The Impact of Endometriosis and Its Treatment on Ovarian Reserve

Endometriosis is a chronic disease mostly affecting women at reproductive age. There is a clear association between endometriosis and infertility; however, exact mechanisms are unknown. Some evidence suggests an adverse effect on oocytes. Endometriosis and its surgical treatment can affect quantitative ovarian reserve as well. In the presence of endometriomas, serum level of anti-Mullerian hormone (AMH) seems a more reliable marker of ovarian reserve than antral follicle count. Women with endometrioma have decreased serum AMH levels as compared with healthy controls. This is further declined after surgical excision, and the decline seems permanent. Bipolar cauterization of the ovary seems to be playing a role on ovarian damage. Extraovarian endometriosis and its surgical treatment can also be associated with decreased ovarian reserve, but there is limited information. Patients with endometriosis should be informed about fertility preservation options, especially in the presence of bilateral endometriomas or prior to surgery.

Summary: In patients with diminished ovarian reserve (DOR), if there is discordance between antral follicle count (AFC) and anti-Mullerian hormone (AMH), individuals with high AMH and low AFC have a better ovarian response. It is important to assess both AFC and AMH before controlled ovarian hyperstimulation in order to predict ovarian response in DOR patients, rather than assessing AFC or AMH alone.

Summary: This article summarizes the impact of SARS-CoV-2 and COVID-19 on human fertility and assisted reproduction, as well as the influence of vaccination. It provides answers to relevant questions for individuals considering pregnancy and healthcare professionals. The findings suggest that SARS-CoV-2 can affect spermatogenesis and menstrual patterns, but has minimal impact on ovarian function and embryo development. COVID-19 infection and vaccination do not negatively affect reproductive outcomes.

Why ovarian stimulation should be aimed to maximize oocyte yield

Summary: The ultimate measure of success in assisted reproductive technology (ART) is the cumulative live birth rate per ovarian stimulation cycle. Ovarian stimulation has adverse effects on endometrial receptivity and increases the risks of ovarian hyperstimulation syndrome and adverse obstetric and neonatal outcomes. Mild stimulation has been proposed as a safer and cost-effective alternative. However, accumulating data show that ovarian stimulation has no adverse effects and preventive strategies have been introduced. Vitrification allows for cycle segmentation and maximizes the contribution of every oocyte to the cumulative live birth rate.

Summary: This study investigates the temporal relationship between LH and progesterone increase in a natural menstrual cycle. The study provides a representative account of women undergoing frozen embryo transfer in a natural cycle.

A critical appraisal of studies on endometrial thickness and embryo transfer outcome

Summary: A receptive endometrium is crucial for successful embryo implantation, and ultrasonography-measured endometrial thickness is commonly used as an indicator of endometrial receptivity in assisted reproductive technology cycles. Factors like age, oestradiol concentration, and oocyte number affect both endometrial thickness and probability of live birth. Most studies on the relationship between endometrial thickness and embryo transfer outcomes lack proper addressing of confounding factors and have limitations. Despite numerous meta-analyses and large-scale studies, controversy still exists. This article provides a critical review of studies on endometrial thickness and embryo transfer outcomes, highlighting methodological issues and suggesting improvements for future research. Currently available evidence does not support modifying management solely based on arbitrary thresholds of endometrial thickness.

Summary: Ovarian stimulation for assisted reproductive technology traditionally starts in the early follicular phase, but recent research suggests that the timing of stimulation does not affect the number or quality of oocytes collected. This provides an opportunity for more flexibility in treatment. Random start cycles, where ovarian stimulation begins at any point in the menstrual cycle, have shown promise in improving individualized care for patients.

Neglecting the OGTT may pose risks to the health of both pregnant women and their babies, emphasizing the importance of improving compliance for the prevention and treatment of gestational diabetes.
