Variants in the promoter region of IKBL/NFKBIL1 gene may mark susceptibility to the development of chronic Chagas' cardiomyopathy among Trypanosoma cruzi-infected individuals

Chagas' disease, caused by Trypanosoma cruzi, is an inflammatory disorder leading to chronic Chagas cardiomyopathy (CCC). Only one third of T cruzi-infected individuals progress to CCC while the others are considered asymptomatic (ASY). The human inhibitory kappa B-like gene (KBLINFKBIL1), homologous to the I kappa B family of proteins that regulate the NF kappa B family of transcription factors, is suggested as a putative inhibitor of NFKB. We investigated two functional polymorphisms, -62A/T and -262A/G, in the promoter of IKBL by PCR-RFLP analysis in 169 patients with CCC and 76 ASY. Genotype distributions for both -62A/T and -262A/G differed between the CCC and ASY (X-2 = 7.3; P = 0.025 and X-2 = 6.8; P = 0.03, respectively). Subjects, homozygous for the -62A allele, had three-fold risk of developing CCC compared with those carrying the TT genotype (P = 0.0095; Odds Ratio [OR] = 2.9; [95% CI 1.2-7.3]). Similar trend was observed for the -262A homozygotes (P = 0.005; OR = 2.7 [95% CI 1.3-6.0]. The haplotype -262A -62A was prevalent in patients with CCC (40% versus 24%; OR 2.1 [95% C1 1.4-3.3j; Pc = 0.00 14). The I kappa BL locus itself or another critical gene in this region may confer susceptibility to the development of CCC. (C) 2007 Elsevier Ltd. All rights reserved.

Isolation and molecular characterization of circulating extracellular vesicles from blood of chronic Chagas disease patients

Summary: Extracellular vesicles (EVs) are lipid bilayer envelopes that encase several types of molecules. They play a role in intercellular communication and can be used as biomarkers for disease detection. This study investigated the effect of different anticoagulants and purification methods on the isolation and characterization of EVs from the blood of chronic Chagas disease (CCD) patients. The results showed that anticoagulants did not interfere with the analyzed parameters and circulating EVs from CCD patients contained T. cruzi antigens and classical human exosomal markers. This protocol provides a basis for further biomarker discovery in Chagas' disease.

Summary: In this study, the genetic and epigenetic deregulations associated with moderate and severe stages of Chagas disease were investigated using bulk RNA-seq and DNA methylation analysis. The results revealed differentially expressed transcripts and regulatory differentially methylated regions in the heart tissue of CCC patients. Transcription factors binding sites affected by DNA methylation were also identified. The study identified biological processes affected by severe CCC and highlighted the importance of ion transport and the nervous system in the development of the disease.

Anti-Trypanosoma cruzi antibody profiling in patients with Chagas disease treated with benznidazole assessed by genome phage display

Summary: This study used the gPhage platform to compare antigens and antibodies before and after treatment in 20 patients with chronic Chagas disease. The results showed a decrease in antibody levels and recognition of specific antigens after treatment. The gPhage data correlated with patient PCR-status and could predict patient response to treatment. In conclusion, the use of the gPhage platform contributes to the development of novel diagnostic tools for Chagas disease.

Detection of Early Diffuse Myocardial Fibrosis and Inflammation in Chagas Cardiomyopathy with T1 Mapping and Extracellular Volume

Summary: The purpose of this study is to evaluate myocardial T1 mapping and extracellular volume (ECV) parameters in different stages of Chagas cardiomyopathy and determine their predictive value for disease severity and prognosis. The results showed that myocardial native T1 and ECV values were correlated with disease severity, indicating that they could serve as markers of myocardial involvement in Chagas cardiomyopathy. The study also found that abnormal remote ECV values increased with disease severity. Overall, the findings suggest the potential importance of T1 mapping and ECV parameters in assessing and predicting Chagas cardiomyopathy.
