Association between muscle strength and metabolic syndrome in older Korean men and women: the Korean Longitudinal Study on Health and Aging

The objective of the study was to investigate the association between metabolic syndrome (MS) and muscle strength in community-dwelling older men and women in Korea. Korean men and women 65 years and older living in a single, typical South Korean city (n = 647) were enrolled in the Korean Longitudinal Study on Health and Aging study. The diagnosis of MS was evaluated according to the definition of the National Cholesterol Education Program Adult Treatment Panel III. Isokinetic muscle strength of the knee extensors, as determined by peak torque per body weight (newton meter per kilogram) and hand-grip strength per body weight (newton per kilogram), was measured. Participants without MS had greater leg muscle strength and grip strength per weight. The effect of MS on muscle strength was more prominent in men than in women in our study population. Only men showed a significant interaction between MS and age for muscle strength (P = .014), and the effect was greater in men aged 65 to 74 years compared with those older than 75 years (119.2 +/- 31.2 vs 134.5 +/- 24.3 N m/kg). Participants with MS had weaker knee extensor strength after controlling the covariates (beta = -90.80, P = .003), and the interaction term (age x MS x male sex) was significant (beta = 1.00, P = .017). Metabolic syndrome is associated with muscle weakness, and this relationship is particularly pronounced in men. Age can modify the impact of MS on muscle strength. Men aged 65 to 74 years with MS need a thorough assessment of muscle strength to prevent disability. (C) 2012 Elsevier Inc. All rights reserved.

Evidence of risky driving in Korean older adults: A longitudinal cohort

Summary: This study aimed to determine the differences in risk factors for dangerous driving between older adults with normal cognition and those with cognitive impairment. The driving risk questionnaire (DRQ) and accident/violation records were analyzed. Significant associations were found between risky driving and crash history, safety concern, reduced mileage, and aggressive driving in the normal cognition group. In the cognitive impairment group, only crash history was significantly associated. These findings contribute to evaluating and predicting risky driving and providing advice on whether to continue driving in clinics.

Real-Time Interactive Digital Health Care System for Postoperative Breast Cancer Patients: A Randomized Controlled Trial

Effects of Initial Combinations of Gemigliptin Plus Metformin Compared with Glimepiride Plus Metformin on Gut Microbiota and Glucose Regulation in Obese Patients with Type 2 Diabetes: The INTESTINE Study

Summary: This study aimed to compare the effects of gemigliptin-metformin and glimepiride-metformin combination therapies on gut microbiota and glucose homeostasis. The results showed that gemigliptin-metformin achieved better glycemic control without hypoglycemia or weight gain compared to glimepiride-metformin. Changes in gut microbiota were associated with treatment outcomes.

Association of estimated white matter hyperintensity age with cognition in elderly with controlled hypertension

Summary: This study investigated the effects of controlled hypertension (cHT) and white matter hyperintensity (WMH) on cognitive function, as well as the mediating role of WMH in the relationship between cHT and cognitive impairment. The results showed that older WMH age was associated with a faster decline in cognitive function, and WMH age mediated the effects of cHT on cognitive function. Therefore, older individuals with cHT and older WMH age may be at a higher risk of cognitive decline.

Effect of Lactobacillus plantarum LMT1-48 on Body Fat in Overweight Subjects: A Randomized, Double- Blind, Placebo-Controlled Trial

Summary: This study investigated the potential use of Lactobacillus plantarum strain LMT1-48 as a probiotic supplement for overweight individuals. A randomized, double-blind, placebo-controlled clinical trial was conducted on 100 volunteers with a body mass index (BMI) of 25 to 30 kg/m2. The results showed that administration of LMT1-48 significantly reduced body weight, abdominal visceral fat area, insulin resistance, and leptin levels.

Vascular and metabolic effects of ipragliflozin versus sitagliptin (IVS) in type 2 diabetes treated with sulphonylurea and metformin: IVS study

Summary: In patients with inadequately controlled type 2 diabetes treated with metformin and sulphonylurea, ipragliflozin showed better glycemic control and metabolic benefits compared to sitagliptin. Ipragliflozin treatment resulted in a significant decrease in glycated hemoglobin levels, improved glucose and fat metabolism, and enhanced vascular function. Both ipragliflozin and sitagliptin were found to be safe.

Comparison of the effects of gemigliptin versus glimepiride on cardiac function in patients with type 2 diabetes uncontrolled with metformin: The gemi-heart study

Summary: The aim of this study was to investigate the effects of gemigliptin on cardiac function and compare its effects with glimepiride in patients with type 2 diabetes. The results showed that gemigliptin and glimepiride both significantly reduced HbA1c levels without a significant difference between the groups. Gemigliptin also had positive effects on insulin resistance, inflammatory markers, lipid profiles, and body composition. No significant changes in cardiac biomarkers were observed. The study received a score of 8 out of 10.

Impacts of whole-body vibration on denervated skeletal-muscle atrophy in rats

Summary: Whole-body vibration was evaluated for its effects on denervated skeletal muscle atrophy. The results showed that whole-body vibration led to a decrease in inclination angle and muscle weight, but not muscle fiber cross-sectional area. Changes in myosin heavy chain isoform composition were observed in denervated muscle after whole-body vibration. However, there were no significant changes in denervated slow-twitch muscle.

