Utilizing 7-Tesla Subthalamic Nucleus Connectivity in Deep Brain Stimulation for Parkinson Disease

Varvara Mathiopoulou, Niels Rijks, Matthan W. A. Caan, Luka C. Liebrand, Francisca Ferreira, Rob M. A. de Bie, Pepijn van den Munckhof, P. Richard Schuurman, Maarten Bot

Summary: This study segmented the motor part of the subthalamic nucleus (STN) in patients with Parkinson's disease using 7.0-Tesla T2 and DWI sequences. The results showed that patients with active electrode contacts located in the predominantly motor-connected segment of the STN had significantly higher motor improvement. This method can optimize deep brain stimulation for Parkinson's disease.

Patient-Related Factors Influencing Caregiver Burden in Parkinson's Disease Patients: Comparison of Effects Before and After Deep Brain Stimulation

Marle M. Van Hienen, Roy Kuiper, Huub A. M. Middelkoop, Jacobus J. Van Hilten, Maria Fiorella Contarino, Victor J. Geraedts

Summary: The study found that deep brain stimulation (DBS) does not appear to impact caregiver burden, but there are different symptom values in caregiver burden between preoperative and postoperative assessments. Early recognition of at-risk caregivers may improve guidance for patient-caregiver dyads throughout the DBS process.

Assessment of Parkinson's Disease Severity From Videos Using Deep Architectures

Zhao Yin, Victor J. Geraedts, Ziqi Wang, Maria Fiorella Contarino, Hamdi Dibeklioglu, Jan van Gemert

Summary: This paper proposes a deep learning based automatic PD diagnosis method using videos to assist clinical practice. By utilizing a 3D Convolutional Neural Network and transfer learning, PD severity classification can be effectively performed. Furthermore, a Temporal Self-Attention mechanism is designed to bridge the domain gap between medical and non-medical datasets.

Level I PD-MCI Using Global Cognitive Tests and the Risk for Parkinson's Disease Dementia

Judith A. Boel, Rob M. A. Bie, Ben A. Schmand, John C. Dalrymple-Alford, Connie Marras, Charles H. Adler, Jennifer G. Goldman, Alexander Troster, David J. Burn, Irene Litvan, Gert J. Geurtsen

Summary: The predictive value of global cognitive tests for conversion from PD-MCI to PDD is lower compared to comprehensive neuropsychological assessment. Level II neuropsychological assessment is the preferred method for evaluating PD-MCI.

Medical and Surgical Treatment for Medication-Induced Tremor: Case Report and Systematic Review

Wardell E. Amerika, Saskia van der Gaag, Arne Mosch, Niels A. Gaag, Carel F. E. Hoffmann, Rodi Zutt, Johan Marinus, Maria Fiorella Contarino

Summary: This study presents a case of refractory medication-induced tremor successfully treated with deep brain stimulation and proposes a treatment algorithm based on a systematic review of the literature. The results suggest that propranolol, tetrabenazine, and metoprolol may be effective treatments, and deep brain stimulation may be considered for selected and refractory cases.

Summary: FMD is a commonly under-recognized diagnosis that may occur more often in patients undergoing DBS, especially those with pre-morbid psychiatric conditions. The prevalence and number of cases varied across centers.

The effect of tremor on disability assessment in chronic inflammatory demyelinating polyradiculoneuropathy

Summary: This study aimed to investigate whether tremor in CIDP is associated with disability. The presence of a clinically relevant tremor and tremor severity were significantly associated with disability in CIDP patients, independent of other impairments. To assess disease activity in CIDP patients with tremor, both impairment and disability outcomes should be assessed, as disability is partly caused by tremor while the effect of immunotherapy on tremor seems limited.

A Screening Tool to Quickly Identify Movement Disorders in Patients with Inborn Errors of Metabolism

Lisette H. Koens, Marrit R. Klamer, Deborah A. Sival, Bettina Balint, Kailash P. Bhatia, Maria Fiorella Contarino, Martje E. van Egmond, Roberto Erro, Jennifer Friedman, Victor S. C. Fung, Christos Ganos, Manju A. Kurian, Anthony E. Lang, Eavan M. McGovern, Emmanuel Roze, Tom J. de Koning, Marina A. J. Tijssen

Summary: This article describes an easy-to-use clinical screening tool to help nonmovement disorder specialists recognize movement disorders in patients with inborn errors of metabolism (IEMs). Through scoring videos of 55 patients with different IEMs, informative tasks including walking, observations of the arms, and drawing a spiral were identified and incorporated into the screening tool. The study suggests that this tool can assist in selecting patients who should be referred to a movement disorder specialist for further evaluation and treatment.

Protocol of a randomized controlled trial investigating Deep Brain Stimulation for MOtor symptoms in patients with Parkinson's disease DEmentia (DBS-MODE)

V. Sisodia, B. E. K. S. Swinnen, J. M. Dijk, E. Verwijk, G. van Rooijen, A. W. Lemstra, P. R. Schuurman, R. M. A. de Bie

Summary: This clinical trial aims to determine the effectiveness and safety of deep brain stimulation (DBS) compared to best medical treatment (BMT) for patients with Parkinson's disease dementia (PDD). The trial will randomly assign 44 patients to either receive STN-DBS with BMT or BMT alone and assess their symptoms and cognitive abilities over a 30-week period, along with other secondary outcome measures.

Predicting Motor Outcome and Quality of Life After Subthalamic Deep Brain Stimulation for Parkinson's Disease: The Role of Standard Screening Measures and Wearable-Data

Summary: Standardized screening for STN DBS in Parkinson's disease patients is crucial to determine eligibility, but its utility to predict postoperative outcomes is inconclusive. This study aimed to evaluate the utility of universal components incorporated in the DBS screening, complemented by a wearable sensor, to predict motor outcomes and QoL one year after surgery. The results showed that preoperative tests, including wearable data, do not predict postoperative motor outcome, but post-DBS QoL is primarily driven by non-motor symptoms.

Summary: This article discusses the application of deep brain stimulation (DBS) in the treatment of movement disorders and psychiatric disorders. The current open-loop approach may limit the effectiveness of DBS, while the emerging closed-loop approach adapts stimulation parameters to disease symptoms and states, potentially paving the way for personalized treatment protocols.
