Summary: The TOLEDO study showed that apomorphine infusion is an effective and safe treatment for reducing motor fluctuations in Parkinson's disease patients. Long-term use of APO led to sustained reductions in OFF time and improvements in ON time without troublesome dyskinesia, allowing for significant decreases in oral PD medication.

Prioritizing rare CNVs that affect known disease genes or are associated with specific microdeletion/microduplication syndromes based on a well-established assessment standard. The study identified pathogenic CNVs in a small percentage of patients, highlighting the importance of combining CNV analysis with the assessment of single-nucleotide variants and small insertions/deletions for improved genetic diagnosis in dystonia.

Levels of most markers tested significantly differed between disease groups and healthy controls, with CSF NfL levels best discriminating between Parkinson's disease and multiple system atrophy.

Summary: The study aimed to assess the diagnostic value of CSF alpha-synuclein RT-QuIC quantitative parameters in regard to disease progression, stratification, and conversion in synucleinopathies. The results showed that alpha-synuclein RT-QuIC adds value in diagnosing Parkinson's disease and may provide a way to distinguish variations within Parkinson's disease phenotype. The assay also showed potential as an early biomarker detecting synucleinopathy in idiopathic REM sleep behaviour disorder patients prior to conversion.

Summary: This study examines the trajectories and dynamic interplay between cognitive functioning and depressive symptoms in patients with Parkinson's disease (PD) compared to healthy controls (HC) from an intraindividual perspective. The results indicate that PD patients have worse cognitive abilities compared to HC, and greater impairments in cognitive abilities are subsequently associated with increased depressive symptoms.

Summary: A systematic analysis of single items within the MDS-UPDRS scale revealed that higher scores for ADL, freezing, and rigidity at baseline were predictive of faster disease progression in both cohorts. Precision and predictability of the MDS-UPDRS were higher in single-center settings, highlighting the need for rigorous training and/or video documentation to improve its utility in multi-center cohorts, such as clinical trials.

Summary: Parkinson's disease is the neurological disorder with the highest increase in prevalence. The lack of precise diagnosis at early stages remains a challenge. Metabolomics has provided valuable insights into the molecular basis of PD and potential biomarkers for early detection and treatment efficacy. In this study, NMR was used to analyze serum samples from German PD patients, revealing more pronounced pathological characteristics in male patients and confirming altered levels of acetone and cholesterol. Additionally, stronger oxidative stress markers were detected.