Factors predictive of functional outcomes and quality of life in patients with fragility hip fracture: A retrospective cohort study

Summary: This is a retrospective cohort study that aimed to determine the predictors of functional outcomes and quality of life for patients who underwent surgery for fragility hip fracture. The study was conducted in three tertiary rehabilitation facilities and followed up with 165 patients for six months postoperatively. The study found that age, fracture site, operation type, comorbidities, and initial physical and cognitive function significantly influenced the recovery of function and quality of life at six months.

Summary: The study aimed to investigate the effectiveness, safety, optimal starting dose, optimal maintenance dose range, and target fasting plasma glucose of five basal insulins in insulin-naive patients with type 2 diabetes mellitus. Meta-analyses and network meta-analyses showed that glargine U-300 or degludec U-100 might be the most effective choices for injecting basal insulin regimens at bedtime, followed by glargine U-100 or detemir, and NPH. Injecting glargine U-100 in the morning may be more effective and result in fewer hypoglycemic events. The optimal starting dose for any basal insulin initiation can be 0.10-0.20 U/kg/day. There is no eligible evidence to determine the optimal maintenance dose for basal insulins.

The guidelines received high agreement rates, and strong and conditional recommendations were made. The recommendations and clinical thresholds set in the guidelines can be valuable references for clinicians worldwide.

A Systematic Review and Meta-Analysis of the Effects of Rehabilitation Using Digital Healthcare on Musculoskeletal Pain and Quality of Life

Summary: Rehabilitation using digital healthcare (DHC) shows potential in enhancing treatment effectiveness for musculoskeletal disorders (MSDs) and associated pain. This systematic review and meta-analysis evaluated the effectiveness of DHC in musculoskeletal rehabilitation. The findings indicate that DHC offers a practical and flexible alternative, with greater pain reduction and improved quality of life compared to conventional rehabilitation. Further research is needed to understand the underlying mechanisms and variations in patient-reported outcomes.

Dysphagia Requiring Medical Attention in Parkinson's Disease: A Korean Population-Based Study

Summary: This study analyzed the prevalence and risk of Parkinson's disease (PD) and dysphagia in Korea from 2006 to 2015. The study found that both the prevalence of PD and dysphagia in PD patients increased continuously during the study period, with the highest prevalence among elderly individuals. Patients with PD had a three times higher risk of dysphagia compared to those without PD, emphasizing the importance of attention to this condition.

Digital Biomarkers for Diagnosis of Muscle Disorders Using Stimulated Muscle Contraction Signal

Summary: We propose a digital biomarker (DB/MS and DB/ME) for diagnosing muscle disorders using a multi-layer perceptron (MLP) and stimulated muscle contraction. Traditional methods of measuring muscle strength and endurance related DBs are difficult and expensive. We introduce a non-volitional DB measurement technique using impact response signal (IRS) based on multi-frequency electrical stimulation (MFES) and an electromyography sensor. The DB estimation model learned through MLP accurately assesses muscle disorders causing decreased physical performance.

Glucocorticoids and intrauterine programming of nonalcoholic fatty liver disease

Summary: Accumulating evidence suggests that NAFLD has an intrauterine origin, with adverse prenatal environments and glucocorticoid exposure playing a crucial role in the developmental programming of fetal hepatic lipid metabolism. The offspring's glucocorticoid-insulin-like growth factor 1 (GC-IGF1) axis is programmed in utero, leading to postnatal catch-up growth and disrupted glucose and lipid metabolism, increasing susceptibility to NAFLD. Mismatch between intrauterine and postnatal environments can further disturb the programmed endocrine axes and accelerate the onset of NAFLD.

CCDC92 deficiency ameliorates podocyte lipotoxicity in diabetic kidney disease

Summary: This study investigates the role of CCDC92 in the pathogenesis of diabetic kidney disease (DKD). The expression of CCDC92 was found to increase in kidney biopsies from patients with DKD and was correlated with glomerular lipid accumulation. Animal studies further confirmed the induction of CCDC92 in the kidney, particularly in podocytes, and the podocyte-specific deletion of Ccdc92 ameliorated podocyte injury and lipid deposition. CCDC92 was shown to promote podocyte lipotoxicity through ABCA1 signaling-mediated lipid homeostasis. Therefore, CCDC92 may serve as a potential biomarker of podocyte injury in DKD and targeting CCDC92 could be an innovative therapeutic strategy for DKD patients.

Brown adipose tissue-derived metabolites and their role in regulating metabolism

Summary: The discovery and revival of brown adipose tissue (BAT) in adult humans have opened up new possibilities for treating obesity and metabolic diseases. BAT not only plays a role in generating heat, but also secretes signaling molecules known as batokines, which regulate overall metabolism. This review highlights the importance of BAT-derived metabolites in controlling thermogenesis, substrate metabolism, and other biological processes, as well as their potential to alleviate obesity and related metabolic complications.
