Cardiovascular manifestations of the emerging dengue pandemic

Dengue is one of the most important emerging viral diseases globally. The majority of symptomatic infections result in a relatively benign disease course. However, a small proportion of patients develop severe clinical manifestations, including bleeding, organ impairment, and endothelial dysfunction with increased capillary permeability causing hypovolaemic shock that can lead to cardiovascular collapse. Evidence is increasing that dengue can also cause myocardial impairment, arrhythmias and, occasionally, fulminant myocarditis. No antiviral agents or vaccines are licensed for dengue, and treatment remains supportive with judicious fluid replacement for patients with severe disease. Defining the role of cardiac dysfunction in the haemodynamic compromise of severe dengue has potentially important management implications. In this Review, we will outline the current understanding of the cardiovascular manifestations of dengue, including myocardial and vascular involvement, and conclude with a discussion of the available therapeutic options and potential future research directions.

Summary: The decay rates of humoral response differ between vaccines 84 days after a third dose of COVID-19 vaccine. The anti-spike IgG concentrations following BNT/BNT initial doses were similar to or higher than for a three-dose BNT schedule. Half dose BNT immune responses were similar to full dose responses.

Summary: An observational study found that extracorporeal membrane oxygenation (ECMO) is associated with a reduced risk of mortality in adults with COVID-19 associated respiratory failure. Age, severity of hypoxaemia, and duration and intensity of mechanical ventilation were identified as modifiers of treatment effectiveness, and should be taken into consideration when deciding to initiate ECMO in patients with COVID-19.

Summary: The immune response to the SARS-CoV-2 virus in fatal COVID-19 cases is enriched for antibodies that target epitopes shared with other endemic coronaviruses, rather than the more protective regions of the virus. This response is correlated with the magnitude of antibody responses to the spike protein of both SARS-CoV-2 and other beta-coronaviruses in individuals admitted to the ICU with fatal outcomes. However, there is no significant difference in antibody responses to the less cross-reactive SARS-CoV-2 nucleocapsid between fatal and nonfatal cases.

Summary: This study conducted exploratory analyses on the priming intervals of COVID-19 vaccines and found that longer intervals can reduce reactogenicity for the BNT162b2 vaccine as the second dose and enhance humoral immunogenicity in homologous priming schedules, but result in overall lower T-cell responses across all schedules. These findings support the flexibility of priming intervals in COVID-19 vaccine schedules.

A Multi-Site, Multi-Wavelength PPG Platform for Continuous Non-Invasive Health Monitoring in Hospital Settings

Summary: This article introduces a novel PPG acquisition platform that can simultaneously acquire multi-wavelength signals from two measurement locations, with up to 4 independent wavelengths from each in parallel. The platform is configurable, operates at 1ksps, and can be used for both research and clinical purposes. The sensing probes presented in this work acquire 4 PPG channels from the wrist and 4 PPG channels from the fingertip, allowing for the extraction of surrogates for pulse wave velocity and haematocrit.

Learning meaningful latent space representations for patient risk stratification: Model development and validation for dengue and other acute febrile illness

Summary: The low adoption of clinical decision support systems is attributed to concerns about recommendation quality and unclear methods for obtaining and presenting results. This study applies autoencoders to a large clinical dataset of dengue patients in Vietnam, producing a two-dimensional representation that aligns with clinical characteristics and disease progression. Balancing distance and density metrics improves visualisation and grouping of similar patients. The findings can be incorporated into an electronic clinical decision support system to guide individual patient management.
