Sunlight exposure and sun sensitivity associated with disability progression in multiple sclerosis

Background: Sunlight and vitamin D have been inversely associated with the risk of multiple sclerosis (MS). Objective: We investigated sunlight exposure and sun sensitivity in relation to disability progression in MS. Methods: We conducted a survey among persons with MS, registered by the Flemish MS society, Belgium, and stratified data according to relapsing-onset and progressive-onset MS. We used Kaplan-Meier survival and Cox proportional hazard regression analyses with time to Expanded Disability Status Scale (EDSS) 6 as outcome measure. Hazard ratios for the time from onset and from birth were calculated for the potentially predictive variables, adjusting for age at onset, gender and immunomodulatory treatment. Results: 704 (51.3%) of the 1372 respondents had reached EDSS 6. In relapsing-onset MS, respondents reporting equal or higher levels of sun exposure than persons of the same age in the last 10 years had a decreased risk of reaching EDSS 6. In progressive-onset MS, increased sun sensitivity was associated with an increased hazard of reaching EDSS 6. Conclusion: The association of higher sun exposure with a better outcome in relapsing-onset MS may be explained by either a protective effect or reverse causality. Mechanisms underlying sun sensitivity might influence progression in progressive-onset MS.

Correlates of patient-reported cognitive performance with regard to disability

Summary: This study examines the relationship between subjective and objective cognitive screening tools in multiple sclerosis (MS) patients and investigates the use of the Multiple Sclerosis Neuropsychological Questionnaire (MSNQ) as a screening tool for cognitive impairment and depression in this population. The results show that MSNQ scores are significantly correlated with depression but not objective cognitive performance. The relationship between subjective and objective cognitive screening tools is not influenced by physical disability. Patient-reported cognitive measures can be useful in identifying patients who may need further psychological assessment.

Cognitive-motor telerehabilitation in multiple sclerosis (CoMoTeMS): study protocol for a randomised controlled trial

Reduced alpha2 power is associated with slowed information processing speed in multiple sclerosis

Summary: This study investigated the association between alpha band power measured with magnetoencephalography (MEG) and cognitive impairment in MS patients. The results showed that alpha2 power was significantly correlated with information processing speed, while alpha1 power was correlated with visual memory. Thalamic volume also had a significant impact on cognitive impairment. These findings suggest that a multimodal assessment is necessary to characterize cognitive impairment in MS.

Gut microbiome composition is associated with long-term disability worsening in multiple sclerosis

Summary: This study investigated the relationship between gut microbial composition and long-term disability worsening in 111 MS patients. The results showed that the inflammation-associated, dysbiotic Bacteroides 2 enterotype (Bact2) was more prevalent in patients with worsening disability compared to non-worsened patients. This association was independent of confounding factors and Bact2 showed a stronger association with disability worsening than neurofilament light chain (NfL) plasma levels. Additionally, Bact2 was relatively stable over time, suggesting its potential use as a prognostic biomarker in MS clinical practice.

The spectral slope as a marker of excitation/inhibition ratio and cognitive functioning in multiple sclerosis

Summary: This study highlights the value of the 1/f spectral slope as a potential biomarker for cognitive deficits in people with multiple sclerosis (pwMS). The use of benzodiazepines is associated with a steeper slope, indicating a stronger inhibition, while cognitively impaired pwMS also demonstrate a steeper slope, suggesting a decreased excitation/inhibition (E/I) ratio and a stronger inhibition. The spectral slope shows significant correlation with verbal and spatial working memory functioning in specific brain regions.
