a decrease in heart rate and cardiac output. Temperature, relative humidity, and PM2.5 air pollution are identified as major factors contributing to these seasonal changes.

Air quality health index (AQHI) based on multiple air pollutants and mortality risks in Taiwan: Construction and validation

Mu-Jean Chen, Yue Leon Guo, Pinpin Lin, Hung-Che Chiang, Pau-Chung Chen, Yu-Cheng Chen

Summary: The currently used air quality index (AQI) is criticized for its inability to capture the additive effects of air pollution on health risks and reflect non-threshold concentration-response relationships. In this study, we propose the air quality health index (AQHI) based on daily air pollution-mortality associations and compare its validity in predicting daily mortality and morbidity risks with the existing AQI.
