Inflammation and Chronic Heart Failure: From Biomarkers to Novel Anti-inflammatory Therapeutic Strategies

Heart failure (HF) is a complex heterogeneous syndrome with immune, metabolic and neurohumoral mechanisms interacting and leading to gradual heart contractility impairment. From the first study-to correlate inflammation with HF, inflammation biomarkers have been the subject of intense inquiry in patients with various forms of HF. Chronic HF (CHF) is strongly associated with inflammation in terms of pathogenesis, progression, severity and prognosis. Inflammatory mediators participate in CHF pathophysiology in various ways like exerting direct impact on cardiac myocytes, fibroblasts and beta-adrenergic receptors leading to hypertrophy, fibrosis and impaired cardiac contractility, respectively, or inducing apoptosis by stimulation of the proper genes. The anti-inflammatory effects of classical heart failure therapeutic strategies such as ACEI and b-blockers are rather conflicting. Whether novel immunomodulating and anti-inflammatory therapeutic approaches should be added to existing therapies in order to ensure additional benefit to HF patients is under investigation. In this review, we summarize the pathophysiological link between inflammatory processes and CHF, focusing on the role of novel and traditional inflammatory biomarkers and highlighting novel anti-inflammatory therapeutic strategies.

Summary: Left atrial peak systolic longitudinal strain (LA-PLSsys) is considered a useful predictor for post-ablation atrial fibrillation (AF) recurrence, but substantial heterogeneity was noted in related studies; the study indicated that LA-PLSsys was negatively correlated with baseline LA volume index, and further uniform methodologies are needed to determine the reference range for LA-PLSsys.

Different venous approaches for implantation of cardiac electronic devices. A network meta-analysis

Summary: In this study, the safety and efficacy of cephalic vein cutdown (CVC), subclavian vein puncture (SVP), and axillary vein puncture (AVP) for cardiac device implantation were compared. The results showed that both CVC and AVP were associated with lower incidence of pneumothorax and lead failure compared to SVP. There were no significant differences between AVP and CVC. Limited data suggests that different approaches did not have a major impact on infectious and bleeding complications. The initial use of CVC required more frequent alternate/additional venous access compared to AVP and SVP. Further clinical testing is suggested for AVP based on these results.

Weekly physical activity and incident atrial fibrillation in females - A dose-response meta-analysis

Summary: This study found a nonlinear inverse relationship between weekly physical activity (PA) and the risk for atrial fibrillation (AF) in females. The risk of AF decreases as PA levels increase up to 50 metabolic equivalents of task-hours per week (METs-h/w). This study provides recommendations for PA dose to reduce the risk of AF in females.

Summary: This meta-analysis examined the value of echocardiographic indices of RV function as potential predictors of CRT outcomes in patients with standard indications for CRT. The results showed that baseline tricuspid annular plane systolic excursion was consistently higher in CRT responders, suggesting its potential as an additional component in the selection process of CRT candidates.

P-wave Indices as Predictors of Atrial Fibrillation: The Lion from a Claw

Summary: The P wave contains information about atrial electrical activity, but technical and biological reasons make it challenging to gather and utilize this information. However, simple ECG descriptors like P-wave dispersion have predictive value for assessing the risk of atrial fibrillation. More complex analysis methods and machine learning techniques may improve the accuracy and reliability of obtaining clinically useful information from the P wave.

Left Distal Radial Artery Access for Coronary Angiography and Interventions: A 12-Month All-Comers Study
