The rapid efficacy of abatacept in a patient with rheumatoid vasculitis

We report a case of rheumatoid vasculitis (RV) that responded well to abatacept, a cytotoxic T lymphocyte-associated antigen 4 (CTLA4)-immunoglobulin fusion protein. A 38-year-old woman developed RV despite treatment with methotrexate and tumor necrosis factor (TNF) inhibitors. The effects of steroid therapy, immunoabsorption plasmapheresis, and interleukin-6 inhibitor were insufficient, however, administration of abatacept rapidly improved her clinical symptoms with almost normalization of the immunological findings. This is the first published case report of the successful treatment of RV with abatacept.

Summary: This study examined the risk factors of surgical site infection, delayed wound healing, and death after orthopedic surgery in patients with rheumatoid arthritis. The use of biological disease modifying antirheumatic drugs was identified as a risk factor for surgical site infection, while rheumatoid arthritis itself, oral glucocorticoid use, age, male sex, comorbidities such as diabetes mellitus and chronic obstructive pulmonary disease, surgical factors, and longer operative time were all found to be risk factors for postoperative complications. Patients with these factors should be carefully monitored to ensure a balance between risks and benefits of orthopedic surgeries.

Summary: Despite the high prevalence of COPD, the cellular and molecular mechanisms of the disease remain poorly understood. This study investigates the changes in cellular and molecular compositions in the alveolar space and peripheral blood of COPD patients compared to controls. The findings suggest that myeloid cells, including invading monocytes and proliferating macrophages, are increased in COPD. Computational modeling predicts TGF-beta 1 as a major regulator of transcriptional changes in alveolar macrophages of COPD patients. Functionally, macrophages in COPD exhibit impaired immune activation.

The study found that AIT is a safe and effective treatment modality for patients with allergic rhinitis complicated by RADs.

Summary: Systemic inflammation is a characteristic feature of late-stage severe lung disease, while the molecular, functional, and phenotypic changes in peripheral immune cells in the early stages of the disease remain poorly understood. In this study, using single-cell analyses, the researchers found that blood neutrophils are already increased in early-stage COPD, and the changes in their molecular and functional states correlate with the decline of lung function. Further investigation using a murine cigarette smoke exposure model revealed similar molecular changes in blood neutrophils and precursor populations in the blood and lung. The findings suggest that systemic molecular alterations in neutrophils and their precursors are present in the early stages of COPD, and may serve as potential therapeutic targets and biomarkers for early diagnosis and patient stratification.

Cervical lymphadenitis caused by Mycobacterium avium in a patient treated with Janus kinase inhibitors

Summary: A female patient with rheumatoid arthritis treated with JAK inhibitors developed gradually enlarging bilateral submandibular lymph nodes swelling. Lymph node biopsy revealed granulomatous lymphadenitis with caseous necrosis, and Mycobacterium avium was identified. A CT scan showed no evidence of mass or infection elsewhere, and the mass was excised without antimicrobial treatment. No recurrence was observed 9 months after excision. Physicians should be aware of rare complications such as cervical lymphadenitis caused by nontuberculous mycobacteria when using JAK inhibitors.

Identification and management of connective tissue disease-associated interstitial lung disease: evidence-based Japanese consensus statements

Summary: The study aimed to develop expert consensus statements and an algorithm for managing connective tissue disease-associated interstitial lung disease (CTD-ILD). Consensus was reached on important statements regarding screening, diagnosis and severity assessment, follow-up, and multidisciplinary management of CTD-ILD. An algorithm was developed based on these consensus statements.
