Summary: In this study, a high prevalence of polypharmacy and inappropriate prescribing was found in elderly individuals living with HIV. Common prescribing issues included incorrect dosage, lack of indication, and drugs not appropriate for the elderly. Risk factors for prescribing issues included polypharmacy, renal impairment, treatment with CNS-active drugs, and being female. Medication reconciliation and regular prescription reviews by experienced physicians are recommended to reduce polypharmacy and inappropriate prescribing in this vulnerable population.

Summary: With population ageing, the number of older people is increasing, leading to an increase in multimorbidity and polypharmacy. This commentary discusses the susceptibility of older people to drug-related problems (DRPs) and potentially inappropriate prescribing (IP), and proposes various strategies to identify and optimize drug therapy in this population. The challenges of evaluating the risks and benefits of pharmacotherapy in older people with multimorbidity and limited life expectancy are highlighted. The importance of comprehensive geriatric assessment and individualized approach to geriatric pharmacotherapy is emphasized.

Summary: Improper drug prescription is a major problem leading to drug-related harms and healthcare resource wastage. Through genomic profiling, we found that only 14.8% of Italian subjects had a fully normal cytochrome profile, while 26.5% had combined cytochrome dysfunction. The discussion focuses on the potential benefits of comprehensive pharmacogenomic testing in polytreated patients to improve drug selection.

Summary: Inappropriate prescribing in the elderly is a risk factor for adverse drug reactions, hospitalization, and mortality. This study aimed to examine and compare the prevalence of potentially inappropriate medications in nursing home residents and determine the prevalence of potential prescribing omissions. The results showed that benzodiazepines and proton pump inhibitors were the most frequent inappropriate medications.

Summary: Medication burden and polypharmacy are common issues in patients with multimorbidity. Deprescribing has emerged as an effective approach to manage polypharmacy and improve health outcomes. However, traditional strategies like drug review and medication therapy management still exist, and there is an overlap between deprescribing and these strategies. This review explores the differences and similarities between deprescribing and other prominent polypharmacy management interventions.

Summary: This study evaluated changes in prescribing, potentially inappropriate prescriptions, and prescribing of low-value medicines in older people with multimorbidity and significant polypharmacy. The results showed reductions in the prescription of most drug groups, with the largest reduction observed in antiplatelet prescriptions. The median number of medication changes per person was five, highlighting the complexity of prescribing for patients with polypharmacy. Frequent medication changes could have important implications for patients in terms of adherence and medication safety.
