A proteomic profile of washing fluid from the colorectal tract to search for potential biomarkers of colon cancer

Washing fluid (WF) from the colon rectal tract after surgical resection might represent a first step in obtaining a mixture of proteins derived from the secretion of tumoral epithelial cells potentially involved in the pathological progression of tissue. In this study, we performed a proteomic analysis of colorectal WF to search for potential biomarkers of colon cancer. The outcome of this approach might open the possibility of using WF to screen for the precancerous and early stages of colorectal cancer (CRC). Samples of WFs were obtained during surgery from 35 patients submitted to colon resection for suspicious adenocarcinoma or carcinoma, while the respective controls were obtained by washing the healthy sections. WFs were immediately centrifuged, concentrated and trichloroacetic acid (TCA) was added to obtain protein pellets. After two-dimensional gel electrophoresis (2DE), the protein patterns of malignant samples were compared with respective normal samples. Forty-one protein spots were found to be differentially expressed exhibiting >= fold-change of mean value spot intensities. After mass spectrometry, these protein spots collapsed into 38 different proteins. Interestingly, 19 of the differentially expressed proteins identified in the study corresponded to those suggested as being potential biomarkers of CRC. In accordance with the literature, these proteins showed the same direction of change (up or down for all proteins). Our results suggest that WF has the potential of being a method for the exploration of clinical samples for biomarker and drug target discovery.

Percutaneous cholecystostomy as bridge to surgery vs surgery in unfit patients with acute calculous cholecystitis: A systematic review and meta-analysis

Summary: Acute cholecystitis is a common cause of acute abdomen, and early laparoscopic cholecystectomy is the standard treatment. However, there is a risk of intraoperative biliary duct injury. Percutaneous gallbladder drainage (PGBD) is an alternative strategy to manage severe acute cholecystitis. Studies show that PGBD combined with laparoscopic cholecystectomy can reduce post-operative complications and the risk of biliary leakage.

Ultrasound can be helpful in ruling out tall cell variant papillary thyroid carcinoma.

Colorectal neoplastic emergencies in immunocompromised patients: preliminary result from the Web-based International Register of Emergency Surgery and Trauma (WIRES-T trial)

Summary: The study presents preliminary data on colorectal neoplastic emergencies in immunocompromised patients. Out of the 839 patients collected, 80.7% had mild-moderate immunocompromission and 10.3% had severe immunocompromission. The study found that the only factor influencing mortality in both groups was the ASA score. The study highlights the need for dedicated emergency surgery units to reduce disease burden and improve outcomes.

Summary: The identification of high-risk patients is crucial in the early stages of infected pancreatic necrosis (IPN) to determine appropriate management strategies. This study found that history of uncontrolled arterial hypertension, qSOFA score, renal failure, haemodynamic failure were independent predictors of mortality in IPN patients. Cholangitis, abdominal compartment syndrome, and gastrointestinal/intra-abdominal bleeding were associated with increased risk of mortality. Upfront open surgical necrosectomy was strongly associated with higher mortality risk, while endoscopic drainage of pancreatic necrosis and enteral nutrition were protective factors.

Summary: A placebo-controlled study was conducted on 16 show jumping horses to evaluate the effects of a dietary supplement containing Verbascum thapsus, Curcuma longa, and Boswellia serrata extracts. The supplementation led to modulation of proteins and downregulation of proinflammatory cytokines in the horses. The study suggests that this phytotherapy can strengthen the innate immune responses and alleviate inflammation in sport horses.

Summary: This systematic review compares percutaneous transhepatic gallbladder biliary drainage (PTGBD) with emergency cholecystectomy (EC) as definitive treatments for high-risk patients with acute cholecystitis (AC). The analysis suggests that PTGBD is not associated with lower morbidity compared to EC. EC is associated with lower postoperative mortality, reduced risk of hospital readmission for biliary complications, and shorter hospital stay. Therefore, EC may be the preferred treatment for high-risk patients.

Biological prosthesis, platelet enriched plasma and bone marrow stem cells in complicated incisional hernia reconstruction in emergency surgery: a prospective case control study

Summary: Numerous innovations in abdominal wall surgical repair have been aided by synthetic materials and tissue engineering. This study compares different approaches in repairing complex incisional hernia in an emergency setting. Results show that direct reconstruction of the abdominal wall experienced higher rates of severe complications and recurrence compared to reconstruction with biological mesh and platelet enriched plasma (PEP). Tissue engineering techniques show promising results in reducing recurrence rates without increasing complications in complicated incisional abdominal wall hernia. Continued research and experimentation in this field is extremely important.

The 5th edition of WHO classification of tumors of endocrine organs: changes in the diagnosis of follicular-derived thyroid carcinoma

Summary: The 5th edition of the WHO classification of endocrine tumors, released in 2022, introduces several novelties in the nomenclature and histopathological diagnosis of follicular-derived thyroid neoplasms. Tumor types are now categorized into benign tumors, low-risk neoplasms, and malignant neoplasms based on prognostic risk. A grading system for differentiated thyroid carcinomas has been implemented for better tumor stratification. The review critically discusses the practical effects on diagnostic pathology and the expected clinical implications of the new classification scheme.
