OPINION Translating metastasis-related biomarkers to the clinic-progress and pitfalls

In the context of metastatic disease, preclinical models have been used primarily to decipher different steps of the metastatic cascade. Numerous molecular processes operate in these model systems, but none of these has been successfully translated to the clinic. We discuss some of the successes and failures of preclinical models in metastasis research and suggest some of the clues for more clinically relevant research. These potential avenues of research include: the use of adequate statistical methods and well-annotated cohorts in biomarker discovery; an objective assessment of the level of evidence provided by each biomarker; the development of robust molecular or cellular surrogates of metastasis in patients; and original designs for clinical trials. Epithelial to Mesenchymal Transition Regulates Surface PD-L1 via CMTM6 and CMTM7 Induction in Breast Cancer

Summary: This study highlights the importance of CMTM6 and CMTM7 in regulating PD-L1 expression in aggressive breast cancer cells. Activating EMT process in breast cancer cells upregulates CMTM6, which is essential for cell surface expression of PD-L1. Dual targeting CMTM6 and CMTM7 can decrease PD-L1 surface expression, suggesting potential therapeutic benefits in highly aggressive breast cancer patients.

Summary: In patients with non-small cell lung cancer (NSCLC), targeting aerobic glycolysis can enhance the therapeutic effects of EGFR tyrosine kinase inhibitors (EGFR TKIs) and ionizing radiation, potentially preventing the development of resistance. This study suggests that inhibiting glycolysis could redirect metabolism towards pyruvate oxidation and reduce lactate production, leading to increased mitochondrial activity and deacidification of the extracellular microenvironment, ultimately enhancing the efficacy of EGFR TKIs and ionizing radiation in NSCLC.

Summary: This study reveals that keratin 16 is a critical protein promoting cancer metastasis, associated with epithelial-mesenchymal transition (EMT) and positive regulation of cell motility, with its expression in circulating tumor cells correlating with shorter relapse-free survival in patients. Therefore, keratin 16 could serve as a prognostic marker to identify cancer patients at higher risk of metastasis.

Summary: Epithelial cell plasticity is a key feature of cancer progression, achieved mainly through epithelial-mesenchymal transition, giving cancer cells unique properties of tumor dissemination and therapy resistance. Different degrees of plasticity are attained through transcriptional regulation and feedback loops. Study on the plasticity in carcinoma may lead to the discovery of new therapeutic strategies against cancer.

This genetic change may have immunomodulatory effects on the tumor microenvironment and play various roles in the tumor biology of East Asian patients.

Summary: This study reveals that different EC repeats in cadherins' extracellular regions can modulate epithelial sheet integrity, cell-cell separation forces, and cell cortical tension, which further affect epithelial tensile strength, ductility, and collective migration. Interestingly, dissipative processes rather than static adhesion energy play a dominant role in cell-cell separation forces. This framework provides insights into how epithelial phenotypes emerge from cell mechanical properties dependent on EC outside-in signaling.

The findings suggest that MSCs-derived CCL5 could act as a stimulator for PD-L1 in breast cancer.
