Summary: Abnormal endothelial function may be associated with adverse 5-year outcomes in patients presenting to a chest pain unit (CPU). The study found that lower baseline RHI was an independent predictor of 5-year major adverse cardiovascular events (MACE).

Summary: The study found no significant differences in severity of chest pain causes, ED work-up, or hospital admission between patients who sought help through a Nurse Line (NL) and those who went directly to the ED. Patients who presented to the ED without calling a NL typically had more comorbidities.

Summary: The study compares the TIMI score with the T-MACS score and concludes that the T-MACS score is more successful in determining low risk, high risk, and estimated 1-month MACE risk in patients with chest pain in the emergency department.

Summary: Acute myocardial infarctions are becoming more common in young adults. This study investigates the sex and racial differences in chest pain evaluation among young adults presenting to the emergency department. The findings reveal that women and people of color with chest pain had longer waiting times, and women were less likely to be admitted. These differences could have an impact on treatment and outcomes.

Summary: This study aims to describe patterns in the evaluation, treatment, and outcomes of patients presenting to an urban, academic emergency department (ED) for atraumatic low back pain (LBP). The results show that patients receiving ED care for LBP had a significant improvement in physical function and mobility scores at 6 weeks, but their return to function continued to lag behind. There were variations in imaging patterns, medication prescriptions, and outpatient follow-up, highlighting the need for clear guidelines and treatment pathways for ED patients with LBP.

Summary: As physicians aim to reduce ionizing radiation by utilizing Magnetic Resonance Imaging (MRI), it is important to be aware of the specific risks associated with MRI, such as thermal burns. Obese patients are particularly susceptible to MRI-related thermal burns, as demonstrated by a case of a 45-year-old female who experienced a delayed, occult full-thickness burn after a diagnostic MRI. Emergency physicians should be knowledgeable about the risks of MRI and able to properly counsel patients before and after the procedure.

Summary: This guideline provides evidence-based recommendations for adult patients with recurrent, low-risk chest pain in the emergency department, including the use of high-sensitivity troponin for excluding acute coronary syndrome, the recommendation against routine stress testing, and the lack of evidence supporting hospitalization versus discharge for mitigating major adverse cardiac events within 30 days.

Summary: This study aimed to evaluate the impact of cardiological and echocardiographic evaluation on diagnostic and prognostic accuracy in patients with chest pain. The results showed that early consultation with a cardiologist and echocardiography can improve clinical judgment and increase diagnostic performance.

Summary: Indigenous Australians with chest pain present at a younger age and have more severe coronary disease and higher rates of coronary bypass graft surgery. There is a need to improve support structures for Indigenous patients in the emergency department. Indigenous patients have a significantly higher risk of major adverse cardiac events (MACE) compared to non-Indigenous patients, indicating the need for collaborative efforts to improve cardiovascular health in the local Aboriginal and Torres Strait Islander population.

Summary: Abdominal pain is a common chief complaint among older adults in US emergency departments. Older patients with abdominal pain are less likely to be triaged as emergent but more likely to be admitted directly to the operating room compared to those without abdominal pain. Around 60% of older adults with abdominal pain undergo ultrasound or CT imaging, while only 39% receive an electrocardiogram.

Summary: This systematic review examined the application of machine learning in risk stratification for adults presenting to the emergency department with undifferentiated chest pain. The study found that ML models outperformed physicians and current risk stratification tools in diagnosing acute myocardial infarction and predicting major adverse cardiovascular events. However, integration of ML models into clinical practice remains limited.

Summary: In this study, the authors investigated the trend in frequency of chest pain patients brought to hospital by ambulance and the subsequent diagnosis of acute myocardial infarction (AMI). They found that the frequency of chest pain patients and AMI increased over the study period, while the proportion of AMI patients remained stable. The overall mortality in chest pain patients was low, suggesting that monitoring the number of chest pain patients and AMI diagnoses can help evaluate ambulance utilization and triage.

Summary: This study found that switching from low-deductible health plans to high-deductible plans mandated by employers was associated with decreased ED visits and hospitalization for nonspecific chest pain, but did not significantly affect post-ED cardiac testing. However, enrollment in high-deductible plans was linked to an increase in 30-day acute myocardial infarction admissions after ED diagnosis of nonspecific chest pain, especially among members from higher-poverty neighborhoods.

Summary: This prospective cohort study aimed to determine the predictors of persistent low back pain and length of sick leave in patients with acute symptoms visiting an emergency department. Lower self-rated health predicted pain after three months, while longer sick leave was predicted by distraction coping strategy and decreased behavioral activity.

Summary: Diabetic patients with chronic total occlusion (CTO) have higher rates of multivessel coronary artery disease. Surgical revascularization and successful percutaneous coronary intervention (PCI) yield better outcomes than medical therapy in these patients, with coronary artery bypass graft surgery showing superior results compared to PCI. Improvements in CTO revascularization techniques and outcomes may help bridge the gap in outcomes for diabetic patients.
